Eye neoplasm Origin and location Eye cancers can be primary (starts within the eye) and metastatic cancer (spread to the eye from another organ). The two most common cancers that spread to the eye from another organ are breast cancer and lung cancer. Other less common sites of origin include the prostate, kidney, thyroid, skin, colon and blood or bone marrow. Types Tumors in the eye and orbit can be benign like dermoid cysts, or malignant like rhabdomyosarcoma and retinoblastoma. Signs and symptoms • Melanomas (choroidal, ciliary body and uveal) - In the early stages there may be no symptoms (the person does not know there is a tumor until an ophthalmologist or optometrist looks into the eye with an ophthalmoscope during a routine test). As the tumor grows, symptoms can be blurred vision, decreased vision, double vision, eventual vision loss and if they continue to grow the tumor can break past the retina causing retinal detachment. Sometimes the tumor can be visible through the pupil. • Nevus - Are benign, freckle in the eye. These should be checked out and regular checks on the eye done to ensure it hasn't turned into a melanoma. • Iris and conjuctival tumors (melanomas) - Presents as a dark spot. Any spot which continues to grow on the iris and the conjunctiva should be checked out. • Retinoblastoma - Strabismus (crossed eyes), a whitish or yellowish glow through the pupil, decreasing/loss of vision, sometimes the eye may be red and painful. Retinoblastoma can occur in one or both eyes. This tumor occurs in babies and young children. It is called RB for short. -

Thus, normal mode pigment-specific lasers and pulsed ablative lasers (CO2/erbium [Er]:yttrium aluminum garnet [YAG]) are more suited to treat acquired melanocytic nevi. The complexities of treating this disorder can be overcome by following a structured approach by using lasers that achieve the appropriate depth to treat the three subtypes of nevi: junctional, compound, and dermal. Thus, junctional nevi respond to Q-switched/normal mode pigment lasers, where for the compound and dermal nevi, pulsed ablative laser (CO2/ Er:YAG) may be needed. -

Extracellular vesicles (EVs) are nanometer-sized, lipid bilayer-membraned particles that are released from cells. In our study we used EVs derived from primary normal choroidal melanocytes and matched primary and metastatic uveal melanoma cell lines from a patient. Analysis of these EVs revealed important protein signatures that may be involved in tumorigenesis and metastatic dissemination. We have established a model to study EV functions and phenotypes which can be used in EV-based liquid biopsy. -
Melanomas Are Comprised of Multiple Biologically Distinct Categories
Melanomas are comprised of multiple biologically distinct categories, which differ in cell of origin, age of onset, clinical and histologic presentation, pattern of metastasis, ethnic distribution, causative role of UV radiation, predisposing germ line alterations, mutational processes, and patterns of somatic mutations. Neoplasms are initiated by gain of function mutations in one of several primary oncogenes, typically leading to benign melanocytic nevi with characteristic histologic features. The progression of nevi is restrained by multiple tumor suppressive mechanisms. Secondary genetic alterations override these barriers and promote intermediate or overtly malignant tumors along distinct progression trajectories. The current knowledge about pathogenesis, clinical, histological and genetic features of primary melanocytic neoplasms is reviewed and integrated into a taxonomic framework. It is characterized as congenital or acquired hamartoma of dermal melanocytes, presents clinically as a blue or gray patch on the face within the distribution of the ophthalmic and maxillary branches of the fifth cranial (trigeminal) nerve. Involvement of the palatal mucosa occurs rarely in nevus of Ota, when it occurs, it usually blends with the oral mucosa and is typically irregular, ill defined and often present as a mottled patch. Nevus of Ota is rare in the Indian subcontinent. So far very less cases of nevus of ota with intraoral involvement have been documented in the English literature. We report a rare case of intraoral nevus of Ota in a 20 year-old female patient. Culver Boldt, MD July 27, 2010 Chief Complaint: 31-year-old man with "floaters and blurry vision" in the right eye (OD). History of Present Illness: In August 2007, a healthy 31-year-old truck driver from Nebraska started noticing floaters in his right eye. The floaters gradually worsened and clouded his central vision. His family doctor tried changing his blood pressure medications, but this did not help. He later saw an ophthalmologist in his home state who told him there was a "mass" in his right eye. He was referred to the University of Iowa Department of Ophthalmology and Visual Sciences. Past Ocular History: The patient has had no prior eye surgery or trauma. Past Medical History: The patient reports prior excision of a benign skin nevus. He also has hypertension. Medications: Metoprolol and triamterene/hydrochlorothiazide Family History: The patient’s mother has a history of neurofibromatosis. His father had an enucleation for an "eye cancer" and subsequently died due to metastatic spread of the cancer. His grandmother had skin melanoma. Social History: The patient lives in Nebraska with his wife and child. He has never smoked and only drinks on "special occasions". Review of Systems: Negative, except as noted above. Ocular Examination: General: Well-developed, well-nourished Caucasian man in a pleasant mood Skin: Several scattered macules and papules on the trunk and all four extremities Distance visual acuity (without correction): o 20/60-2 OD o 20/20 OS Near acuity (without correction) o 20/30 OD o 20/20 OS Ocular motility: Full OU. -

Maguire and Holt Journal of Medical Case Reports (2019) 13:174 https://doi.org/10.1186/s13256-019-2101-0 CASEREPORT Open Access Nevus of Ota – an intraoral presentation: a case report Jennifer Maguire1* and Deborah Holt2 Abstract Background: Nevus of Ota or “oculodermal melanocytosis” is a rare congenital hamartoma of dermal melanocytes causing a blue-gray hyperpigmentation of the eye and surrounding structures. The condition, originally described by Ota and Tanino in 1939, mainly affects the ophthalmic and maxillary divisions of the trigeminal nerve. We describe the first reported case of unilateral oculodermal melanocytosis in a Caucasian woman with oral buccal mucosal involvement. Oral involvement of nevus of Ota is very rare. Case presentation: A 48-year-old Caucasian woman was referred by the dermatology division to the oral medicine department at the University of Liverpool School of Dentistry with new-onset oral pigmentation to the left buccal mucosa. The patient had a previous diagnosis of oculodermal nevus. Conclusion: An incisional biopsy of the left buccal mucosa was completed. The report stated that histological and immunohistochemical features were in keeping with a blue nevus, but within the context of the preexisting occulodermal pigmentation, a diagnosis of oculodermal melanocytosis, also known as “nevus of Ota,” was made. The patient will be kept under review in the oral medicine department because the progression of the lesion on the left buccal mucosa requires active monitoring owing to the potential for malignant change.
