Received: 20 February 2020; Accepted: 13 March 2020; Published: 20 March 2020 Abstract: Homocystinuria is an inborn error of metabolism due to the deficiency in cystathionine beta-synthase (CBS) enzyme activity. It leads to the elevation of both homocysteine and methionine levels in the blood and urine. Consequently, this build-up could lead to several complications such as nearsightedness, dislocated eye lenses, a variety of psychiatric and behavioral disorders, as well as vascular system complications. The prevalence of homocystinuria is around 1/200,000 births worldwide. However, its prevalence in the Gulf region, notably Qatar, is exceptionally high and reached 1:1800. To date, more than 191 pathogenic CBS mutations have been documented. The majority of these mutations were identified in Caucasians of European ancestry, whereas only a few mutations from African-Americans or Asians were reported. Approximately 87% of all CBS mutations are missense and do not target the CBS catalytic site, but rather result in unstable misfolded proteins lacking the normal biological function, designating them for degradation. The early detection of homocystinuria along with low protein and methionine-restricted diet is the best treatment approach for all types of homocystinuria patients. Yet, less than 50% of affected individuals show a significant reduction in plasma homocysteine levels after treatment. Patients who fail to lower the elevated homocysteine levels, through high protein-restricted diet or by B6 and folic acid supplements, are at higher risk for cardiovascular diseases, neurodegenerative diseases, neural tube defects, and other severe clinical complications. This review aims to examine the mutations spectrum of the CBS gene, the disease management, as well as the current and potential treatment approaches with a greater emphasis on studies reported in the Middle East and North Africa (MENA) region. Introduction Inborn errors of metabolism represent a large group of genetic disorders that result from an inherited deficiency or lack of proteins involved in metabolic pathways. The majority are defects of single genes that code for enzymes, which catalyze essential biologically and biochemical reactions. Although each specific inborn error of metabolism is rare, taken together, they represent a significant source of congenital disabilities. For instance, it is estimated that a genetically caused inborn error of metabolism occurs in 1/2500 births [1]. A prototypical example of an inborn error of metabolism is homocystinuria due to CBS deficiency [2]. Classical homocystinuria (OMIM 236200) is an autosomal recessive disease and the most common form of homocystinuria worldwide. It is an inborn error of metabolism with high clinical variability resulting in a metabolic disorder due to deficiency in cystathionine beta-synthase CBS enzyme activity (Figure1). The availability of large internet databases of users’ ratings has permitted quantitative analysis of polarization trends—for instance, previous studies have included analyses of controversial topics on Wikipedia, as well as the relationship between online reviews and a product’s perceived quality. Here, we study the dynamics of polarization in the movie ratings collected by the Internet Movie database (IMDb) website in relation to films produced over the period 1915–2015. We define two statistical indexes, dubbed hard and soft controversiality, which quantify polarized and uniform rating distributions, respec- tively. We find that controversy decreases with popularity and that hard controversy is relatively rare. Our findings also suggest that more recent movies are more controversial than older ones and we detect a trend of “convergence to the mainstream” with a time scale of roughly 40–50 years. This phenomenon appears qualitatively different from trends observed in both online reviews of commercial products and in political debate, and we speculate that it may be connected with the absence of long-lived “echo chambers” in the cultural domain. The aim of this study was to investigate the association between dietary patterns and depression via the intermediary role of the serum folate and vitamin B12, total homocysteine, tryptophan, and tryptophan/competing amino acids ratio. Methods: This was an individually matched case-control study in which 110 patients with depression and 220 healthy individuals, who completed a semi-quantitative food frequency questionnaire were recruited. We selected the depressed patients from three districts in Tehran through non-probable convenience sampling from which healthy individuals were selected, as well. The samples selection and data collection were performed during October 2012 to June 2013. In addition, to measure the serum biomarkers 43 patients with depression and 43 healthy people were randomly selected from the study population. To diagnose depression the criteria of Diagnostic and StatisticalManualofMentalDisorders, fourth edition, were utilized. Results: The findings suggest that the healthy dietary pattern was significantly associated with a reduced odds of depression (OR: 0.75; 95% CI: 0.61–0.93) whereas the unhealthy dietary pattern increased it (OR: 1.382, CI: 1.116–1.71).

Recent studies demonstrated that an alteration in HCY metabolism or a deficiency in folate or vitamin B12 can cause altered methylation and/or redox potentials, that leads to a modification on calcium influx in cells, or into an accumulation in amyloid and/or tau protein involving a cascade of events that culminate in apoptosis, and, in the worst conditions, neuronal death. The present review will thus summarize how much is known about the possible role of HHCY in neurodegenerative disease.

    Introduction to Proteins and Amino Acids Introduction • Twenty percent of the human body is made up of proteins. Proteins are the large, complex molecules that are critical for normal functioning of cells. • They are essential for the structure, function, and regulation of the body’s tissues and organs. • Proteins are made up of smaller units called amino acids, which are building blocks of proteins. They are attached to one another by peptide bonds forming a long chain of proteins. Amino acid structure and its classification • An amino acid contains both a carboxylic group and an amino group. Amino acids that have an amino group bonded directly to the alpha-carbon are referred to as alpha amino acids. • Every alpha amino acid has a carbon atom, called an alpha carbon, Cα ; bonded to a carboxylic acid, –COOH group; an amino, –NH2 group; a hydrogen atom; and an R group that is unique for every amino acid. Classification of amino acids • There are 20 amino acids. Based on the nature of their ‘R’ group, they are classified based on their polarity as: Classification based on essentiality: Essential amino acids are the amino acids which you need through your diet because your body cannot make them. Whereas non essential amino acids are the amino acids which are not an essential part of your diet because they can be synthesized by your body.
