The project is a first of a kind wraparound program targeting the highest risk youths in the San Francisco juvenile justice system. It combines mental health and foster care funding to reduce the number of youth in out-of-home placement and institutional care. Sheriff’s Nova Reentry Project. (October 2006 – present). Pilot reentry project that provides case management services to inmates releases from the State prison or the San Francico County Jail.
