When the needs are com- pared to the country’s response capacity, this disaster was truly unprecedented. The level of response, especially in the health sector, was generous, even overwhelming. Information was scarce, decisions were often not evidence-based, and there were serious gaps in overall or sectoral coordination. This book presents lessons to be learned from Haiti with the aim of improving the health sector’s January 2010 response in major, sudden-onset disasters in the future. It also identifies opportunities provided by the disaster for making significant changes in health services in Haiti. One of the key lessons of the Haiti tragedy is that coordination can only be effective where national authorities are equipped to assume leadership and establish relief and recovery priorities. Lessons to be learned for the next The authors have drawn on their own extensive experience in international disaster management, massive sudden-onset disaster and synthesized material from reports, evaluations, peer-reviewed scientific publications, and over 150 interviews. A review group was convened by PAHO/WHO to corroborate the findings and conclusions of this publication. The Haiti earthquake is no exception. This publication, for practical reasons, focuses on the first three months of the response. A similar study might be needed for the mid- and long-term recovery and reconstruction process that occurs after the first three months. The key lessons outlined in this publication are not merely the views of the author, Claude de Ville de Goyet, and co-authors, Juan Pablo Sarmiento and François Grünewald. They reflect the collective vision of a large number of partners and experts. International experts of the review group made significant contributions to the findings and conclusions of this important publication. The 12 January 2010 earthquake could not have occurred in a more vulnerable environment than the capital of Haiti. The immediate health impact of the earth- quake in absolute terms— number of dead and injured—was among the highest in recent times. When the needs are compared to the national response capacity, this disaster was truly unprecedented. The international community responded rapidly with an outpouring of generos- ity. Beyond traditional global donors, relief came from Haiti’s immediate neigh- bors, as well as from every country in Latin America and the Caribbean. Such support offered an encouraging example of solidarity in the true spirit of Pan- Americanism. If the impact was unprecedented, the organization of the response was not. It fol- lowed the same chaotic pattern as in past disasters. Information was scarce, deci- sions were often not evidence-based, and overall sectoral coordination presented serious shortcomings. Management gaps noted in past crises were repeated and amplified in Haiti. The humanitarian community failed to put in practice the lessons learned. The “proliferation of actors”, to borrow an expression used in the evaluation of the tsunami, ensured a massive offer of services from competent partners. The overwhelming number of people in need of immediate assistance ensured that each minimally prepared and equipped health responder provided valuable

Rix, M.EERI The 12 January 2010 Mw 7.0 earthquake in the Republic of Haiti caused an estimated 300,000 deaths, displaced more than a million people, and damaged nearly half of all structures in the epicentral area. We provide an overview of the historical, seismological, geotechnical, structural, lifeline-related, and socioeco- nomic factors that contributed to the catastrophe. We also describe some of the many challenges that must be overcome to enable Haiti to recover from this event. Detailed analyses of these issues are presented in other papers in this volume. [DOI: 10.1193/1.3630129] INTRODUCTION On 12 January 2010, at 4:53 p.m. local time, a magnitude 7.0 earthquake struck the Republic of Haiti, with an epicenter located approximately 25 km south and west of the cap- ital city of Port-au-Prince. Near the epicenter of the earthquake, in the city of Le´ogaˆne, it is estimated that 80%–90% of the buildings were critically damaged or destroyed. The metro- politan Port-au-Prince region, which includes the cities of Carrefour, Pe´tion-Ville, Delmas, Tabarre, Cite Soleil, and Kenscoff, was also severely affected. According to the Govern- ment of Haiti, the earthquake left more than 316,000 dead or missing, 300,0001 injured, and over 1.3 million homeless (GOH 2010).     Perspectives Mental Health in Haiti: Beyond Disaster Relief Olivia Tiberi School of Medicine, Imperial College London Haiti, one of the poorest countries in the world, was devastated by an earthquake in 2010. Te disaster uncovered the realities of a non-existent mental health care system with only ten psychiatrists nationwide. Attempts were made to assess the increased prevalence of mental illness, likely due to the trauma to which many were exposed. Several interventions were carried out with aims to integrate mental health into primary health care services. Te interplay between socio-cultural beliefs and health (both mental and physical) in Haiti has been widely commented upon by both foreign aid and local caregivers. Observations frequently highlight barriers to the willingness of patients to seek care and to their acceptance of biomedicine over traditional Vodou beliefs. Te perception of Haitian beliefs as barriers to the availability and acceptance of mental health care has intensifed the difculty in providing efective recommendations and interventions both before and after the earthquake. Argued in this review is the importance of considering the interactions between socio-cultural beliefs and mental health when developing models for the prevention, screening, classifcation and management of mental illness in Haiti. Tese interactions, especially relevant in mental health care and post-disaster contexts, need to be acknowledged in any healthcare setting.     CENTER FOR EXCELLENCE IN DISASTER MANAGEMENT AND HUMANITARIAN ASSISTANCE (CFE-DM) CASE STUDY SERIES Case Study No. 6 • A review of Operation Unified Response, the U.S. military’s effort in support of Haiti following the January 2010 Earthquake Introduction On January 12, 2010, a massive earthquake struck southwest of Port-au-Prince, the capital of Haiti, the poorest country in the Western Hemisphere. The magnitude 7.0 earthquake,1 the strongest to strike Haiti in 200 years, lasted for 35 seconds, killing 230,000 people, injuring 300,000 and displacing 1.6 million.2 The day following the earthquake, U.S. President Barack Obama ordered a whole of government response to the disaster. The Office of Foreign Disaster Assistance (OFDA), part of the U.S. Agency for Interna- tional Development (USAID) led the massive undertaking, marshalling the collective resources of the entire U.S. interagency community, to include the U.S. Department of Defense. Ultimately, over 22,000 members of the U.S. armed services participated in Operation Unified Response, the name of the U.S. military’s effort in support of the Haitian people. “I have directed my administration to respond with a swift, coordinated, and aggressive effort to save lives. This particular period of the twentieth century is not only crucial to study since it presents the continuing conflict between the dominant white heteronormative society and subjugated minority cultures but, more importantly, because the Enlightenment’s libertarian ideal of individual freedom finally encouraged non-conformists of diverse backgrounds such as gender, race, and sexuality to take a collective stance against oppression. At the same time, it is important to stress that the cultural production of these popular texts emerged from and within the conditions of American culture rather than the native context of Haiti. Hence, Vodou in these American popular texts is subject to cultural appropriation, a paradigm that is broadly defined as the use of cultural practices and objects by members of another culture.

    HAITI NEWS ROUNDUP: OCTOBER 6 – 24, 2005 International donors half-way to fulfilling Haiti aid pledge Caribbean Net News Monday, October 24, 2005 BRUSSELS (AFP): International donors have given Haiti some 600 million dollars so far to see it through a transition phase and help its next government, they said in a joint declaration on Friday. The money is half the amount of one billion dollars promised under the interim cooperation framework (CCI) aimed at meeting the priorities of the violence-scarred and impoverished island state. "The meeting in Brussels has allowed us to underline that the engagements undertaken in respect of Haiti are confirmed and will be respected," the joint statement said. The donors also said they would extend the cooperation framework by a year until the end of 2007 to allow the new government, due to take office in February, "the time and the means to continue reforms." A new conference will take place at the end of next year to "mobilise the additional finances necessary for this extension." Haiti and the donors at the two-day conference in Brussels also reaffirmed that the "transfer of power to a new elected government must happen in line with the timeframe set out in the constitution; that is February 7, 2006." At a press conference earlier, Prime Minister Gerard Latortue promised that long-delayed general elections in Haiti, the first since the fall of president Jean Bertrand Aristide, would start in December. "The first round of the presidential and legislative elections will take place in December, probably in the first half of the month," he said.
