Fraternity, Incorporated THETA SIGMA ALUMNI CHAPTER 2021 College Scholarship Application

Iota Theta Sigma (ITS) Alumni Chapter of Phi Beta Sigma Fraternity, Inc. is pleased to offer multiple scholarships to high school minority male seniors in Dorchester County who plan to attend an accredited college or university in fall semester of 2021. It is our goal to encourage young minority men.

Eligibility

➢ Must be a high school minority male senior. ➢ Plan to attend an accredited 2 or 4 year college or university. ➢ Can only apply for one scholarship.

ITS Scholarships Offered

Iota Theta Sigma Scholarship has a total value of $1000 Applicants will compete for one of two $500 scholarships offered to a high school senior who plans to attend an accredited two– or four-year college or university in the fall semester of 2021, who has a record of scholastic achievement. Must have a GPA of 3.0 or higher. Cecil Alston Jr. Scholarship has a value of $250 This scholarship was established in 2020 to assist a Dorchester County high school senior enrolled in the field of math or computer science. Must have a GPA of 3.0 or higher.

Kevin Blake Scholarship has a value of $500 This scholarship was established in 2020 by Kevin R. Blake, St. John's High School alumnus (Class of 1992) to assists St. John's High School senior enrolled in any program of study. This award will be awarded based on academic achievement, leadership, and character. If no St. Johns senior qualify, the scholarship will default to a Dorchester County senior. Must have a GPA of 2.5 or higher.

Michael Bligen Business Scholarship has a value of $500 This scholarship was established in 2020 by Michael Bligen, St. John's High School alumnus (Class of 1992) to assist a St. John's High School senior enrolled in the business field of study. This includes business administration, finance, accounting, marketing. If no St. Johns senior qualify, the scholarship will default to a Dorchester County senior. Must have a GPA of 3.0 or higher. John A. Calhoun Mathematics Scholarship has a value of $150 This scholarship was established in 2020 to assist a Dorchester County high school senior enrolled in the field of mathematics. Must have a GPA of 3.0 or higher. Bakari Jackson Academic Scholarship has a value of $600 This scholarship was established in 2020 by Bakari Jackson, St. Andrew High School alumnus (Class of 1991) to assist a West Ashley High School senior who have demonstrated academic excellence. Must have a GPA of 3.6 or higher. If no West Ashley senior qualify, the scholarship will default to a Dorchester County senior.

Chris Jess Engineering Scholarship has a value of $500 This scholarship was established in 2020 to assist a Dorchester County high school senior enrolled in the field of engineering. Must have a GPA of 3.0 or higher.

Tony McBride IT Scholarship has a value of $600 This scholarship was established in 2020 to assist a Dorchester County high school senior enrolled in the field of information technology. Must have a GPA of 3.0 or higher.

Dr. Daniel Middleton Health Scholarship has a value of $250 This scholarship was established in 2020 to assist a Dorchester County high school senior enrolled in the field of health. Must have a GPA of 3.0 or higher. Scholarship Application Checklist

Use this checklist to make sure that you have submitted all required materials to receive consideration for the scholarship selected. Incomplete applications will not be considered. Your application must include the following documents:

Completed scholarship application – handwritten application will be accepted.

Your signature at the end of the application.

Your parent or guardian’s signature at the end of the application.

Media Release and Photography Form signed by a parent or guardian.

An official, signed high school transcript.

Headshot Photograph (Dress Shirt, Tie and Jacket)

Two (2) letters of recommendation from any of the following persons (no more than one per category): • High School Teacher • High School Counselor • High School Principal • Community Leader • Minister • Employer All letters must include the contact information of the person providing the recommendation and must be signed in ink. Recommendations from family members in the categories above will not be accepted.

It is suggested that you ask the people who are writing your recommendations to comment upon: (1) the length of time they have known you; (2) your personal qualities, character, leadership abilities, and/or any special attributes; and (3) why they believe you have the perseverance to succeed at the college/university level.

If Selected: o Applicant selected to receive a scholarship award will be expected to participate in a chapter presentation event in person or virtually depending on COVID-19 Pandemic guidelines.

o Applicant will be required to produce a of acceptance and a copy of their class schedule for the Fall 2021 semester as proof of registration to an accredited University or College. Class registration will be verified before the scholarship dollars are released. Failure to provide this information by August 16, 2021 will result in forfeiting the scholarship award and will require no further communication from Iota Theta Sigma Scholarship Committee or the applicant regarding the scholarship award.
