Supplemental Application Procedures Dates and Deadlines
In addition to the UC Application for Admission and Scholarships, which must be submitted by November 30, the majors listed below require supplemental materials that you must submit directly to the department.
Note: you should submit these materials only if you have selected one of these programs as your primary major at UCLA. Applicants who selected these programs as their alternate majors will be reviewed for their primary major only.

UCLA Herb Alpert School of Music Supplemental Application Information Music Composition Supplemental Application All applicants to the Music Composition major are required to submit a supplemental application. Access to the supplemental application will be provided to applicants via email on a rolling basis following the submission of the UC Application. The supplemental application includes the following components:
Personal statement Unofficial transcripts Performance resume (optional) Letters of recommendation (optional) Pre-screening portfolio
The submission deadline for the supplemental application is Wednesday, December 11, 2019 (11:59pm PST)
Music Education Supplemental Application
All applicants to the Music Education major are required to submit a supplemental application. Access to the supplemental application will be provided to applicants via email on a rolling basis following the submission of the UC Application. The supplemental application includes the following components:
Personal statement Unofficial transcripts Performance resume (optional) Letters of recommendation (2-3 letters required for applicants to Cello, Viola, Violin, and Voice; optional for all other applicants) Pre-screening audition (required for applicants to Cello, Clarinet, Classical Guitar, Flute, French Horn, Piano, Saxophone, String Bass, Trombone, Trumpet, Viola, Violin, and Voice)
The submission deadline for the supplemental application is Wednesday, December 11, 2019 (11:59pm PST).

All applicants to the Music Performance major are required to submit a supplemental application. UCLA School of Nursing Supplemental Application Information All applicants must submit the appropriate online UCLA School of Nursing Supplemental Application which includes a Statement of Purpose, Resume and two Recommendation Forms by January 15th. Please allow 3-5 days after completing your UCOP application for the information to be synced to the Supplemental Application. Transfer applicants have the option to mail official transcripts now or wait until the School of Nursing sends a request for them. Complete the Undergraduate Theater Supplemental Application.
Supplemental material deadline: December 9, 2019 A supplemental application fee of $20 will be required. Create an account with Acceptd (https://app.getacceptd.com/uclatheater) o Select Program: Indicate Program (Acting, Design/Production, Integrated Studies, Musical Theater). Indicate Area of Emphasis within Theater. Upload Supplemental Materials Personal Insight Questions. Resume 1. Resume 2 (Optional). Photo Area of Emphasis Materials. Unofficial copies of all Transcripts.
INTERVIEW/AUDITION TOUR Applicants will be contacted ONLY if more information is needed and an Interview/Audition will be required. (An Interview/Audition is not always required for admittance. Therefore, not all Applicants will be contacted.) Please go to the UCLA Department of Theater Interview/Audition site for more information and a schedule of dates.

Film Supplemental Application Complete the TFT Undergraduate Supplemental Application.
Must be completed ONLINE by January 13, 2020. o A supplemental application fee of $20 will be required. o Create an account with Acceptd. (https://app.getacceptd.com/uclafilmtv) o Complete all required information. o Indicate areas of interest within Film.
o Select the GE Plan completed. For information on the TFT General Education requirements, visit http://www.tft.ucla.edu/ge. o Upload Supplemental Materials. . Personal Essay (2 page max). . Life Challenge Essay (2 page max) . Critical Essay (3 page max). . Creative Writing (5 page max). o Submit two letters of recommendation. . Enter the names and emails of all recommenders into the film Undergraduate Supplemental Application o Unofficial copies of all transcripts.
Important Information
By the time of entrance, Transfer Applicants must: Have at least a 3.2 GPA. Satisfy the University of California’s General Transfer Admission Requirements. Complete a minimum of 60 semester/90 quarter units of transferable work. Complete ONE of the following General Education Plans: o UCLA School of Theater, Film and Television GE Requirements. o Another UC campus GE Requirements. o Intersegmental General Education Transfer Curriculum (IGETC) at a California Community College.
