Finally, I would like to thank all of the people who gave their time to participate in this research, without whom this work would not be possible. ii Summary Sleep loss may act as a trigger or early warning sign of manic episodes in individuals with bipolar disorder (BD) but the nature of this relationship remains unclear. The aim of this thesis was to explore the relationship between sleep disruption and mania in individuals with BD. The datasets used in this thesis were obtained from the Bipolar Disorder Research Network (BDRN), an ongoing research programme of over 6000 individuals with mood disorders recruited from across the UK. Psychiatric diagnoses were determined using a semi-structured diagnostic research interview and case notes. First, in 3140 BDRN participants with bipolar-I disorder (BD-I) or bipolar-II disorder (BD-II), I found that 20% of participants reported that sleep loss had triggered episodes of high mood. This was more commonly reported by individuals with BD-I than those with BD-II, and more commonly reported by women than men. Second, I found that women were more likely to have experienced episodes of mania or psychosis after childbirth (termed postpartum psychosis, PP) if sleep loss had triggered episodes of high mood. This effect suggested that a tendency for sleep loss to trigger episodes of high mood might be associated with vulnerability to PP. Third, in BDRN participants who had used an online mood monitoring system to track symptoms of mania and depression, I found that participants could be grouped into three classes based on their trajectories in symptoms of insomnia prior to episodes of high mood. Finally, I designed and conducted a pilot study using actigraphy to measure perinatal sleep in pregnant women at high risk of developing PP. I found that this methodology was challenging to implement in this population but can produce detailed information on sleep during the perinatal period. It was a retrospective medical record review of 42 children. We investigated demographic information, sleep pattern, sleep environment, and the mother’s dysfunctional beliefs about the child’s sleep. The mean age of subjects was 6.3± 3.1. The diagnosis was night terror (n = 21), sleepwalking (n = 8), confusional arousal (n = 2), and unspeci- fied (n = 11). The average time of sleep pattern was as follow; bedtime 21:39± 0:54 pm, sleep onset time 22:13 ± 0:54 pm, wake-up time 7:37 ± 0:42 am and NREM-related parasomnia occurrence time 1:09 ± 2:04 am. The average number of co-sleeping members was 2.8. 48.5% (n = 16) mothers experienced coldness while sleeping, and 64.7% (n = 22) parents had dysfunctional beliefs about their children’s sleep. The large number of co-sleeping members, coldness mothers experienced while sleeping, and dysfunctional beliefs about their children’s sleep may influence the NREM-pa- rasomnia in children. The rise and fall of the diagnosis of functional psychoses: an essay Per Bergsholm Abstract Background: The categories of functional psychoses build on views of influential professionals. There have long been four main categories – affective, schizophrenic, schizoaffective/cycloid/reactive/polymorphic, and delusional/ paranoid psychoses. The last three are included in “psychotic disorders”. However, this dichotomy and the distinctions between categories may have been over-estimated and contributed to lack of progress. Ten topics relevant for the diagnosis of functional psychoses: 1. The categories of functional psychoses have varied with time, place and professionals’ views, with moving boundaries, especially between schizophrenia and affective psychoses. 2. Catatonia is most often related to affective and organic psychoses, and paranoia is related to grandiosity and guilt, calling in question catatonic and paranoid schizophrenia. Arguments exist for schizophrenia being a “misdiagnosis”. 3. In some countries schizophrenia has been renamed, with positive consequences. 4. The doctrine of “unitary psychosis”, which included abnormal affect, was left in the second half of the 1800s. 5. This was followed by a dichotomy between schizophrenia and affective psychoses and broadening of the schizophrenia concept, whereas affective symptoms were strongly downgraded. 6. Many homogeneous psychoses with mixtures of schizophrenic and affective symptoms were described and related to “psychotic disorders”, although they might as well be affective disorders. 7. Critique of the extensive schizophrenia concept led to, in DSM-III and ICD-10, affective symptoms being exclusion criteria for schizophrenia and acceptance of mood-incongruent psychotic symptoms in affective psychoses.
