I am Dr. Bob Kadlec, the Assistant Secretary for Preparedness and Response (ASPR) at the Department of Health and Human Services (HHS). Today, I will provide background about how ASPR is partnering with the private sector to develop influenza vaccines, antivirals, and diagnostics to ensure we are as prepared for both seasonal as well as pandemic influenza. I will also provide an overview of the current challenges we face in preparedness due to dependence on active pharmaceutical ingredients (API) and other raw materials manufactured in other countries. Pandemic Influenza: a Costly National Security Threat Influenza has long posed a serious threat to human health. Seasonal influenza epidemics occur every year, leading to hundreds of thousands of hospitalizations and tens of thousands of deaths,1 and billions of dollars in economic loss. In the simplest of terms, the difference between seasonal influenza and a flu pandemic is that a pandemic occurs when a new flu virus emerges that humans have little or no immunity against, allowing the virus to spread easily from person to person worldwide. A pandemic influenza event could occur at any time, potentially claiming hundreds of thousands of lives.

The global COVID-19 pandemic has temporarily increased demand for basic medical equipment and supplies, and disrupted global supply chains. Governments at all levels and the private sector have found themselves scrambling — and often competing — for the supplies they need. Federal law anticipates that emergencies can generate this kind of sudden demand for medical equipment. Federal agencies not only have ample legal authority to respond to shortages, but also the duty and the authority to prepare for emergencies by planning, supply-chain monitoring, investment and partnership with the private sector, and stockpiling. Perhaps the most important federal law for preventing and ameliorating shortages, and the primary focus of this Chapter, is the federal Defense Production Act (DPA). The DPA provides a menu of powers to stimulate production, strengthen supply chains, coordinate expertise, and resolve market failures. The exercise occurred on January 14, 2005, in Washington, DC, and was organized and convened by the Center for Biosecurity of UPMC, the Center for Transatlantic Relations of Johns Hopkins University, and the Transatlantic Biosecurity Network. Atlantic Storm portrayed a summit meeting of presidents, prime ministers, and other international leaders from both sides of the At- lantic Ocean in which they responded to a campaign of bioterrorist attacks in several countries. The summit principals, who were all current or former senior government leaders, were challenged to address issues such as attaining situational awareness in the wake of a bioattack, coping with scarcity of critical medical resources such as vaccine, deciding how to manage the movement of people across borders, and communicating with their publics. Atlantic Storm illustrated that much might be done in advance to minimize the illness and death, as well as the social, economic, and political disruption, that could be caused by an international epidemic, be it natural or the result of a bioterrorist attack. These lessons are especially timely given the growing concerns over the possibility of an avian in- fluenza pandemic that would require an international response.

    Introduction The Crimson Contagion 2019 Functional Exercise, henceforth referred to as “Crimson Contagion 19 FE,” is part four of a four part exercise series hosted by the Department of Health and Human Services/Office of the Assistant Secretary for Preparedness and Response. The exercise will be a multi-state, whole of government and community effort focused on the response and policy issues of workforce viability, critical infrastructure protection, economic impact, social distancing, scarce resource allocation, prioritization of vaccines and other countermeasures, and medical surge operations. The Crimson Contagion 19 FE will afford all partners the opportunity to exercise information exchange, coordination of resources, and policy decisions in an environment in which the Department of Health and Human Services is the lead federal agency in accordance with the Biological Incident Annex and the Pandemic Crisis Action Plan. The exercise will be conducted from August 13 – 16, 2019 at multiple locations across the U.S. Exercise Scenario The Crimson Contagion 19 FE depicts a scenario based on a novel influenza A(H7N9) virus that originates in China and quickly spreads to the continental U.S., where the virus is first detected in Chicago, Illinois. The virus rapidly spreads via human-to-human transmission to other metropolitan areas across the nation with high rates of morbidity. Figure 1 below depicts the pandemic severity assessment for H7N9. Stockpiled H7N9 vaccines do not match the pandemic virus but can be used as a priming dose. The H7N9 virus is susceptible to neuraminidase inhibitor drugs but resistant to adamantane antiviral drugs. Founded in 1921, CFR carries out its mission by maintaining a diverse membership, with special programs to promote interest and develop expertise in the next generation of foreign policy leaders; convening meetings at its headquarters in New York and in Washington, DC, and other cities where senior government officials, members of Congress, global leaders, and prominent thinkers come together with Council members to discuss and debate major international issues; supporting a Studies Program that fosters independent research, enabling CFR scholars to produce articles, reports, and books and hold roundtables that analyze foreign policy issues and make concrete policy recommendations; publishing Foreign Affairs, the preeminent journal on international affairs and U.S. foreign policy; sponsoring Independent Task Forces that produce reports with both findings and policy prescriptions on the most important foreign policy topics; and providing up-to- date information and analysis about world events and American foreign policy on its website, CFR.org.

    ATLANTIC STORM –– A TRANSATLANTIC BIOTERRORISM EXERCISE Atlantic Storm was a ministerial table-top exercise convened in Washington, DC, on January 14, 2005. It was created and presented by the Center for Biosecurity of the University of Pittsburgh Medical Center, the Center for Transatlantic Relations of Johns Hopkins University, and the Transatlantic Biosecurity Network and was funded by the Alfred P. Sloan Foundation, the German Marshall Fund of the United States, and the Nuclear Threat Initiative. The exercise was held in real-time using the world’s current geopolitical context as a backdrop, and used a fictitious scenario designed to mimic a summit of transatlantic leaders forced to respond to a bioterrorist attack.
