# 1 Center Co-op Inclusion Preschool Parent Participation Preschool in the Center Unified School District

Preschool Participation Please Please Initial Contract 2020/2021 _____ Children must be at least 2 years & 9 months old, current on their immunizations and toilet trained to enroll.

_____ $50 registration fee for Fall & Spring Semesters.

Preschool Hours: _____ Morning starts at 8:45 AM & ends at 11:25 AM. Afternoon starts at 12:15 PM and ends at 2:55 PM.

_____ If you arrive more than 5 minutes late for pick-up you will be charged $1 per minute, payable upon arrival.

Monthly fees are due the first week of the month & late after class on Thursday. _____ Fees are non-refundable. Late fees: $10 if received the following Monday, $25 after that.

_____ Provide snack for your child’s class as assigned on the monthly snack list. Pay $20 for missed snack day.

_____ Give one week’s notice when leaving or making changes in the program.

Participation Fee Choice: 1 day a week/$200 monthly 2 days a week/$80 monthly No Participation/$320 monthly

For Adults Working in the Classroom. Adult TB (tuberculosis) Risk Assessment Questionnaire & TB skin or X-ray test results, if required. _____ Must be current within the last twelve months (once enrolled good for 4 years).

Adults participating in the classroom must clear Megan’s Law Database Check and obtain fingerprint clearance _____ for the Center Unified School District at their own cost.

_____ Sign in & out on the Adult Attendance Sheet to verify competition of on-time & full session participation.

_____ Weekly In-class, 3 hour Workday, 8:30 to 11:30AM or 12:00 to 3:00PM

Arrive on time for your workday (8:30 AM or 12:00 Noon). Late arrivals will incur a $25 late fee: 10 to 20 minutes late two times in a month, or 30 minutes late one day. _____ The late fee will be charged to the parent the following month.

Be responsible for covering your workday, preferably by switching workdays with another adult. If you don’t find a replacement for your workday, you can pay $30 to buy out the day or schedule a make-up _____ day with the teacher. The cleaning day in May is counted as a workday.

If your workday is a Monday and it falls on a school week with a Monday holiday you must schedule a _____ make-up day with the teacher or pay $30 to buy out the day.
