About the job You will be working with a dedicated manager, a small team of partner organisations and a range of stakeholders. You will be required to create and deliver a communications and promotion plan, with a clear social media element, research and compile a comprehensive database of EBP opportunities, plan events, co-design a service web site and keep it up to date, oversee and input to the Customer Relationship Management (CRM) system, assist the manager in compiling labour market information, statistics and have a keen eye on the world of work and what employers need from young people coming through education. You will have a mix of office-based work with face to face contact with businesses, agencies, educationalists and young people. About you We’re looking for someone who wants a varied and challenging co-ordination role. You need excellent organisational skills, the ability to be self-motivated, imaginative and creative, a good team player and able to multi task. You will need to be able to work flexibly to suit the requirements of the role including some work outside of office hours in the delivery of the role. Excellent communication skills are required. You will need to be familiar with IT and administrative duties. Office experience is essential. Knowledge of existing EBP services in other areas is not essential, but the core skills required to operate websites, communications campaigns and project co-ordination with the aptitude to learn is vital to being a success in this very rewarding role. About the Job You will work a flexible shift pattern across 7 days a week, Mon-Fri 8.30am-8pm and Sat and Sun 9am- 4pm with your future colleagues who are friendly and very supportive. The 7 day service is due to begin operations in October so you will have the opportunity to work hours that suit you as long as the new out of hours service is covered. We will be offering pay enhancements for weekend working. Working as part of the multi-disciplinary team, supporting people to get back to their home whenever possible and as quickly as possible as soon as they are able. You will be expected to work with patients, families and our community networks to agree creative solutions to help people return and remain at home. You will work closely with health colleagues to provide better outcomes for people and improve the patient experience. You will provide really useful information and advice to people and their families. You will have conversations with people to identify what they are worried about or what they want to achieve, what support they already have and what they need. You will advise them on the best local services and solutions to suit the person’s needs. It is your role to encourage people to be independent, to help them grow their confidence and make the most of the support available in their local community. You will make connections in the area that you are based, and you will work in partnership with local organisations to achieve the best outcomes for local people. You will be expected to be flexible in your work base and to be willing to travel within the area of Somerset where you work, you will be covering the acute hospital as well as the community hospitals in the area and potentially working within admission avoidance as well. About You We are looking for people who are not afraid to do things differently to create support that is better for people.
