We would like to obtain permission from parents before Personal information collected by Forrestdale Junior Football using players’ photographs in our publications or Club is for the primary purpose of membership requirements on our website. Photographs will be taken at training, and/or competition purposes. It will not be released for any form game day and official club functions. WAIVER AND INDEMNITY

“Football” Means Australia Rules Football “Club: Means Forrestdale Junior Football Club Inc “League” Means East Fremantle District Development Council Inc “Child” Means child or ward (whichever is applicable)

I am aware that playing Football, observing Football, learning to play Football and training to play Football, and participating in any activity carried out by the Club are activities that inherently involve risk, and that in undertaking these activities my child does so at his/her own risk. I am also aware that, as a condition of my child's admission to membership of the Club, its office bearers, , coaches, managers, , members, and/or agents are absolved from all liability arising from injury or damage howsoever caused. That injury or damage may arise out of membership of the Club, playing Football, observing Football, learning to play Football and training to play Football, or negligence of any nature whatsoever on the part of the Club or League, their representatives, office bearers, officials, coaches, managers, umpires, members, and/or agents. I understand and agree that nothing in this agreement purports to exclude any liability that may be owed by the Club or the League and their representatives pursuant to the provisions of any relevant Statutory Act/s covering these activities. To the extent that any part of this agreement may contravene any of the Statutory Act/s I agree that part of the agreement may be severed and rendered void but the rest of the agreement will remain valid and continue to have effect.

Players Code of Conduct This Code of Conduct applies to all Forrestdale Junior Football Club Players. By registering as a player with the Forrestdale Junior Football Club, each player has agreed to abide by the following principles:

 Play by the rules – of both the laws of the Game & the Rules of the Club.  Never argue with an official. If you disagree, discuss the matter with your or team manager during a break o after the game.  Control your temper. Verbal of Officials or other players, or provoking/intimidating an opponent will not be tolerated.  Work equally hard for yourself and for the team. Your team’s performance will benefit and so will you.  Be a good . Applaud all efforts by your team and the opposition.  Treat all players as you would like to be treated. Do not interfere with, bully or take unfair advantage of another player.  Play for the “fun of it” and not just to please parents and coaches.  Avoid using derogatory language.  Support the Club in it’s undertakings and encourage the Club to take any necessary disciplinary actions including the suspension and/or exclusion where warranted of any players for repeated or serious breaches of the Players Code of Conduct.  Enjoy the game and the company of teammates and opponents.  Remember that you are an ambassador for the Forrestdale Junior Football Club, the East Fremantle Football League and AFL Football. Be prepared to take responsibility for your actions.

Parents & Spectators Code of Conduct This Code of Conduct applies to all of our Parents and Spectators at any Junior/Youth/Auskick games where the FJFC is represented:

AFL Kids First Policy – basic principles are to remind adults that:

 Sport for children is a vital part of their growing up.  Children like to win, but more importantly, they want to have fun.  Parents/Coaches need to set the right example at the ground, not just at home.  Adults should be proud of a child’s efforts irrespective of the result.  Sport is important to children’s self esteem.

All adults that are involved in junior football need to be aware that their actions will have an effect on:

 their child’s ongoing participation in sport,  other children’s ongoing participation in sport, and  umpires and official’s ongoing participation in sport.

Adults shall:  Encourage participation but do not force participation upon them.  Encourage children to always participate according to the rules.  Never ridicule mistakes or yell at a child for making mistakes or losing a game.  Remember, they learn best by example. Applaud good plays by all teams.  If you disagree with an Official, raise the issue through the Team Manager. NEVER question an Officials judgement or integrity in public.  Always encourage players to play by the rules and accept the umpire’s decision at all times. The Forrestdale Junior Football Club will take a zero tolerance approach to umpire abuse.  Teach that honest effort is the victory, not the winning.  Remember that children play organised sports for their enjoyment. They are not playing for the entertainment of spectators and parents.  Recognise positively the recreational contribution of voluntary officials, coaches and administrators from both clubs / sides, without them the kids could not participate.  Support all efforts to remove verbal, racial and and to settle disagreements without resorting to hostility or violence.  Remember that smoking and the consumption of alcohol is unacceptable at junior sport. Offenders will be asked to remove the alcohol and, if necessary, leave the facility.  Support your club officials to foster high standards of behaviour for your club and the District.
