IBM Explorys EPM Application Suite

IBM Explorys EPM Suite empowers healthcare stakeholders to measure performance of their accountable providers, and the patients and members they manage. The suite facilitates identification of suboptimal performance and care gaps across hundreds of standardized quality metrics, including those needed for national programs from the Centers for Medicare and Medicaid Services (CMS), National Committee for Quality Assurance (NCQA), among others, and managed care coordination programs in order to predict outcomes around cost and utilization, and allows users to identify potentially avoidable events.

Explorys EPM: Registry allows you to quickly identify your target population and view data that empowers decision making and risk stratified care management. The matrix of rows (patients) and columns (data elements) can be added, removed, sorted, or filtered as needed. – Starter Pack Templates for EPM: Registry includes three registries: diabetes, heart failure, and hypertension. • IBM Explorys EPM: Engage is an integrated solution that plugs into other applications within Platform with EPM Application Suite for managing patient work lists and facilitating workflow management. Prebuilt analytics Registries: Fifteen core registries are available within IBM Explorys Platform with EPM, with the capability to create more. Measure sets: Seventy-five measure sets are available within IBM Explorys Platform with EPM. The measure sets are tools that help track the quality and cost of health care services provided. The measure sets can be defined by public National Committee for Quality Assurance (NCQA) or configurable based on the client requirements defined by the National Quality Forum (NQF) or Centers for Medicare and Medicaid Services (CMS). • SuperMart includes a set of prebuilt tables and schemas designed to meet the demands of value-based care, analytics, and reporting. SuperMart is used to support existing business intelligence tools and frameworks, as well as applications that can benefit from curated data contained within it. This is a production system with access directly to the SQL database. In addition, a SuperMart Extension is available as an add-on application that provides an analytic sandbox for users to extract, transform and load (ETL) their datasets to SuperMart regardless of source.

IBM Explorys Population Assessment, an analytic offering, allows a healthcare system to effectively manage utilization and health of a value-based care population of interest. Using this solution, providers can better understand the pertinent characteristics of their target populations and identify the right initiatives to better manage the health of their patients. IBM Explorys Population Assessment is a cost-effective solution that lets providers more efficiently manage any population in accordance with the Institute of Healthcare Improvement's Triple Aim Initiative by improving the patient experience, improving the health of populations, and reducing the per capita cost of healthcare while significantly shortening time to value. Data regulations and governance

With the governance engine, you can get rules-based access to data based upon the client's data governance policies and conditions, such as organizational role, patient attribution, and contract rights between entities. This enables patient data to be made available to various personnel and functions in appropriate ways consistent with a client's business rules and identified regulatory policies. It also enables the rapid formation of relationships between payers, providers, and plans as models grow and evolve.

All data that enters any aspect of the IBM Explorys offering does so through encrypted data channels using a virtual private network (VPN) that has been established between the Explorys data center that is called the Patient Data Vault (PDV). The PDV is a specially encrypted database (MySQL) that utilizes Advanced Encryption Standard (AES-256) disk encryption. During runtime, IBM Explorys EPM: Measure accesses the PDV using client certificate web services. Access to the PDV and its web services are restricted to registered applications, such as IBM Explorys EPM: Measure and are not available to users directly.

An Item is an occurrence of a specific item that is processed by, managed by, or related to the use of the Cloud Service. Sufficient entitlements must be obtained to cover every Item processed by, managed by, or related to the use of the Cloud Service during the measurement period specified in a PoE or Transaction Document.

Health Care Provider: Health Care provider is a unit of measure by which the IBM SaaS can be obtained. Health Care is the maintenance or improvement of a person's physical or mental health. A Provider is a Health Care institution or person of a Health Care team rendering Health Care services. Sufficient entitlements must be obtained to cover the total number of Health Care Providers managed, processed, or tracked by the IBM SaaS during the measurement period specified in Client's PoE or Transaction Document.

Terabyte: Terabyte is a unit of measure by which the Cloud Service can be obtained. A Terabyte is 2 to the 40th power bytes. Sufficient entitlements must be obtained to cover the total number of Terabytes processed by the Cloud Service during the measurement period specified in Client's PoE or Transaction Document.

Thousand Individuals: Thousand Individuals is a unit of measure by which the IBM SaaS can be obtained. An Individual is a single unique person. Sufficient entitlements must be obtained to cover the total number of Individuals, rounded up to the nearest one thousand, managed, processed, or tracked by the IBM SaaS during the measurement period specified in Client's PoE or Transaction Document.
