Clinical Investigation of Gastrointestinal Function went into a second edition with co-­worker Malcolm Bateson in 1981. This became Clinical Investigations in Gastroenterology in 1988 (2nd Edition 1997), taking into account new struc- tural, isotope and kinetic tests used in actual practice in Scotland and North East England. Times have moved on. There is now an emphasis on low-risk non-invasive tests, though many patients will always require endoscopy, intubation and biopsy for adequate assessment. 1. The widespread use of anti-Helicobacter pylori therapy in dyspepsia has dra- matically changed the prevalence and significance of this organism in peptic ulcers. 2. Multichannel intraluminal impedance (MII) with high resolution manometry (HRM) to locate accurately the lower oesophageal sphincter has joined pH monitoring in the assessment of gastro-oesophageal reflux. 3. Frequent self-diagnosis by patients of gluten and wheat intolerance, and conse- quent adoption of partial “gluten-free” diets prior to evaluation, is now com- mon and may lead to equivocal results of tests. 4. Conversely, the increasing ethnic minority populations have an appreciable prevalence of alactasia and are often aware of milk intolerance, which is easy to verify if required. 5. Magnetic resonance cholangiopancreatography (MRCP) is now the optimal system for imaging the bile ducts and pancreatic ducts. 6. ERCP is no longer a first-line diagnostic test, but has ceded place to the much safer MRCP and endoscopic endosonography (EUS). ERCP is now generally reserved for patients where therapeutic procedures are planned. 7. Calprotectin assay screening of stool may save some irritable bowel patients unnecessary investigation for inflammatory bowel disease (IBD). Calprotectin measurement is useful to assess activity in asymptomatic Crohn’s disease and in predicting response of IBD to treatment and its risk of relapse. 8. Video colonoscopy and virtual computed tomography (CT) colonoscopy have become frequently used techniques in screening programs in apparently healthy v vi Preface populations and in surveillance and management of polyps, ulcerative colitis and large bowel cancer. 9. Liver imaging now includes transient elastography, as well as MRI, CT, isotope scanning, ultrasonography (US) and laparoscopy. 10. Non-alcoholic steatohepatitis (NASH) as a cause of liver dysfunction and cir- rhosis in the very obese is now well recognised and is an extra incentive to stay slim. 11. Ideas on antibiotic prophylaxis are now quite different and often the reverse of what was previously believed. The focus is again on those tests which have been useful in modern patient care, with special emphasis on those of which the authors have personal experience. Reference is also made to newer tests used elsewhere and to traditional procedures which have historical significance and may still be needed in less well-equipped regions. Information is provided about some investigations which help under- stand disease patterns, without necessarily being required for individual patient management. A subset of patients, however, have long-term dysfunctional stooling, characterized by either frequent soiling or obstructive symptoms. The evaluation and management of a child with poor function after pull-through for Hirschsprung disease should be conducted by an experienced multidisciplinary team. A systematic workup is focused on detecting pathologic and anatomic causes of pull- through dysfunction. This includes an exam under anesthesia, pathologic confirmation including a repeat biopsy, and a contrast enema, with additional studies depending on the suspected etiology. Obstructive symptoms may be due to technique-specific types of mechanical obstruction, histopathologic obstruction, or dysmotility—each of which may benefit from reoperative surgery. The causes of soiling symptoms include loss of the dentate line and damage to the anal sphincter, which generally do not benefit from revision of the pull-through, and pseudo-incontinence, which may reveal underlying obstruction. A thorough understanding of the types of complications associated with various pull-through techniques aids in the evaluation of a child with postoperative dysfunction.

    STUDIES ON THE BREAKING PATTERN IN MAN AT REST AND DURING SLEEP by Steven Andrew Shea A thesis submitted to the Faculty of Science, University of London for the degree of Doctor of Philosophy 1988 Department of Medicine, Charing Cross and Westminster Medical School, London. 2 ABSTRACT . This thesis quantifies the breathing pattern and the extent of the reproducibility of this pattern within an individual at rest and during sleep. From breath-by-breath measurements of respiratory frequency, tidal volune and end-tidal POO2 made under standardised conditions of relaxed wakefulness - with a minimum of sensory stimulation - the results show that differences between individuals are highly significantly greater than differences seen on repeated measurements within an individual: people tend to breathe in a reproducible and characteristic fashion. The basic respiratory pattern is shown to have long-term reproducibility for periods of up to 5 years and may be, to some extent, inherited since it is shown to be similar between identical twins. The individual’s ’respiratory personality’ also persists during deep non-rapid eye movement (non- REM) sleep when any forebrain influences upon breathing are minimal. Further studies, using similar techniques, examine the effect upon this basic respiratory pattern of some behavioural, metabolic and pulmonary reflex control mechanisms. These studies reveal that visual, and auditory stimulation, and altered cognitive activity (performing mental arithmetic) affects the pattern of breathing; principally by increasing respiratory frequency. However, these changes in breathing which occur between the different ’states’ are not solely behavioural responses since they are also related to increases in cerebral and/or somatic metabolism.

Five patients with progressive fibrotic lung disease are described. The dominant symptom was slowly increasing dyspnoea, and cough and sputum were not prominent. Marked weight loss was also a feature. There was severe restrictive impairment of ventilation with normal arterial gas tensions. The changes were confined to the upper parts of the lung in some but others had more generalized disease. The duration has varied so far from two to 17 years. The lung changes are considered to be due to dense progressive fibrosis. Necropsy in two confirmed this. Histologically there was monotonous fibrosis with lymphoid collections and secondary bronchiectasis, a picture similar to that found in association with ankylosing spondylitis. None of these patients had joint disease. Tuberculosis was excluded as a cause by exhaustive bacteriological tests and the failure of chemotherapy to stop deterioration. All other recognized types of infective and non-infective progressive lung fibrosis were also excluded, and this is not considered to be a variant of cryptogenic fibrosing alveolitis. Though these patients have many features in common they do not necessarily have the same pathogenesis. They are http://thorax.bmj.com/ presented as an encouragement to further study. Expert consensus was then used to summarize the current state of knowledge regarding causes, methods of diagnosis, and treatment approaches to children with soiling symptoms following pull-through for Hirschsprung disease. Causes of soiling after pull-through are broadly categorized as abnormalities in sensation, abnormalities in sphincter control, and “pseudo-incontinence.” A stepwise algorithm for the diagnosis and management of soiling after a pull-through for Hirschsprung disease is presented; it is our hope that this rational approach will facilitate treatment and optimize outcomes. They commonly present with overlapping symptoms indicating that their pathophysiology affects multiple segments of the gut as well as brain and gut interactions. Clinically, although some conditions are readily recognized, dyssynergic defecation, fecal incontinence, and anorectal pain are often missed or misdiagnosed. Consequently, the assessment of lower gastrointestinal symptoms in patients with suspected colonic or anorectal motility disorder(s) remains challenging for most clinicians. A detailed history, use of the Bristol stool form scale, prospective stool diaries, ideally through a phone App, digital rectal examination, and judicious use of complementary diagnostic tests are essential. Additionally, it is important to evaluate the impact of these problems on quality of life and psychosocial issues, because they are intricately linked with these disorders. The Rome IV diagnostic questionnaire for functional gastrointestinal disorders can provide additional information often missed during history taking. Here, we discuss a systematic approach for the clinical evaluation of patients with suspected lower gastrointestinal problems, grouped under 4 common diagnostic categories. We describe how to take a detailed history, perform meticulous digital rectal examination, and use validated tools to supplement clinical evaluation, including assessments of quality of life and scoring systems for disease severity and digital Apps.

The aim of this study was to evaluate enteropathy symptoms and anorectal function using high-resolution anorectal manometry (HRAM). Fifty DM patients and 20 non-DM controls were enrolled into the study. Clinical data and laboratory tests were collected, physical examination and HRAM were performed. Symptoms in the lower gastrointestinal tract were reported by 72% of patients. DM patients with a long disease duration reported anal region discomfort (p = 0.028) and a sensation of incomplete evacuation (p = 0.036) more often than patients with shorter diabetes duration. Overall, DM patients had a lower maximal squeeze pressure (MSP) (p = 0.001) and a higher mean threshold of minimal rectal sensation (p < 0.01) than control subjects. They presented with enhanced features of dyssynergic defection than the control group. MSP and maximal resting pressure (MRP) were significantly lower in the group of long-term diabetes (p = 0.024; p = 0.026 respectively) than in patients with a short-term diabetes. Patients were divided into two groups. Group 1 (37 patients): functional faecal retention, group 2 (10 patients): functional faecal soiling without retention. Ganglion cells or normal acetylcholinesterase staining, or both, was demonstrated in all cases. Normal inhibition of internal sphincter could be achieved by rectal distension in all except 2 children with severe constipation. Resting sphincteric pressures, pressure responses, and conscious rectal sensitivity thresholds were similar in groups 1 and 2, but were increased compared with controls. In group 1 alone, the critical volume increased parallel with conscious rectal sensitivity threshold. Since the complete relaxation of internal sphincter occurs before conscious rectal sensation arises in children with soiling without retention, this may be an important factor, at least in some of the soilers. Between 10 and 20% of children referred for (n=2), preoperative ureter implantation to bowel gastroenterological consultation have disorders of (n= 1), asymptomatic diastematomyelia (n= 1), defecation.
