GOVERNMENT OF NEPAL AIRCRAFT ACCIDENT INVESTIGATION COMMISSION 2013 FINAL REPORT ON THE ACCIDENT INVESTIGATION OF 9N-ABO TWIN OTTER (DHC6/300) AIRCRAFT OWNED AND OPERATED BY NEPAL AIRLINES CORPORATION AT JOMSOM AIRPORT, MUSTANG DISTRICT, NEPAL ON 16 MAY 2013 [FINAL REPORT] SUBMITTED BY THE COMMISSION FOR THE ACCIDENT INVESTIGATION TO THE GOVERNMENT OF NEPAL MINISTRY OF CULTURE, TOURISM AND CIVIL AVIATION 18/2/2014 (6/11/ 2070 BS) FINAL REPORT ON THE ACCIDENT INVESTIGATION OF 9N-ABO, TWIN OTTER (DHC-6/300) AIRCRAFT OWNED AND OPERATED BY 2013 NEPAL AIRLINES CORPORATION AT JOMSOM AIRPORT MUSTANG DISTRICT, NEPAL ON 16 MAY 2013 FOREWORD This Final Report on the accident of the Chartered Flight of Nepal Airlines Corporation 9N-ABO, Twin Otter (DHC6/300) aircraft has been prepared by the Aircraft Accident Investigation Commission constituted by the Government of Nepal, Ministry of Culture, Tourism and Civil Aviation, in accordance with Annex 13 to the Convention on International Civil Aviation and Civil Aviation (Accident Investigation) Rules, 2024 B.S. to identify the probable cause of the accident and suggest remedial measures so as to prevent the recurrence of such accidents in future. The Commission carried out thorough investigation and extensive analysis of the available information and evidences, statements and interviews with concerned persons, study of reports, records and documents etc. The Commission had submitted some interim safety recommendations as immediate remedial measures. The Commission in its final report presented safety recommendations to be implemented by the Ministry of Culture, Tourism and Civil Aviation, Civil Aviation Authority of Nepal and Nepal Airlines Corporation respectively. -

Richard Morcombe, who is currently flying with the UK National Police Air Service, is a former pilot with the British army and formerly flew the high skid EC135T1 helicopter VP-CPS now operated by the RCIPS for four years when he was stationed at the East Midlands Air Support Unit base before it was purchased by the Cay- man government. Morcombe will join the team in mid-September. He has been flying police helicopters for the last 13 years and has 7,000 hours of operational flight experience, includ- ing numerous day and night overwater operations around the world. This will not be his first time in Cayman as he has previously flown two relief duty periods in the Cayman Islands in 2010 and 2011. -

The crash initiated panic within UK Oil and Gas industry, demanding grounding of the Super Puma fleet of helicopters. Four people died when the CHC Super Puma crashed on approach to Sumburgh Airport on 23 August 2013. 1 All Super Puma helicopter passenger flights to UK oil installations were suspended after a crash off Shetland claimed the lives of four people. The Helicopter Safety Steering Group (HSSG) had advised grounding all variants of the helicopter. The HSSG, which is made up of oil industry representatives, advised that all models of the Super Puma series including: AS332 L, L1, L2 and EC225 should be grounded for "all Super Puma commercial passenger flights to and from offshore oil and gas installations within the UK." The Norwegian civil aviation authority had earlier rejected appeals from its unions to ground all its Super Pumas – which operate in the North Sea in very similar weather conditions to the UK fleet – insisting that Friday's crash was an isolated incident. -

R. China Abstract The purpose of this paper is to give a summary of aircraft collision/accidents and bird strikes in Nepal. It presents national and international registered aircraft statistics of bird strikes and aircraft collisions between 1946 and 2016 in Nepal. The paper enlightens bird strike probe risk and challenges of aircraft operations in Nepal, details of victim/ collided aircraft with/and aircraft brief specification/models. The data was collected by reviewing different sources from Civil Aviation Authority of Nepal (CAAN), International Civil Aviation Organization (ICAO), European Aviation Safety Agency (EASA), Bureau of Aircraft Accident Achieves (B3A), World Bird-Strike Association (WBA) and qualitative approach articles/newspaper/ interviews. Finally, this paper enhances safety measures to be taken by CAAN, obligation to investigate accidents with professional method of detection with prevention of such accidents in the near and the distance future from hull losses-hull fatalities to be enshrined regulators of ICAO and EASA. Press Brief Alpine Rescue Operation In Nepal 1. Introduction We are pleased to inform you all that Fishtail Air is in the process of introducing advanced Alpine Rescue System in the Nepalese Himalaya. In this connection we have partnered with Air Zermatt, a helicopter company based in Zermatt, Switzerland. Since its inception in 1968, Air Zermatt has pioneered Mountain rescue and flight operations in the Swiss Alps. The Swiss company is now helping to develop and establish an Alpine Rescue System in Nepal through transfer of technology, in partnership with Fishtail Air. In March this year, Air Zermatt organized a week long observation training programme on Alpine Rescue Operation in the Swiss Alps for a five member Nepalese team. The team included Fishtail Air’s CEO Mr.Suman Pandey, Capt. Sabin Basnyat - Chief Pilot, Mr.Pradip Gautam- Manager Operations, Mr. Purna Awale - senior technician and Mr. Binod Giri, Chief of Air Safety Department, Civil Aviation Authority of Nepal. The training was conducted by rescue experts from Air Zermatt. Capt Sabin Basnyat and Purna Awale received familiarization as well as practical training, while others received orientation on Alpine Rescue System’s operation. During 2010 spring climbing season, Air Zermatt has based a set of specialist Pilot and rescue expert Mountain Guide in Nepal to support Fishtail Air’s team for carrying out special rescue missions in the Himalaya. Capt. Daniel Aufdenblatten and Lehner Richard were into assignment from 24th of April to 8th May whereas, Capt. Gerold Biner and Mr. Bruno Jelk, a celebrity name in the field of Alpine rescue is in duty from 5th of May, 2010. -
