GUIDELINES 1. This Application Form can be photocopied/reproduced. 2. Only true and correct entries on this form are accepted; any false information will be a ground for your disqualification. 3. Write the code number in the testing center code box after selecting your preferred examination center from the table of Testing Centers. 4. Applicants have 2 options to file the Application Form:  Personal – From October (right after the last Entrance Examination) to September (the following year) @ PMMA Zambales only  Mail – From October (right after the last Entrance Examination) to August 31 (the following year) only 5. ADMISSIONS REQUIREMENTS:  3 copies of recent 2x2 colored pictures with name tag, front view, white background.  2 self-addressed long white mailing envelopes with P40.00 worth of mailing stamp per envelope for mailing Test Permit and Examination Result. EXAM: The PMMA entrance examination covers the following subjects/areas: English (Reading Comprehension, Grammar, Vocabulary), High school Mathematics (Algebra, Geometry, Statistics and Trigonometry), Science (Gen. Science, Chemistry & Physics), and Abstract Reasoning Each applicant whose qualifications have been approved shall be given a permit to take the test by the Admissions Office. For mailed applications, permit to take the test shall be sent to the applicant through mail. Please take note of the following:  Examination is from 8:00am – 12:00nn  Arrive one hour before the examination proper at your designated testing center  Bring your test permit (Show any valid I.D. if you did not receive your test permit)  Bring any valid Identification  Use pencil mongol #2  Wear casual clothes (Sleeveless shirt, Short pants and slippers are not allowed).  Calculators are not allowed 7. QUALIFICATION REQUIREMENTS GROUNDS FOR MEDICAL DISQUALIFICATION Applicant should/must be:  Defective Visual Perception (color blindness, near sighted/far sighted with  Male or female, physically fit, natural born Filipino citizen; corrective eyeglasses or lens);  Single and never been married and has no children out of wedlock;  Physical Deformities (bowlegged) congenital defects;  Free from being accused or convicted of any crime against the law  Perforated ear drum, chronic sinusitis, bleeding tendencies, hemorrhoids, hernia and the state; (if not treated), varicocoele, extensive skin disorders;  At least 17 years old and not more than 23 years old upon  History of heart disease, hypertension, asthma, active PTB, kidney and liver enrollment on August 01; diseases, epileptic seizure disorders, major operations, communicable diseases  Grade 12 student; College Level (such as AIDS, Hepatitis, VD, etc.);  At least 160 centimeters (5 ft. & 3 in.) for male and 155  Full dentures, upper and lower; Cleft lip and palate, Malocclusion, Deformities of centimeters (5 ft. & 1 in.) for female; the face, Open bite, Tongue-tied; Missing of four anterior/posterior teeth.  BMI of 25;  Tattoos and other brotherhood marks;  Good academic performance  Additional for Females: Goiter, History of pregnancy, presence of breast mass,  Has never been enrolled in PMMA. painful menstruation.
