1. Do you offer financial assistance and/or payment plans? If so, how do I sign up?

2. Can I speak with someone prior to your appointment, surgery, etc. to receive a quote for the anticipated cost of services provided?

3. Do you know of local financial resources available to patients?

4. Do you have transportation partners that help facilitate rides to and from physician, imaging, lab appointments? (i.e. Uber, Lyft, Kaizan Health, etc.)

5. I’m coming in from out of town, do you work with any local hotels to offer patients and families discounts?

6. Are there any additional resources for cancer patients and their families?
