Richman, Vice President of Philanthropy Program Director Milford Regional Medical Center 508.422.2236 | [email protected] Nicole Heim, Chief Information Officer Program Manager Information Systems, Milford Regional Medical Center 508.422.2971 | [email protected] The Milford Regional Medical Center (MRMC) will use the Mass HIway to transmit hospital discharge summaries to two facilities: Medway Country Manor, a skilled nursing facility close to full implemen- tation of an electronic medical record; and Care Tenders, a home health agency currently not using an electronic medical record. By partnering with two different types of trading partners, MRMC can establish a process of connecting to the Mass HIway that could be replicated with other agencies in the future. MRMC will use the grant to accelerate the connection of their trading partners to the Mass HIway by providing funding for their initial set-up on direct webmail HIE services and any needed tech- nical support. Currently, hundreds of discharge summaries are faxed to home health agencies, skilled nursing facilities, primary care offices and other providers, creating a situation that is highly inefficient and undesirable for confidentiality of patient information or timeliness of delivery. The primary goal of this project is to accelerate MRMC’s connection to the Mass HIway, and to facilitate that connection for their two trading partners. Their secondary goal is to successfully transmit transactions of patient discharge summaries to both trading partners, with the overarching aim of improving patient care, by creating a reliable communication pathway between the hospital and providers in the community. MRMC will measure their success by the number of organizations enabled for directed exchange and the number of successfully directed transactions. In addition, they will conduct a short survey at the end of the grant period with both trading partners to evaluate the process and capture any recommen- dations for future implementation. Using a three-phased approach, this exchange will improve communication between laboratory services and providers to enhance continuity of care, by: 1) identifying the lab result data elements in the MRMC Meditech Lab System; 2) transferring the data via HL7 from Meditech Lab to the Mass HIway; and 3) accepting the data into a UMMHC application functioning solely within the UMMHC system. Currently, when UMMHC patients have surgical procedures, lab tests and other work performed at MRMC, UMMHC providers are not able to the view test data performed there, resulting in the possibility for duplicate testing and missed opportunities for care. This project will be the first step in enabling the data exchange between two collaborative, non-affiliated trading partners that will allow MRMC to share the lab result data of UMMHC patients with UMMHC, allowing for a more complete picture of the patient experience. This data exchange holds the key to more substantive medical records, less duplication of services, reduction in emergency department visits, more informed providers and care givers, all leading to cost reduction and improved quality of care. The success of this project will be measured by the number of UMMHC laboratory results transferred from MRMC to UMMHC. Using a three-phased approach, this exchange will enhance patient care and lower overall system costs: 1) identify the continuity of care elements in the CVSMC electronic healthcare record system and the UMMHC dbMotion (dbMotion) application; 2) test the export data from one system to the Mass HIway and receipt of the data on the other system, ensuring both systems can send and receive patient care data; and 3) implement and monitor the production transfer of data from the CVSMC system to UMMHC dbMotion, a health information exchange application functioning solely within the UMMHC system. Cur- rently, UMMHC does not electronically exchange patient data with outside provider systems. As UMMHC moves into Value Based Healthcare, the ability to exchange information to these low cost, efficient and highly patient-centered sites will enhance patient care and lower overall system costs. The Mass HIway will allow for continuity of care, giving providers a more complete picture of the patient, and patients ease of access to quality care when needed. CVSMC and UMMHC will work towards enhanced integration of electronic healthcare record systems to streamline communication around all aspects of a patient’s care. UMMCH will measure the success of this project by the increased access and number of two-way validat- ed, exchanged patient care records during the grant period. UMMHC and CVSMC expect the increase in available patient data will improve the quality of patient care, while reducing the cost. Front Street, 3 FL New Bedford, MA 02740-7327 Collaborating Massachusetts Department of Mental Health Organizations Netsmart Technologies Amount of Award $55,143 Daniel Mumbauer, President & CEO Program Director 508.997.0765 | [email protected] Brad Scott, Chief Information Officer Program Manager 774.628.1088 | [email protected] The High Point Treatment Center (High Point) will be the first agency in Massachusetts to extract EIM/ESM enrollment/disenrollment data from their Electronic Medical Record (EMR) and electronically send it through the Mass HIway to the Department of Public Health (DPH). High Point will use a Mass HIway connection to exchange the data from High Point’s EMR directly to the electronic storage of DPH, bypassing the need to re-enter any information through DPH’s portal process. A secure encrypted con- nection will transport the data from High Point through an interface server transforming the data to HL7 format, which will cross the Virtual Private Network (VPN) tunnel and be stored within DPH servers. Each transaction will occur upon every enrollment or disenrollment by clinicians or other support staff handling the information required by DPH. The collective participating agencies include DPH, High Point, and Netsmart Technologies. DPH will work with High Point to make sure all EIM/ESM information is being collected correctly and securely. High Point will be providing the technology and assisting with gathering the data to transport. Netsmart will design the interface and database matching to transfer data through the Mass HIway. The goal is to allow real-time transfer of EIM/ESM data through the Mass HIway to be stored at DPH. This storage will allow other agencies to participate in this data exchange and provide out- come measurements, trends, comparisons and other statistical data to companies so they can improve their services.
