The last year has seen major NF research advances. The dovetailing of discovery, translation and the clinic can be seen throughout the meeting. We are firmly in the age of NF clinical trials and proud that the Children’s Tumor Foundation is part of this advance: in 2009 we funded our first two pilot Clinical Trial Awards. We continue to build a pipeline of candidate NF drug therapies through the Foundation’s multi-center NF Preclinical Consortium and the seed grant Drug Discovery Initiative (DDI) program. Through these translational initiatives we are cultivating NF collaborations with the biotechnology and pharmaceutical sector, a critical factor in moving NF research forward to the clinic.     International Journal of Molecular Sciences Review Current Understanding of Neurofibromatosis Type 1, 2, and Schwannomatosis Ryota Tamura Department of Neurosurgery, Kawasaki Municipal Hospital, Shinkawadori, Kanagawa, Kawasaki-ku 210-0013, Japan; [email protected] Abstract: Neurofibromatosis (NF) is a neurocutaneous syndrome characterized by the development of tumors of the central or peripheral nervous system including the brain, spinal cord, organs, skin, and bones. There are three types of NF: NF1 accounting for 96% of all cases, NF2 in 3%, and schwannomatosis (SWN) in <1%. The NF1 gene is located on chromosome 17q11.2, which encodes for a tumor suppressor protein, neurofibromin, that functions as a negative regulator of Ras/MAPK and PI3K/mTOR signaling pathways. The NF2 gene is identified on chromosome 22q12, which encodes for merlin, a tumor suppressor protein related to ezrin-radixin-moesin that modulates the activity of PI3K/AKT, Raf/MEK/ERK, and mTOR signaling pathways. In contrast, molecular insights on the different forms of SWN remain unclear. Inactivating mutations in the tumor suppressor genes SMARCB1 and LZTR1 are considered responsible for a majority of cases. Recently, treatment strategies to target specific genetic or molecular events involved in their tumorigenesis are developed. This study discusses molecular pathways and related targeted therapies for NF1, NF2, and SWN and reviews recent clinical trials which involve NF patients. METHODS The authors conducted a retrospective review of a case series of patients with giant spinal schwannomas at a tertiary cancer hospital. RESULTS Thirty-two patients with giant spinal schwannomas underwent surgery between September 1998 and May 2013. Tumor size ranged from 2.5 cm to 14.6 cm with a median size of 5.8 cm. There were 9 females (28.1%) and 23 males (71.9%), and the median age was 47 years (range 23–83 years). The median follow-up duration was 36.0 months (range 12.2–132.4 months). Three patients (9.4%) experienced recurrence and required further treatment. All recurrenc- es developed following subtotal resection (STR) of cellular or melanotic schwannoma. There were 3 melanotic (9.4%) and 6 cellular (18.8%) schwannomas included in this study. Among these histological variants, a 33.3% recurrence rate was noted. In 1 case of melanotic schwannoma, malignant transformation occurred. No recurrence occurred following gross-total resection (GTR) or when a fibrous capsule remained due to its adherence to functional nerve roots.

Although the world of virtual communication—iPhones, Facebook, e-mail, Snapchat, Skype, Instagram (and much more)—has gained a lot of traction, I am an old-fashioned European who very much believes in authentic human connections. I am convinced that in order to build tangible, working collaborations and turn process into progress, trust is the ultimate key, and it is impossible to build trust with those you do not genuinely know. It is because of this principle that bringing those from the NF community together to mingle, share data, exchange knowledge, share projects and dreams, and simply enjoy one another’s company is very high on the Foundation’s priority list. This is what we aim to accomplish every year with the NF Conference, and this year in Washington, D.C. will be no exception! The Foundation is not alone in its gratitude for the annual NF Conference. At our Research Strategic Planning Retreat last fall, I was very happy to learn that a dynamic group of Board members and experts from both the NF community and other rare disease communities agreed that it should remain a top priority of the Foundation’s—and it will.
