DAVID GRATZER* He used twenty-two pens to sign the Patient Protection and Affordable Care Act (PPACA)1 into law.2 On March 23, 2010, President Obama finally put the ink on the legislative drive that had stretched for more than a year, and had burned brightly in the imagination and dreams of Democrats since the demise of President Clinton’s efforts in 1994. The journey to the East Room signing ceremony had been long and hard. Not surprisingly, then, the ceremony itself seemed unusually festive. With the final pen stroke, he com- mented: “We are a nation that faces its challenges and accepts its responsibilities. We are a nation that does what is hard. What is necessary. What is right. Here, in this country, we shape our destiny.”3 The rhetoric may have been tall, but the legislation was sweeping.     News and analysis Fourth-year medical student takes Donner Prize When the winner of the Donner Prize for the best book on public policy was announced prematurely last month, many people couldn’t believe who had won — including the author himself. It took a call from a Toronto reporter to convince 25-year-old David Gratzer, who graduated from the University of Manitoba’s medical school this spring, that he had won the $25 000 first prize. William Frist is a nationally recognized heart and transplant surgeon, and a former majority leader of the U.S. Senate. Many of you will have seen him on television in the immediate aftermath of the Haiti earthquake, delivering primary care to the wounded and critically ill. Healthcare runs deep in the Frist family. His brother is currently operating the world’s largest health facilities company, a multibillion dollar enterprise in the United States with 173 hospitals and 107 surgery centres in 20 states. He is currently a professor of business and medicine at Vanderbilt University. David Gratzer, a practicing physician in both Canada and the United States, is a graduate of the University of Manitoba with post graduate medical training at the University of Toronto. He works here in the city of Toronto with patients suffering from severe mood disorders and he’s published widely in academic press. More important to us, he’s also a best selling author, a winner of the Donner Book Prize and a fellow at the prestigious Manhattan Institute in New York City. I think it is safe to say he is one of a few but true cross­border healthcare experts. Certainly it’s a topic that seems perhaps, unfortunately, even more relevant than ever before. As you know, with the pandemic, we’ve seen different groups affected differently. In fact, 83 percent of people diagnosed with COVID in the greater Toronto area have been black people or people of colour. Joining me today to talk about these weighty matters we have two doctors. Dr. Amy Gajaria is a child and adolescent psychiatrist at the Center for Addiction and Mental Health. She’s also inaugural associate director EDI Education and training with the University of Toronto’s department of Psychiatry. She’s been very vocal on these issues and has written for a variety of publications, including The Globe and Mail. Also joining me is Dr. Saadia Sediqzadah, who is a psychiatrist at St. Michael’s Hospital. She is with the Department of Psychiatry at the University of Toronto, where she’s co-lead of Underserviced/Marginalized/Selective and curriculum. Doctor, we’re talking about technology in medical education. How are things likely to change moving forward? Sanjeev Sockalingam: Well, David, I think it’s an interesting question, because I think change has been some of the challenge in technology in medical education. As I think of some of your earlier podcasts, you’ve talked about technology and its use in clinical practice. And I think in health professions, and specifically medical education, there’s a need for us to consider how we prepare the current and future clinicians to be able to use, adapt to, and incorporate technology into our clinical care. And then with that, how can we use these same technologies that are currently in practice, and as they advance, to better create more accessible, interactive, practice-based types of training opportunities so that we can measure our outcomes better in terms of changes in provider practice, and quality improvement, and better patient care overall. David Gratzer: Let’s talk about your first point. Patients are doing therapy differently. They’re learning about care differently. How should education adjust? Sanjeev Sockalingam: I think we probably don’t do a great job in our medical school training, for example, or residency training with our psychiatry trainees. But, more broadly, in postgraduate education and for our existing providers who are seeing patients in their practice.

The purpose of the US Constitution, as stated in the Preamble, is to "promote the general welfare" of the people. [43] According to ​ ​ former Congressman Dennis Kucinich (D­OH), as part of efforts to "promote the general welfare," health care "is a legitimate function of government." [44] ​ ​ 2) Instituting a right to health care could lower the cost of health care in the United ​ States. According to a 2013 study, under a single­payer system, in which all citizens are ​ guaranteed a right to health care, total public and private health care spending could be lowered by $592 billion in 2014 and up to $1.8 trillion over the next decade due to lowered administrative and prescription drug costs.[51] According to the American Medical Association, on average, ​ ​ private health insurance plans spend 11.7% of premiums on administrative costs vs. 6.3% spent by public health programs. [52] According to a study in the American Journal of Public Health, ​ ​ ​ ​ Canada, a country that provides a universal right to health care, spends half as much per capita on health care as the United States. [53] In 2010 the United Kingdom, another country with a ​ ​ right to health care, managed to provide health care to all citizens while spending just 41.5% of what the United States did per capita.
