O R D E R Administrative Approval is hereby accorded for the scheme ‘Construction of 03(three) nos. of Non-GP Head Quarter Sub Centre at Darjeeling (GTA) district’ at an estimated cost of Rs.52,53,183/- (Rupees Fifty two lakh fifty three thousand & one hundred & eighty three only). 2. Each Sub Centre will have to be built at the sites as communicated to this Department at an unit cost not exceeding Rs.17,51,061.00 (Rupees Seventeen lakh fifty one thousand & sixty one only) as per model plan & revised estimate with revised PWD SoR and introduction of GST from 1st July, 2017 (Annexure –I) Details are as follows:

The implementing agency for construction of the Sub Centre is to be decided by the Executive Committee of the District Health & Family Welfare Samity, Darjeeling GTA chaired by the Secretary, GTA. 4. On receiving requisition, fund will be released in favour of District Health & Family Welfare Samity, Darjeeling GTA for subsequent placement of the same to the implementing agency and will book this expenditure in the FMR. 5. No price escalation will be allowed further to implement the work. No other works other than the scheme sanctioned are to be implemented / take up against the approval accorded. 6. The charge is debitable from the fund of NHM Flexi-Pool 2017-18 under the head of account ‘Construction of Non-GPHQ Sub Centres’ [ Tally A/c head – B.5.3.2] 7. Statement of Expenditure & Utilization Certificate of the respective fund should be submitted to this Department in due course. 8. Senior Accounts Officer, National Health Mission will act as the Drawing & Disbursing Officer for the said purpose.

Document1 1 9. Engineer-in-Charge, SPMU will make periodical on-site visits to review the progress of work and check the quality of work being implemented. 10. This issues with the approval of Mission Director, NHM & Secretary, Health & Family Welfare Department, Government of West Bengal. 11. All concerned are being informed.
