2019 was yet another breakthrough year for Changi General Hospital (CGH) Sport and Exercise Medicine (SEM) as we expanded our footprint to make CGH SEM services more accessible to the public. Fulfilling our National Role to improve patients’ lives, we launched our latest clinic, SSMC@SSI, located at Singapore Sports Hub, to provide specialised care to elite Team Singapore athletes. Additionally, leveraging on SingHealth’s network of acute and community hospitals, we set up two SEM services in collaboration with Outram Community Hospital (OCH) and Sengkang General Hospital (SKH). These SEM services draw on the extensive SingHealth platform to integrate SEM into the treatment paradigms of various patient segments.

Education remains a priority for CGH SEM. In 2019, we launched the inaugural Graduate Diploma in Sports Medicine (GDSM) and saw the graduation of the first cohort. The number of places for the second intake was increased from 17 to 25 as the course was oversubscribed by 52%. Students gave feedback that the course was comprehensive and the knowledge and skills acquired were applicable to their daily practice.

As a training institution, SSMC at Novena and SSI are both now fully accredited for SEM sub-specialty training, allowing Sports Medicine Advanced Specialist Trainees posted to CGH SEM to benefit from all three SSMCs through cross-institution training and exposure. The Joint Committee on Specialist Training (JSCT) had also approved the Adjunct Associate Professor accreditation of additional supervisors, from four to six. We are currently the largest Sports Medicine Sub-specialty training institution and account for 75% of trainees in Benedict Tan Singapore. The above developments underscore our position as the SEM training Chief, CGH Sport & Exercise Medicine institution of choice. SSMC@CGH is the first accredited centre for 1 Sports Medicine Advanced Specialist Training (SpMed ASTs) in Singapore.

The SEM department has the largest number of 6 accredited supervisors and training positions for 6 SpMed AST. We currently account for 75% of the trainees in Singapore.

Since 2012, SSMC@CGH has trained over 30 30 medical officers.

EXPANSION OF SEM ACROSS SINGHEALTH CLUSTER TO SGH, SKH AND KKH

SingHealth Duke-NUS Sport and Exercise Medicine Centre was formed in 2018 with an aim to optimise cluster-wide resources and talent to provide seamless and accessible patient-centric care. The operations of 3 SEM centres – SSMC@CGH, SSMC@Novena, SSMC@SSI, and 3 SEM services – SGH, SKH and upcoming KKH come with different sets of needs and will require excellent coordination within the cluster. By expanding SEM across SingHealth, we increase access while optimising resources. We also attract and retain SEM talent by offering them the best training and working environment, as well as plentiful opportunities to match our clinicians’ career aspirations.

The proposal for expansion of SEM across the cluster via central employment of SEM Advanced Specialist Trainees (AST)s and physicians by SingHealth, and hosting of these staff at CGH SEM Department, was approved at the SingHealth Medical Board meeting on 26 July 2019. This is supplemented by the Resident Physician (SEM) Scheme, where RPs (SEM) are employed by each institution and trained via a cluster-harmonised training programme. The centrally-employed Sports Physicians, together with the institution-employed RPs (SEM), allow for organic growth and development at each site while ensuring efficient resourcing, quality assurance, and harmonisation at Cluster level. 09 EXPANDING OUR FOOTPRINT

SSMC@SSI

Located at Singapore Sports Hub, SSMC@SSI started operations on 1 September 2019 to provide our national athletes with the best in Sport & Exercise Medicine that Singapore can offer. The launch of SSMC@SSI added almost 6,700 sqft. of floor area and 15 staff to the Department, and now serves Team Singapore athletes with a daily patient load of 50. Our team is committed to working with the Singapore Sports Institute and its Sports Scientists to deliver an integrated Sports Medicine and Sports Science platform to Team Singapore athletes, enabling them achieve their full potential in their sporting careers.

SEM SERVICES @ OCH, SKH

Accessibility to SEM services remains a key goal, as a means of fostering a physically active and resilient population. With this in mind, CGH SEM launched two SEM services at OCH and SKH for those living in the respective neighbourhoods. Dr Lim Ang Tee was appointed Director of SEM Services to oversee the two services at OCH and SKH, as well as the upcoming SEM service at KKH. 10 MAJOR GAMES & SPORTING EVENTS

SUPPORTING OUR NATIONAL ATHLETES AT THE 2019 SOUTHEAST ASIAN GAMES

The 2019 Southeast Asian Games (SEA Games) took place in the Philippines from 30 November to 11 December 2019. As in previous years, CGH SEM continued to play an essential role in providing medical support to our national athletes at major games. Drs Benedict Tan, Kelvin Chew, Mandy Zhang and Victor Tan took on the responsibility as medical cover for Team Singapore at the 2019 SEA Games.

SPORTS SAFETY DAY 2019

Sports Safety is integral in the promotion of a sporting culture among Singaporeans. Formed in July 2007, the Sports Safety Community is co-chaired by Adj A/Prof Benedict Tan and Mr Muhammad Rostam Bin Umar, Chief Strategy Group, SportSG. The 2019 Sport Safety Working committee members, including Drs Ivy Lim, Lim Ang Tee and Mandy Zhang, had published the third edition of Sports Safety Committee Report – a comprehensive report on sports safety standards and systems to be in line with international best practices to improve safety measures and precautions at sports events and facilities, which are practical and applicable to the Singapore sports context. Traditional sprint interval training (SIT) is known to be strenuous and requires high level of self-motivation to complete it successfully. For SIT to be more appealing, the team proposed an increase in the rest period between each sprint bout, termed as ‘dispersed’ protocol. For the same number of sprint bouts, the ‘dispersed’ protocol would require half the amount of time to complete compared to traditional SIT.

Conclusion: The ‘dispersed’ protocol training regimen is a time-effective strategy that significantly improves cardiorespiratory fitness. It is specifically designed around a typical office worker’s schedule and eliminates the barrier of lack of time for exercise.

The primary aim of the research is to determine if there is a

correlation between knee osteoarthritis (OA) compartment and improvement in pain score using Visual Analogue Score (VAS) post viscosupplementation. The secondary aim is to evaluate which OA grading, based on Kellgren-Lawrence grading, had the greatest response to viscosupplementation.

Conclusion: Viscosupplementation outcomes vary. In this series, 51% of patients had successful response to viscosupplementation therapy. Tricompartment and lateral compartment OA were more likely to have significant clinical response following viscosupplementation, compared to other compartments. No previous studies have investigated this effect and the reasons are unclear. 14 EDUCATION

10th ANNUAL EIMS WORLD CONGRESS

The 10th Annual EIMS World Congress, in conjunction with the 66th American College of Sport Medicine (ACSM) Annual Meeting was held in Orlando, Florida on 28 May to 1 June 2019. Led by Dr Ivy Lim and Dr Fadzil Hamzah, the Singapore team had the pleasure of meeting up with leaders from various EIM National Centres during the National Centre Director’s meeting. Healthcare providers often focus on interventions aimed at improving healthcare delivery to patients. However, it will be more cost effective and sustainable if we focus our efforts in nurturing a healthy nation. During the ACSM Annual Meeting, Dr Fadzil presented a poster titled “Moving Beyond Healthcare to Health: A Preliminary and Descriptive Statistical Report on the Health Peers Programme in the War on Diabetes In Singapore”. In his poster presented to an international audience attending the world’s largest sports medicine and exercise science conference, Dr Fadzil illustrated how healthcare providers in Singapore collaborated to pilot a community health programme in partnership with community stakeholders and supported by government health initiatives in the early intervention of diabetes. 2019 had been a fruitful year for the EIMS research arm, with increasing collaborations with external organisations and tertiary institutions. EIMS collaborated with the Nursing Department in the School of Health Science in Ngee Ann Polytechnic to look at the level of physical activities among nursing students as part of students’ final-year-project. With Dr Fadzil Hamzah was one of the panelists at guidance and input from Dr Lim Ang Tee, the team won best poster in the Eat, Sleep and Exercise Well public forum the 2019 Ngee Ann Health Science Research Symposium. 19 COMMUNITY PROGRAMMES

Care Team Education Workgroup Following the successful implementation of the Health Peers Programme by CGH in collaboration with community stakeholders and government agencies, the Ministry of Health appointed Dr Fadzil Hamzah as a member of the Care Team Education Workgroup under the Disease Management Workgroup (War on Diabetes). The workgroup developed a structured curriculum on patient empowerment for self-care framework using validated learning and coaching techniques. They have since conducted their first workshop with medical and allied health staff in November 2019.

Outside of clinical settings, the SEM department supports various community efforts to promote physical activity and healthy living. This is in line with the MOH “Beyond Healthcare 2020” key thrust of moving “Beyond Healthcare to Health”, where sport and exercise medicine can play a vital role.

M3 (Muis, Mendaki, Mesra)

M3 was officially launched in January 2019 and is made up of self-help group Mendaki, the Islamic Religious Council of Singapore (Muis) and the People’s Association Malay Activity Executive Committees Council (Mesra). The M3 Network was launched with the objective of bringing community programmes to heartlands and creating opportunities for community members to volunteer and serve. Dr Fadzil Hamzah was frequently engaged in several M3 events and contributed greatly to public health education.

As a multidisciplinary field, SEM recognises the importance of having a cohesive and collaborative team for vibrant and efficient partnerships. Thus, in CGH SEM, we have the following platforms to promote effective communication:

• Monthly multidisciplinary rounds for clinical sharing; • Bi-monthly leadership meetings, where HODs across disciplines (e.g. SEM, physiotherapy, exercise physiology, nursing, podiatry, dietitian, operation, administration) discuss clinical and operational matters; • Bi-monthly town hall, for discussion and updates to all team members and stakeholders across disciplines; • Quarterly SEM Directors’ meeting for strategic planning; • Quarterly SingHealth Duke-NUS Sport and Exercise Medicine Centre EXCO meeting; • Annual Strategic Planning Meeting where HODs discuss future strategic directions and plans, and; • Team bonding activities, such as annual team building day, festive celebrations and gatherings.
