THE US HEALTH CARE CRISIS: IMPLICATIONS FOR EDUCATION, MEDICAL PRAXIS, AND DEMOCRACY by MICHAEL KLEINPETER (Under the Direction of Rosemarie Stallworth-Clark) ABSTRACT This theoretical inquiry sought to critically analyze the various ideological and political aspects of the culture of health care reform in the United States (US) and to investigate the underlying ideologies, values, and purposes of medical practice in a changing democratic society. Using Michel Foucault’s cultural studies approach as the theoretical framework for the study, agent and client relationships within the US health care system were examined in efforts to describe the articulation between the health care crisis and the government’s social responsibility to prevent the imminent bankruptcy of its health care system. State and national data were collected from governmental web sites of the Department of Health and Human Services, the Center for Disease Control and Prevention, and the Center for Medicare and Medicaid Services. This data describes the percentage of the uninsured population in both Georgia and US; total health care costs of the private and public sector in the US; total health care costs in both Georgia and the US; breakdown of health care costs in Georgia; breakdown of health care costs in US; and the Medicare allowables for five random current procedural terminology (CPT) codes. Socioeconomic trends in the US indicate a health care crisis. This study revealed the numbers of uninsured citizens in this country are steadily increasing and the economic impact on these individuals, as well as the rest of the country, is devastating. Literally every hospital in the US is subsidized by the federal and/or state government for indigent and charity care. Therefore, the tax payers ultimately pay for medical care for these individuals. All in all, there is a tremendous financial and health strain to the entire nation because of the uninsured problem in the US. Implications of this study included the following: the health care industry must to do a better job of educating patients on health care issues; catastrophic insurance can provide a useful solution to protect patients from rising costs; patients must begin to think preventive instead of curative; tax deductions should be extended to individuals to pay for health insurance; and a nationwide information system should be implemented. DAVID GRATZER* He used twenty-two pens to sign the Patient Protection and Affordable Care Act (PPACA)1 into law.2 On March 23, 2010, President Obama finally put the ink on the legislative drive that had stretched for more than a year, and had burned brightly in the imagination and dreams of Democrats since the demise of President Clinton’s efforts in 1994. The journey to the East Room signing ceremony had been long and hard. Not surprisingly, then, the ceremony itself seemed unusually festive. Doctor, we’re talking about technology in medical education. How are things likely to change moving forward? Sanjeev Sockalingam: Well, David, I think it’s an interesting question, because I think change has been some of the challenge in technology in medical education. As I think of some of your earlier podcasts, you’ve talked about technology and its use in clinical practice. And I think in health professions, and specifically medical education, there’s a need for us to consider how we prepare the current and future clinicians to be able to use, adapt to, and incorporate technology into our clinical care. And then with that, how can we use these same technologies that are currently in practice, and as they advance, to better create more accessible, interactive, practice-based types of training opportunities so that we can measure our outcomes better in terms of changes in provider practice, and quality improvement, and better patient care overall. David Gratzer: Let’s talk about your first point. Patients are doing therapy differently. They’re learning about care differently. How should education adjust? Sanjeev Sockalingam: I think we probably don’t do a great job in our medical school training, for example, or residency training with our psychiatry trainees. [44] ​ ​ 2) Instituting a right to health care could lower the cost of health care in the United ​ States. According to a 2013 study, under a single­payer system, in which all citizens are ​ guaranteed a right to health care, total public and private health care spending could be lowered by $592 billion in 2014 and up to $1.8 trillion over the next decade due to lowered administrative and prescription drug costs.[51] According to the American Medical Association, on average, ​ ​ private health insurance plans spend 11.7% of premiums on administrative costs vs. 6.3% spent by public health programs. [52] According to a study in the American Journal of Public Health, ​ ​ ​ ​ Canada, a country that provides a universal right to health care, spends half as much per capita on health care as the United States. [53] In 2010 the United Kingdom, another country with a ​ ​ right to health care, managed to provide health care to all citizens while spending just 41.5% of what the United States did per capita.

When the concept is rigorously specified, as more than just adopting similar policies and/or strategies, but as engaging in a discourse or exchange that reshape an actor’s understanding of an issue, empirical studies have found that policy actors sometimes fail to live up to its requirements. Nevertheless it is a concept that has persisted and is used frequently. Among the most frequently cited types of policy learning is “international” policy learning, where one jurisdiction learns from the success of another. What is less well researched is the use that opponents of change make of international experience so as to rally resistance to policy reform. In a recent article, Lowry (2006) has proposed that this sort of resistive learning is sparked by “focusing projects”.
