Dear colleagues, If an immediate family member within Your Household fits into the JCVI 1- 4 criteria cohorts (over 70 years old, clinically vulnerable or works in Health and Social Care), and has not yet had their vaccine we are able to offer appointments to allow you to book their COVID-19 vaccine at any of the MFT vaccination clinics. The definition of Your Household includes either: 1. Someone who lives in your house. This includes, but is not limited to: - Your partner - Your adult child - Extended family members (parents, grandparents etc.) - Housemates or others who may or may not be related but live with you 2. Someone who is a family member who is in your ‘support bubble’ but does not live in your house but based in Greater Manchester. This includes, but is not limited to: - Extended family members - Single parent Use the online booking link https://mft.nhsbookings.com/v2/ to book your eligible family members into a vacant slot at one of the clinics. Choose ‘MFT Affiliates’ and ‘Your Household’ from the dropdown options. Then print off a copy of this letter, fill out the details requested and sign it. Your family member must bring this confirmation letter to their appointment as proof of eligibility for vaccination, as well as a form of personal ID (e.g. passport, driving license), otherwise they will not be able to receive the vaccine.
