Providing parent/s and or carer/s with key health promotion messages in the primary health care setting 4.1 Work in partnership [2] with families to provide health promotion that builds on parent/s and or carer/s capacity and competence to make informed decisions for the health and wellbeing of themselves and their infant /child [3]. 4.2 Provide parent/s and or carer/s with health promotion messages that are congruent with national and state health policies and guidelines, and, appropriate for the infant/child’s age and stage of development. Refer to appendix 1 for key anticipatory health promotion messages that are relevant for each age and stage of development, in line with the current scheduled child health check ages in the personal health record [4]. 4.3 Provide parent/s and or carer/s with relevant evidenced-based, contemporary health promotion information and resources. Appendix 2 of this guideline provides further explanation of key health promotion messages outlined in Appendix 1with links to relevant resources. 4.4 When planning health promotion activities, consider; • Health promotion needs as identified by the community, group, or individual (parent / carer). • Appropriate resources for parent/s and or carer/s with literacy and/or learning difficulties. • Tailoring relevant health promotion messages to rural and remote communities. • Resources that are culturally sensitive and meet the specific needs of CALD populations. Refer to appendix 2 for “additional resources”. • Appropriate resources for parent/s and or carer/s with mental health conditions. 4.5 Provide health promotion in a variety of settings, including; • pre-planned programs • opportunistically • individually • group and family situations • various environments i.e. home, workplace, community centres, childcare and pre school [1] and Aboriginal and Torres Strait Islander Organisations. Hint: they are not just small full term infants • Preemies have less fat to maintain body temperature. • Preemies are easily overwhelmed by sensory stimulation. • The brain develops rapidly in the last trimester of gestation. • Early experiences shape the developing brain. Minimizing stressful experiences is important. Why Immersion Bathe or Swaddle Bathe a Full-Term Well Baby? • Decreased crying • Decreased incidence of hypoglycemia • Evidenced based practice for developmentally supportive care • No difference in umbilical • Delayed bathing of 6-24 hours enables parents to bathe their cord healing or colonization baby • Decreased weight loss • Decreased risk of hypothermia • Improved bonding and parent satisfaction • Improved breast feeding Family Involvement • Swaddle bathing provides a “typical” parenting activity that carries to home care.

The infants’ body temperature, heart rate, and oxygen saturation were monitored to examine hypothermia risks during bathing. The participants of the study were 32 premature infants who were hospitalized right after their birth in the V hospital in Korea between December 2012 and August 2013. The informed consents of the study were received from the infants’ parents. The infants were randomly assigned into two-day and four-day bath cycle groups and their physiological reactions were monitored before bathing as well as 5 and 10 min after bathing. The collected data were analyzed using the SPSS statistical package through t-test. A significant drop in body temperature was noted in both groups; that is, 4-day bathing cycle and 2-day bathing cycle (p < 0.001). However, there were no significant changes in heart rate or transcutaneous oxygen levels. There was no significant change between groups at each measurement point. In order to minimize the physiological instability that may be caused during bathing, the care providers should try to complete bathing within the shortest possible time and to make bathing a pleasant and useful stimulus for infants.

By © Susan Warren A Thesis submitted to the School of Graduate Studies in partial fulfillment of the requirements for the degree of Master of Science in Medicine (Clinical Epidemiology) Discipline of Clinical Epidemiology, Faculty of Medicine Memorial University of Newfoundland October 2018 St. John’s Newfoundland and Labrador ABSTRACT Objective: To determine if delaying a newborn’s first bath until at least 24 hours of life, as recommended by the World Health Organization, effects in-hospital breastfeeding rates, infant hypothermia rates and/or infant hypoglycemia rates. Methods: Retrospective cohort study comparing 680 infants bathed before 24 hours to 545 infants bathed after 24 hours. The primary outcome was comparison of the rates of in-hospital breastfeeding initiation and exclusive breastfeeding at discharge. Secondary outcomes were a comparison of rates of infant hypothermia and hypoglycemia. Results: Exclusive breastfeeding rates were 33% higher in the delayed bathing cohort compared to the early bathing cohort (AOR 1.334, 95% CI 1.049-1.698, p=0.019). No significant difference in breastfeeding initiation rates were observed in the total population or high-risk subgroup but in the average risk subgroup there was a significant 43% increase in breastfeeding initiation rates when bathing was delayed (AOR 1.433, 95% CI 1.008-2.039, p=0.045). Infants bathed after 24 hours were 2.5 times more likely to experience a hypothermic event than those bathed before 24 hours (AOR 2.524, 95% CI 1.239-5.142, p=0.011).
