MV-75 (12/15) NOTIFICATION OF MILITARY SERVICE

Part 1 For a NYS licensed driver who has entered military service, Section 9196(3) of the Unconsolidated Laws of New York State extends the expiration date of their driver license if it is set to expire prior to their separation from service; the expiration date of the license will be extended to six months after the date of separation from service. Anyone who does not use this form to notify the Department of Motor Vehicles of their entry into service may not be able to renew their license when discharged. INSTRUCTIONS: A. Complete both parts of this form using the information on your New York State driver license. Update your address if it has changed. B. Separate the two parts and mail Part 1 to: NEW YORK STATE DEPARTMENT OF MOTOR VEHICLES LICENSE PRODUCTION BUREAU PO BOX 2895 ALBANY NY 12220-0895 C. Cut out, fold, and keep Part 2 with your NYS driver license.
