Our Wilderness and Urban Disaster First Responder course is designed with the outdoor professional and urban citizen in mind. It focuses on developing skills for treating problems outside the “ golden hour” of first response, under extreme conditions or when forced to improvise. This could be in a remote mountain setting, or in a huge city when there is a disaster that requires more medical resources than are available. The course is based on the protocols set by the Wilderness Medical Society and instruction encourages the students ability to think through body systems in an effort to determine the best response to traumatic, environmental or medical situations. Classroom sessions are followed with hands on practice to integrate the information in a field setting. 100% attendance is required and there will be a night scenario included. The students will get to see their skills in action, as video coverage of intensive scenarios will be reviewed towards the end of the class. A comprehensive written exam is also administered. The course includes AEHS adult, child and infant CPR certification, which is OSHA approved. We will be spending quite a bit of time outside (possibly rain, hopefully sun, etc.). This concept in neonatology has been adopted from adult trauma where the initial first hour of trauma management is considered as golden hour. The “Golden hour” concept includes practicing all the evidence based intervention for term and preterm neonates, in the initial sixty minutes of postnatal life for better long-term outcome. Although the current evidence supports the concept of golden hour in preterm and still there is no evidence seeking the benefit of golden hour approach in term neonates, but neonatologist around the globe feel the importance of golden hour concept equally in both preterm and term neonates. Initial first hour of neonatal life includes neonatal resuscitation, post-resuscitation care, transportation of sick newborn to neonatal intensive care unit, respiratory and cardiovascular support and initial course in nursery. The studies that evaluated the concept of golden hour in preterm neonates showed marked reduction in hypothermia, hypoglycemia, intraventricular hemorrhage (IVH), bronchopulmonary dysplasia (BPD), and retinopathy of prematurity (ROP).
