According to NCDEQ files, the previous CEI was conducted on May 27, 2009. PPE & SAFETY REQUIREMENTS: PPE & Safety requirements include: safety glasses, steel toed shoes. FACILITY DESCRIPTION: Cherry Hospital is an inpatient regional referral psychiatric hospital. As one of three psychiatric hospitals operated by the NC Department of Health and Human Services, Cherry Hospital provides services to 38 counties in the eastern region of North Carolina. Cherry Hospital NCD099816084 Compliance Evaluation Inspection Report Page 2 of 4 February 24, 2017 “As a place of hope, care and recovery,” Cherry Hospital has been working to improve the lives of those experiencing severe mental illness for more than a century. A teaching site for the Brody School of Medicine at East Carolina University and Campbell University, Cherry Hospital welcomes psychiatry residents, fellows and medical students routinely as they embrace the opportunity to train with the professional staff holding clinical faculty appointments. In addition, Cherry Hospital has affiliations with several colleges and universities for internship placements in social work, psychology, teaching (exceptional children), dental hygiene, pharmacy and therapeutic recreation. The hospital has affiliations with colleges/universities in several other states for occupational therapy internships. Cherry Hospital offers mental health services to adolescents, adults and geriatric patients. With a full staff of licensed psychiatrists as well as internal medicine, family medicine physicians and mid-level practitioners, the hospital provides short term care as well as long term care when necessary. Treatment is comprehensive and includes physical, diagnostic, an array of therapeutic approaches including individual, group, behavior, milieu therapy, occupational, recreational and creative arts therapies. In September 2016, the 201 Stevens Mill Road site began the discontinuation of patient care services (continued maintenance activities only) which were transferred to the new location (psychiatric facility of the same name) at 1401 West Ash Street, Goldsboro, North Carolina. The new site is located approximately one-half mile from the old hospital. The “old” Cherry Hospital remains active in regards to maintaining two areas of maintenance activities which are located adjacent to one another: The Paint Shop and the Garage (vehicle maintenance). There are no known wells on the property. The distance to the nearest offsite well is unknown and the distance to the nearest residence is approximately 1 mile. The Neuse River is located less than one mile (0.9) to the south of the facility. The facility (201 Stevens Mill Road location) employs up to 24 personnel working a 7:30 a.m. – 5:00 p.m. shift. Water and sewer service is provided to the facility by the City of Goldsboro. The original hospital has been located at the subject site since August 01, 1880 (date of first patient admission). On December 12, 2008, the facility resubmitted the 8700-12 in order to notify NCDEQ DWM as a Conditionally Exempt Small Quantity Generator (CESQG) of hazardous waste (previously operated as a Small Quantity Generator (SQG). WASTE STREAMS INCLUDE: Hazardous Waste Streams and Waste Codes include: • D001 Waste Aerosols The facility also generates used lamps and used batteries, both of which are managed as universal waste and recycled. Used oil is generated as part of vehicle maintenance activities. AREAS OF REVIEW AND INSPECTION: Emergency Preparedness: The facility is operated to minimize the possibility of fire, explosion or any unplanned sudden or nonsudden release of hazardous waste that threatens health or the environment. All safety and emergency equipment is inspected or tested on a regular basis to ensure operation in case of an emergency. Training Records: Training for employees regarding proper handling and emergency procedures for Universal Waste appeared to be in compliance for a Conditionally Exempt Small Quantity Generator (CESQG). Cherry Hospital NCD099816084 Compliance Evaluation Inspection Report Page 3 of 4 February 24, 2017 Manifests: Hazardous waste manifests dated November 03, 2016 and January 17, 2017 were reviewed and observed to be in compliance and complete with land disposal restriction documents. Waste Streams noted on the manifests included: waste aerosols (flammable), used lamps and batteries. Transporters: Clean Harbors Environmental Services Inc MAD039322250 TSD’s: Clean Harbors Reidsville LLC NCD000648451 ACCUMULATION AREAS Paint Shop Daniel Vaccaro assisted with the Paint Shop walk through. Observed the following: Two Storage Lockers containing various paints (latex/oil based; no lead based), finishing products, staining products, lacquers, enamels, penetrating catalyst/lubricants and solvents. Dedicated red metal containers for the disposal of used rags. Aerosol Cans: Dialogue included utilizing puncturing process and recycling as scrap metal. Dialogue included protocols for hazardous waste determinations of any product utilized prior to disposal. Garage Gary Langston assisted with the Garage walk through. Observed the following: Dedicated red metal containers for the disposal of oily rags; red parts washer with a 40-gallon drum attachment; one portable used oil caddie; one 5-gallon container for used oil filters (taken to the county landfill’s collection point). Used oil is generated through the Garage maintenance shop primarily from the servicing of facility vehicles. Two storage lockers are utilized; wherein, two 55-gallon drums are in place in each storage locker for used oil recycling. Dialogue included definition of used oil versus waste oil and proper labelling in regards to each. Lithium, Nicad, and Lead Batteries are properly stored and picked up by a contracted transporter. Used Lamps and ballasts are properly stored for pick up by contracted transporter. Storage Area: The facility’s hazardous waste storage area is located outside adjacent to the Paint Shop. The area is fenced, secured and posted properly. An empty 55-gallon storage drum (lid present and on a pallet) was present and labelled as “Hazardous Waste.” Dialogue included proper identification and storage of any containers within the storage area. CONDITION OF FACILITY: The overall condition of the facility sites, both internally and externally, was good. No adverse conditions were observed. The facility appeared to be operating in compliance as a Conditionally Exempt Small Quantity Generator (CESQG) regarding regulations described at 40 CFR Parts 260 – 270, 273 and 279.
