The directory is the result of an ongoing strategic relationship between the Holbrook and Northside Hospital Cherokee. We feel the information contained within will be a tremendous benefit to our residents, and it is a pleasure to provide our community access to the best medical care all conveniently located in Cherokee County. This directory provides information on multiple specialties tailored to the needs of the senior population. You will find that each specialty has a detailed description of the medical practice, including services offered, office location and contact information, and a brief biography of each physician. Sincerely, Brad Bickford Executive Director Holbrook at Woodstock DIRECT ACCESS PRACTICES What is the Northside Hospital Cherokee Direct Access Program – • Our goal is to provide community members with the ability to connect with their Northside specialty support physicians quickly upon identification of illness/issue in order to prevent unnecessary Emergency Department visits/hospitalizations. • Provides quick accessibility (48-72 hours) for sick or post hospital visits. How to use Direct Access Program – • Refer to Direct Access Table of Contents - page 5 • Call the practice office during standard business hours. (M-F 8am-4pm, excluding holidays) • Inform staff you live in Holbrook and need to schedule a sick or hospital follow up visit. • Staff will schedule appointment within 48-72 hours • For all well, routine, and follow up visits please schedule through normal office scheduling protocols. If you have an emergency, please call 911. If I have any/all issues with Direct Access Program, whom should I contact- • Any concerns, comments and suggestions are welcomed. Residents are asked to email [email protected]. Comments are reviewed weekly. Additional Outpatient Physician Practices These practices are not a part of the Direct Access Program, however, we felt it necessary to include into this directory for your convenience. • Refer to Table of Content-page 45 • You may call any of these practices Monday-Friday, 8am-4pm, to schedule an appointment through normal office scheduling protocols. Azar, Jr., MD Dr. Gordon J. Azar, Jr. is a board certified physician in Cardiovascular Disease. A native of Atlanta, Dr. Azar has nearly three decades of expertise serving patients in the Greater Atlanta area as a founding member of Cardiovascular Physicians or North Atlanta. Dr. Azar enjoys building relationships with each patient, and treats them using the latest in evidence-based medicine, personalizing care for each patient’s specific needs. Bapat, MD Amol S. Bapat, MD, F.A.C.C., F.S.C.A.I. is a board certified physician in Cardiovascular Disease, Nuclear and Interventional Cardiology. He was born in Birmingham, Alabama. He attended Johns Hopkins University in Baltimore, Maryland where he received his B.S. in Chemistry. He then proceeded to the University of Alabama School of Medicine, and after receiving his MD, pursued his Internal Medicine Residency at the Baylor College of Medicine. He stayed at Baylor for his subspecialty training in Cardiovascular Disease and Interventional Cardiology. While at Baylor, he was the recipient of a NIH National Research Award Training Grant and his research involved platelet and thrombosis physiology. He was the first physician to work with the Larry King Cardiac Foundation (LKCF) and its partnership with Saint Joseph’s Hospital. He was awarded the Physician Laureate Award by the LKCF in 2006. Dr. Bapat has had the distinction of serving as Director of the Congestive Heart Failure Clinic at St. Joseph’s Hospital of Atlanta, on the Board of Directors of the St. Joseph’s Hospital Heart and Vascular Institute, Vice-Chairman of the Peer Review Committee at St. Joseph’s Hospital and as Vice-Chairman

This year w as no G e o rgia - 1 0 different but as I attended many sessions, speakers, CE’s, and other events my ex - perience was tempered by comparing the scope and quality of MLA against our up - Mississippi - 1 1 coming chapt er meetin g in Novem ber. The re is no thing like participating in a South C arolina - 1 1 process to fully apprec iate the creat ivity, logistic s, time , and energy required to put it together and make it work.
