Consultant in Old Age Psychiatry Hospital, Lochgilphead 40 hours per week £80,653 (GBP) to £107,170 (GBP) per annum Tenure: Permanent

This is a new post within the HSCP and is included in a major redesign of service that has seen the development of the community mental health teams throughout the area. The post is devoted to the further development of services for the elderly mentally unwell. This post covers out-patient across and Bute and In-patient cover for the elderly at Mid-Argyll Hospital. It offers an opportunity to work with a small group of committed clinicians who deliver an excellent standard of care. The post is for 10 programmed activities (sessions) per week and the post holder will be welcomed into a friendly and forward thinking team of consultants where peer support is valued and protected.

There will be an opportunity to develop an area wide functional clinic besides coordinating area wide dementia service delivery. This post will cover Out Patient services for Cowal and Bute and the In-patient ward at Mid Argyll Hospital. Participation in teaching will be possible. Junior support is provided for inpatients.

Applicants must have full GMC registration, a licence to practise and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to a CCT in Old Age Psychiatry or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview.

Consultant Psychiatrist Old Age Psychiatry

Introduction This post offers an exciting opportunity to work as part of a team to develop older age services in line with the Scottish Government Dementia strategy. Currently we have an eight bed dementia assessment unit and community staff in each area. This post covers In-patient and out-patient work.

This post is being offered as part of a major redesign of the mental health service throughout Argyll and Bute which will aim to expand community mental health teams and retain a small inpatient campus at Lochgilphead.

The post involves participation in multi-disciplinary team meetings, out-patient work and provision of on call cover for the mental health inpatients in a newly refurbished 21 bedded unit in July 2017 providing a fantastic environment for patients and staff.

The post-holder could be based in the locality or in Lochgilphead and will join an enthusiastic team of consultant colleagues and multi-disciplinary staff. The development of a special interest within psychiatry will be encouraged.

Clinical Duties: Sector team work Work includes psychiatric assessment, overview, developing care plans, mental health act work and working in liaison with GPs. There is a good multi-disciplinary team to work with. It is anticipated that the consultant will work in a modernised consultative role.

Liaison There will be opportunity to liaise with local authority, GP’s, general hospital colleagues, community staff and other agencies, including voluntary bodies.

Management The post-holder will be encouraged to play a role in the planning and development of future services within older age service and also within the hospital. Additional Supporting Professional Activity time could be negotiated for this.

The post-holder will be invited to join in regular management meetings, including monthly formal consultant meetings and monthly peer group reviews. Involvement of clinicians in service provision is encouraged by our mental health service manager, via the Acute In-Patients’ Forum (In-patient services), and Mental Health improvement Group (pan-Highland psychiatric services). Consultants are represented at both meetings by the clinical lead for mental health. Current projects focus on preventing unnecessary admission to hospital by development of appropriate community resources, including use of the community hospitals.

Education and Training Duties include the training of junior medical staff and medical students from Dundee University when attached to the team. There is also the opportunity to participate in the internal academic programme to which the post-holder will be invited to be deliver seminars from time to time.

Research A mental health research strategy is currently being developed in Highland and we have strong links with Aberdeen University, Stirling University, West of Scotland University and the University of the Highlands and Islands. Research educational meetings occur on a regular basis.

Clinical Governance The Clinical Governance Committee meets on a bi-monthly basis and reviews appropriate matters such audit outcomes, ICP development, etc. Clinical governance also features as agenda item in the monthly consultant meetings and also in the redesign board and project groups.

Contractual Obligations Along with other consultants, the post-holder will provide specialist reports as required, eg to the Court, local authority and other agencies, including working as an Approved Medical Practitioner under the Mental Health (Care and Treatment) (Scotland) Act 2003 and the Adults with Incapacity (Scotland) 2000.

On-call responsibilities will be included within the contract, currently at 1 in 8, attracting a 5% availability supplement. The post-holder will supervise and support middle-grade staff and/or doctors in training.

Cross cover arrangements with consultant colleagues for holidays and study leave will be included on a reciprocal arrangement.

CPD NHS Highland supports CPD and attendance at appropriate internal and external educational events is encouraged. We have a departmental peer group to support college CPD certification and an in-house teaching programme.

Supervision Regular supervision of doctors in training and team medical staff is included in the job plan following agreement in Supporting Professional Activities (SPAs). The post holder will be supported to access appropriate professional development to maintain skills and accreditation with professional bodies.

Video linking This facility has existed in Highland for some time and provides a means of link-up throughout the CHP and beyond and is being used successfully for management and clinical purposes for example to input to multi-disciplinary reviews, Care Programme Approach meetings and to support patients on a CBT programme. There are plans for a project to assess the benefit of video-links to support out of hours links to community staff.

Section 22 Approved Medical Practitioner Status The post holder will be expected to have undergone appropriate training to carry out work of this kind, including Mental Health (Care & Treatment)(Scotland) Act 2003 and Adults with Incapacity Act 2000 duties.

Health and Safety Medical staff, employed by NHS Highland, are expected to comply with appropriate health and safety policies.

Current Services Catchment Area and Out-Patient Clinics The hospital has a large rural and partly urban catchment area; serving a total population of approximately 64,000. The area extends from Mid Argyll south-east to Helensburgh; south and west to include the Cowal peninsula and Bute, the Kintyre peninsula and the islands of Gigha, Islay, Jura, Tiree and Colonsay; west and north to include Mull and the mainland extending beyond Oban.

Out-patients are seen at clinics held in Lochgilphead, Dunoon, Rothesay, Tarbert, Inveraray, Campbeltown and Oban. Regular clinics are also held on the islands of Mull, Islay, Coll and Tiree. Junior staff participate in these out-patient clinics which, between them, provide access to a unique mixture of rural and urban psychiatry in Scotland. With the required weekly outpatient clinic commitment, you will be expected to travel efficiently and effectively between various work locations within Highland to meet the operational requirements of the Service and therefore a driving license is essential. NHS Highland operates a leased car scheme, terms of eligibility apply.

Inpatient Mental Health Unit, Mid Argyll Hospital. Inpatient services for General Adult have recently moved to a renovated and modern facility within the Mid Argyll Hospital. It is situated on a hill overlooking the West Highland town of Lochgilphead and, beyond, Loch Gilp and Loch Fyne. Within the hospital complex are all the usual ancillary facilities and also a well-equipped professional library. The Dementia Assessment unit currently has 8 beds. Next door is the general adult mental health unit which currently has 21 beds. Inter-disciplinary activity occurs at all levels of care and there are active hospital departments of occupational therapy and ready access to social work.

In the vicinity of the hospital is the availability of health service accommodation with some staff flats. The CHP management offices are situated on campus and there is thus a convenient concentration of virtually all aspects of the local health service within a relatively compact area.

The HSCP has recently completed a major consultation exercise on redesign of the mental health service. We are now in the process of completing a major service change to enhance community services, retaining a small central inpatient unit and education centre.

Mid Argyll Hospital – current position • Acute admissions: 21 beds (4 of these beds can be used for enhanced care if required)

• Dementia assessment: 8 beds

Other services include: • Sectorised Community Mental Health services • Psychological therapies • Trauma therapies including EMDR • Adult Community Learning Disability service • Adult Community Addictions service • Child and Adolescent Community Mental Health Service (via a service level agreement with Greater Glasgow and Clyde Health Board)

There is no separate service for the functional elderly who are cared for by the sector and acute inpatient teams. There is a developing multi-disciplinary dementia clinical network covering the whole of Argyll and Bute and specialty doctor clinics are held in Dunoon and Rothesay on a fortnightly basis.

All sector consultants provide Court reports if time allows. Access to forensic medium and low secure beds is available via a service level agreement with Greater Glasgow and there is direct access to Rowanbank medium secure unit at Stobhill Hospital, Glasgow, or the State Hospital at Carstairs if required. Both the West and North of Scotland forensic networks provides peer support and access to regular training and educational events.

There is a service level agreement with Greater Glasgow for access to the inpatient child and adolescent units and also the specialist mother and baby unit at the Southern General Hospital.

Inpatient care of people with life threatening eating disorders and psychiatry of LD is currently provided by the private sector.

Academic Activities The hospital has full approval by relevant bodies to train psychiatric trainees, trainees in General Practice and Foundation Year 2 trainees. A regular programme of in-service training is provided within the hospital itself. This takes the form of weekly seminars, journal clubs and case conferences held during three 10-week terms each year.

Research Activities Research topics vary from year to year, but in general they have come within the category of biological psychiatry.

Grade and salary Consultant: £80,653 to £107,170 per annum (GBP) New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
