2. White Sulfur Springs Phase 9 - CDBG Grant Work . Project Budget: 3749,500 - OCR grant limit: $750,000 . Scope: This project involves installing 2,100 linear feet ol8" diameter HDPE ductile iron water main and appurtenances on Shore Road and County Route 145, reconnecting existing water service lines, installing 4 hydrants and site restoration. . A test dig conducted by Town Water & Sewer in December 2019 on Dahlia Road (CRl45) indicated that the asbestos cement pipe continues to the end ofthe line (approximately 400' further); so we plan to design for replacement ofthe additional +/- 400' as a bid altemate. . Design Phase Work: o Site Surveying:

3. The Next CDBG Grant Project - White Sulphur Springs Boyd Road and Well #1 o 2019 CFA Awards were for annormcd l2l19ll9. This project was not selected for award. . As such, the need to purchase and install a new punp ard redevelop now dormant Well l, and associated piping and electrical work would need to be done using District fimds. . Current back up for Well 2 is to utilize the Stevensville system by operating valving in the Shore Road water building - short term emergency use - not long term backup o Could consider resubmitting in 2020 for a CFA OCR CDBG grant for the work requested in 2019 application and providing a new pump and pitless - focus on providing a redundant pump for White Sulphur Dstrict? . We most likely will have to do an extension of the maturity date because we can't proceed with converting the financing to long-term until all costs have been fully locked down. 5. Stevensville Radio Work . The radio system will take the place ofthe landline that currently conveys the tank level reading at Water Tower Road tank to the controls for the Sherwood and Roth pumps, and Stevensville pumps. The scope of work provided by General Controls includes providing the necessary radio telemetry and appurtenances at the Water Tower Road tank, the Sherwood,tsooster pump building on Shore Road and at the Stevensville wellfi eld conhol building. o Town awarded work on August 19, 2019 to the sole responder, General Controls, for a cost of$2 t,800 for the radio systerq plus an optional cost of $ I ,000 additional for a new pressure transducer at Water Tower road, if needed. . G€neral Controls work continuing; with a anticipated finish this month - needed to fabricate some better supports for new panels and antennaes

6. Stevensville Water CR 55 and Briscoe Road Lines o SAME AS TIIE LAST FEW MONTHS FOR TOWN CONSIDERATION: o Finance advised that the cost to add $lM in capital cost to the 2.59% loan with NYSEFC results in a S50.000/year debt service. . When divided over the 80 MG ofwater sold ($50,000/80,000,000 gallons or 80,000 MG) = a $0.63/thousand gallon rate increase for each addilional $lM to be financed. . Adding $2.5M to the project could add about S1.56/thousand gallon to the rate . Continuing leaks on CR 55 and simplex pipe (susceptible to deterioration) on Briscoe Road critical connection from main Town water sources (Stevensville wellfield and Roth /Sherwood) into the S'ville and all other water districts. o See attached figure for current project and balance of line (about 4,500') to the water tank and the remaining portion on the Briscoe Road (6,700') - ifboth done would add about $2.5 to the project o This was included in the original scope of the project o $6M project cut back to S3.5M to get the then maximum grant of $2M o Maximum grant now is $3M o Is the Town interested in inquiring with NYSEFC to consider financing for this? 4 7. Key Infrastructure Needs . At February 19, 2019 meeting, the Board expressed desire to focus on water for Femdale (priority l) and Swan Lake WWTP work (second priority) with a capacity/build out review and continue with lssislance on Crossingers . Ferndale Water District: o Hydrogeological Study to idcntili potential wcll sites completed by Alpha Gcoscicncc o Delaware continuing to communicate and work with Alpha and the Town: o At the July meeting, the town board decidcd that they would like to consider drilling a well on the Town's property on Infirmary Road o Alpha submitted a proposal dated Sept I l, 2019 to develop a scope of work and budget cost for exploratory work at the Infirmary Road site. o Town approved Alpha's proposal but wants to choose a site for which Alpha will write the exploratory estimatc. o Delaware and Stacey reviewed the drawing/aerial with property lines for a potential desired area for well exploration and discussed via phone with Alpha on Nov 4. . Discussed potential of current Town-owned site with Alpha based on their discussion with Town Highway personnel familiar with the site. This site has potential; Delaware and Stacey looked at infrastructure coffrection needs and it would be very costly to bring water all the way fiom that site o Based on review ofthese sites with the Board in executive session on Nov 4, several other sites were deemed more desirable for various reasons. o Delaware sent email request to Alpha to provide feedback on feasibility or prioritization of the four sites - Alpha requested Town and Delaware investigate items including location of septics, wetlands, etc. (more discussion ifdesired in executive session).

Swan Lake WWTP: o No action necessary just restating below; Ifthe Town is interested in doing any upgrades to the plant then consider the following: . Currently eligible for 0% financing loan thru NYSEFC (Jan 20l9letter) . Updated the ruP listing in April 2019 so that the project budget on the NYSEFC IUP listing is $10M. . No work requested by the Town since February so no work this month . Since the town is finalizing the budget for next year, if the Town wants to move the Swan Lake WWTP upgrade forward next year, the town should: . agree upon a final flow number (we had tentatively agreed to 800,000 gallons per day back in January/February = build out under current conditions with no new major contributors included) and to what magnitude you want to do something. That is, just deal with dewatering upgrade and screenings, or look at upgading the facility to handle 800,000 gpd on average and be able to do this for a 30 year design life (whole plant review and upgrade possible), and o Budget for an engineering report to address the desired upgrade . There was an energy efficiency/engineering report done in 2007 by Barton and Loguidice that looked at some options for plant upgrade. That was funded in part by I.IYSERDA so that town cannot apply for another grant to update or revise that report. . Remember, the project is currently on the NYSEFC lntended Use Plan eligible for 0% financing up to $10M. A 25% grant is available to supplement the 0% loan from NYSEFC - you have to apply for it and see ifthe project is selected. . In order for the town to apply for either NYSEFC or USDA RD funds for the upgrade, that engineering report will need to be updated and amended and then resubmitted with a funding application. Assuming that a significant amendment is

5 needed, a recommended budget for that work would be$30,000. This is the typical cost for an engineering report suitable for applying for funding. . This amended report, or a new engineering report, is not eligible for an engineering planning grant funding but could be included in the overall project financing cost. . So if the town wants to move forward and develop a scope and cost and plan forward, and seek funding, we'd recommend having $30,000 in the budget for an engineering report amendment.

Dear Supervisor-elect DeMayo: On October 29, 2019, inspections were conducted for the above noted water districts to determine compliance with Part 5 of the New York State Sanitary Code. Throughout the lnspection I was accompanied by Mr. Stacey Yaun, Water/Sewer Supervisor for the Town of Liberty. water supply inspections for each of the referenced water distrrcts revealed the following: Stevensville WD The Stevensville Water District provides water for approximately 349 service connections. Water sources for the district are located in the Stevensville Well Field and the Sherwood Well Field The Sherwood well (Well-6) and the Roth well (\/Vell-S) located in the sheruvood well Field and Well-4 located in the Stevensville well field are currently the primary sources producing water for the system. well-3 located in the stevensville well field serves as an auxiliary source. Well-1 and Well-2, also located in the Stevensville well field are both inactive. Auxiliary Well-3 is a drilled well which produces approximately 350 gallons per minute using a 30-horsepower high efficiency pump motor and variable frequency drive. Auxiliary Well-3 supple,"nents the supply of water to customers during storage tank shutdowns or \.'hen wate!' main breaks occur. Well-4 is a caisson well approximately 9 ft. in diameter and 35 feet deep. This source is equipped with two pumps. Pump "A" is driven by a S0-horsepower motor and Pump "8" is driven by a 40-horsepower motor. These pumps operate concurrently, and a variable frequency drive maintains a total output of about 420 gallons per minute. Orthophosphate (Carus 8600) is injected directly into the well for corrosion control. The sources delivering water from the Stevensville well field are connected to a common treatment plant where disinfection is provided using sodium hypochlorite. This water also receives treatment with caustic soda for pH adjustment. The free chlorine residual measured at entry to the system was 'l .35 mg/1.

The Roth Well (Well-S) and the Sheruood Well (Well-6) are constructed to an approximate depth of 57 ft. with 53 ft. of 12-inch diameter steel casing. The two wells are located within 20 ft. of one another and normally operate on an alternating pump cycle. The Shenrvood Well pump and motor were replaced in 2019 and the Roth Well is scheduled for the same in 2020.

Treatment for the Roth and Sherwood Wells consists of disinfection with sodium hypochlorite and corrosion control with orthophosphate. Caustic soda is also added to the water for pH adjustment. The Roth/Sherwood Treatment Plant includes (4) 5000-9allon atmospheric storage tanks which provide contact time prior to pumping to distribution using two 7s-horsepower booster pumps that alternate with each pump cycle. The 5000-gallon tanks are drained and cleaned on an annual basis. The facility also includes one 2s-horsepower booster pump to supply an emergency backup connection to the White Sulphur Springs WD. The free chlorine residual measured at entry point to the system from the Roth/Sherwood treatment plant was 'l .25 mg/L. The distribution system includes a combination of ductile iron, cast iron, and simplex pipe ranging in size from 8-inch to 12-inch diameter. The seetion of '1o-inch cast iron pipe spanning between the Swan Lake Dam and Tower Road requires replacement due age and severe tuberculation which compromises fire safety for the community. This section of water main is also very prone to water main breaks and serves as the only means of transmission to supply the Route 55, Loomis, and Ferndale water districts. Engineering plans are currently being reviewed by the Department to replace this very old and fragile section of water main. Emergency backup power for the Roth/Sherwood Treatment Plant is provided by a 200-kW diesel generator. Additional finished water storage is provided by a 500,000-gallon standpipe located at the end of Water Tower Road (T.R. No. 128). This storage tank was cleaned and inspected as recently as June 2019. The inspection report recommended new coating on all intemal and exterior surfaces. This must be done as soon as possible to prevent further deterioration that could compromise much needed storage capacity and fire flow.

Route 55 WD

The Route 55 Water District serves approximately 62 service connections and operates as a consecutive connection receiving water from the Stevensville Water District. Normally inactive but available interconnections also exist with the Village of Liberty at the Village boundary on Liberty Road (Rt. 55) and Upper Ferndale Road.

The district is served by a pump station/treatment facility located off Swan Lake Road where booster chlorination is provided using sodium hypochlorite. The free chlorine residual measured in finished water delivered to the distribution system from this location was 2.70 mglL. Two 20 horsepowetl211 GPM booster pumps configured with variable frequency drives draw water from a 120,oOo-gallon covered concrete reservoir supplied by the Stevensville Water District. The concrete reservoir is believed to be leaking profusely and the soil surrounding the reservoir is extremely saturated. Replacement of the covered reservoir is proposed with a 500,000 above ground storage tank very close to the location of the existing concrete reservoir. The new tank will facilitate removing the Stevensville tank from service for recoating and will ultimately increase the gross storage volume available to the Stevensville Water District from 620,000 gallons to 1,000,000 gallons.

Finished water from the Route 55 pump station/treatment facility is delivered to distribution and a 256,000-gallon glass lined/steel panel storage tank located near the end of Walnut Mountain Road. Tank level is maintained by signal from a pressure sensor located in a valve pit near the tank. The pressure sensor signal is transmitted to the pump station through a landline to control operation of the booster pumps. This storage tank was also cleaned/inspected in June 2019. Loomis WD The Loomis Water District serves approximately 80 service connections and operates as a consecutive connection to the Route 55 Water District. The pump station/treatment facility located on the north end of Ferndale Loomis Road supplies the Loomis Water District with water provided by the Route 55 Water District. The pump station/treatment facility provides booster chlorination with Sodium Hypochlorite. Two 10 horsepower booster pumps operate with variable frequency drives signaled by a pressure transducer on the discharge line connected to distribution. At the time of the inspection, the free chlorine residual was 1.'14 mg/L in a water sample collected at this location. The Loomis distribution system consists of 6-inch cast iron mains. The pump statron/treatment facility includes a transfer switch interface for connection to a portable, 200 kW, emergency backup generator. The 120,000-gallon covered concrete reservoir and a 96,000 gallon glass lined/steel storage tank iocated off Oid Loomis Road provide finished water storage and fire flow for the Loomis water district.

Ferndale WD The Ferndale Water District serves approximately 235 service connecttons and also operates as consecutive connection to the Stevensville Water District. Although normally inactive, another interconnection with the Village of Liberty is available on Pump Station Road. Under normal operating conditions, water provided by the Stevensville Water District is conveyed to a pump station/treatment facility located at the westerly end of Ferndale Road. The Ferndale Pump Station delivers water directly into the Ferndale distribution system and finished storage. Treatment at the pump station includes booster chlorination with sodium hypochlorite. This facility includes a dedicated 50kW generator for emergency standby power. Two 7.5 horsepower booster pumps configured with variable frequency drives deliver water to distribution and a 559,000-gallon glass lined/steel panel storage tank located approximately 2000 ft. northwest of the Solid Waste Transfer Station in Ferndale. The storage tank was cleaned and inspected in June 2019. Storage tank level is maintained by signal from a pressure sensor located in a valve pit near the tank. The pressure sensor signal is transmitted to the pump station on Ferndale Road by means of a landline to control booster pump operation. There remain several locations in the Ferndale Water District where very old cast iron mains ranging in size from 8-inch down to 4-inch diameter that have developed severe tuberculation and compromise needed fire flow. The following locations are affected by these conditions; '1. The entire length of main on Harris Road (CR 175). This main consists of 6-inch and 4-inch diameter cast iron. 2. The entire length of &inch diameter cast iron main between the connection with the Village of Lib€rty on Pump Stdion Rd. and the southerly end of Triangle Road. This main conveys water under Highway 17 (Future lnterstate-86) and the Middle Mongaup River, before extending dorn Triarple Road.

During the inspection, a water sample was collected for total coliform analysis at the Old Ivlonticello Road bleeder. The free chlorine residual measured at that location was 1.0 mg/L and results of analysis from that water sample were satisfactory with an absence of total coliform. The New York State Department of Heatth has no record of an inter-municipal agreement between the Town of Liberty and Village of Liberty to supply water through existing interconnedions from one municipality to the other during an emergency situation. The existence of a legally binding written agreement for conveyance of water between the Town of Liberty and Village of Liberty should be established as soon as possible.

White Sulohur SDrinos WD

The White Sulphur Springs Water District includes two ground water sources located off Shore Road and serves approximately 160 service connections. The primary source delivering water to the system is Well-2 which is equipped with a 2O-horsepower submersible pump. Well-1 is not currently in service. With completion of the most recent phase of the water main replacement project (Phase 8), typical consumption is about 30,000-35,000 gallons per day. Treatment for Well-2 consists of disinfection with sodium hypochlorite and corrosion control using orthophosphate. Finished water storage is provided by a 300,000-9allon standpipe located on White Sulphui'Springs Road. Recommendattcns nded in 20't6 inspection report from Underwater Solutions indicate the need for recoating all interior and exterior surfaces. This recommenddion should be complied with as soon as possible to averl any furlher deterioration of the storage tank. At the time of inspection, the free chlorine residual measured at entry point to the distribution system was 1 .17 mgl-

Cold S nn R The Cold Spring Road Water District is a small distribution system located outside the Village of Liberty boundaries. The service area operates as consecutive connection to the Liberty Village water system. This consecutive system consists of 8-inch ductile iron mains with hydrants, valves, and services for approximately 19 service connections. During the inspection the free chlorine residual measured 0.8 mg/L at the Kelly bleeder on Cold Spring Road. For your records, l've enclosed copies of the Water System Field Compliance Repoft (DOH- 4234) fot each of the systems inspected. Also enclosed is a current copy ol Repofting Emergency Changes in Public Water Systems (DOH-358). The following is an abstract of the New York State Sanitary Code, Section 5-'1 .23:

Reporting Emergency Changes

a) The supplier of water shall not take, use, or cause to be taken for use water from any emergency source or discontinue or alter disinfection or other treatment processes without first having notified by telephone or telegram, and received the approval of, the State. Upon receipt of such notification, the State shall advise the supplier of water and interested local officials of the approved aclion or proposed action by the supplier of water to protect the public health during the emergency.

b) The supplier of water must make State notification when he determines that the delivery of water is interrupted to a minimum of 25 individuals or 5 service connections, or to a minimum of one percenl of the total number of individuals served or service connections, whichever is larger, for a period cf four hours or more.

c) A printed copy of this section shall be conspicuously posted in the office used by the supplier of water.

Agreement between the Town of Liberty and the Humane Societl of Middletown, Inc. for the period beginning January l,2020 and ending December 31,2020.

The Humane Society of Middletown, Inc. agrees to accept any dog brought to our shelter by your Animal Control Officer (ACO), or Dog Control Oflicer (DCO), from your municipality space permitting.

The dogs. in accordance rvith New York State Agricultural lau, must be held lbr the legal holding period, after which thc)' may be put up fbr adoption ifnot claimed by the owner. Ifsaid dogs are not claimed or adopted, they may be humanely euthanized.

If the Shelter must hold a dog beyond the legal holding time for your municipality because of a pending court case. bite case, etc., we will charge your municipality $25.00 per diem, per dog. We can only hold up to three (3) animals for your municipality when the animals are seized by an ACO/DCO in the event of a cruelty case.

When leaving a dog that the DCO/ACO believes may be dangerous. the dog must be kept in the outside only part of the ACO/DCO pen and follow the agreed upon protocol. Written information must be given to the shelter or the shelter manager that clearly communicates that the animal may be dangerous.

Since we do not have a veterinarian on premises, we are unable to accept any animal with suspected mange, rabies, distemper, parvovirus or an injury that requires immediate medical attention. These animalswill have to be taken to a veterinarian byyour ACO/DCO for treatment or euthanasia. Any resulting bills for these animals will be paid directly to the veterinarian by your Town/City.

Ifthe Humane Society assists your municipality due to tlre unavaihbility ofyour animal control officer or police departrnent. your municipality will be billed an additional fee of$100.00.

The municipality rvill pay a fee of $300.00 per dog delivered to the Society The municipalitl, will pay a fee of $75.00 per cat delivered ro rhe Society. A rate of $100.00 will be charged for each dog delivered DOA. A flat rate of $50.00 will be charged for each cat delivered DOA.

The Humane Society will send out vouchers once a month A record ofthe disposition ofthe dogs liom the prior month will be sent out by the end ofthe month.

I'm writing to remind your town to assign a delegate (and an altemate) to attend our Annual Business Meeting with the purpose ofelecting the president, five vice presidents and voting on the Association's annual legislative platform. Assignments should be made to town offrcials who plan on attending the 2020 Training School and Annual Meeting, to be held February 16-19,2020 at the New York Marriott Marquis in .

To assign your delegate: The town board must adopt a resolution designating its delegate (and alternate). The altemate will assume delegate voting responsibilities il the delegate cannot vote for any reason. After the resolution complete and submit the enclosed Certificate of Designation to us no later than February 3, 2020.

Important dates for the delegate: The optional Informational Budget Hearing will be held Sunday, February 16,2020 at 3 p.m. Voting will take place at the Annual Business Meeting on Wednesday, February 19,2020 at 8 a.m.

A note about membership dues: For a delegate to cast their vote at the Annual Business Meeting, your town's 2020 dues must be received inthe office by February 7,2020. We are unable to apply dues payments received by mail in our offrce after that date, no exceptions.
