Extension Activities and Institutional Social Responsibility (ISR) of Yenepoya (Deemed to be University), Mangalore- 2016-2018

Extension of curriculum from institutional level to community:

As per the regulations laid down by various national regulatory bodies, extension is a part of curriculum to the community and is an essential requirement. At Yenepoya (Deemed to be University) sensitization on Institutional social responsibility is imparted through class room teaching as well as providing direct exposure to the community. Accordingly for MBBS, Community Medicine is one of the subjects. Similarly, Community Dentistry, Public Health and Community Health Nursing departments also offer similar components in the curriculum. The constituent colleges of Yenepoya (Deemed to be University) have made elaborate arrangements by way of infrastructure, faculty, support staff, learning resources etc. to enable these departments to carry out curricular activities to sensitize the students to community environment. During exposure to the community both students and the faculty members are involved in health care delivery, health education, health checkup camps and house to house visits. This activity not only gives them the rationale for institutional social responsibility but also reinforcement of social welfare values and concepts.

Details of social outreach programs undertaken by the University: Rural Health Care and Development Centre (RHCDC): Yenepoya (Deemed to be University) basically is an inclusive institution and has on its role moderately higher number of students from minority and other backward class and is located in a Panchayath/Rural area. It has the commitment to take medical education and health care delivery to the less privileged classes of the community.

To satisfy this objective, University has started a dedicated Rural Health Care and Development Centre in addition to routine community departments of the constituent

colleges. The centre is playing a pivotal role in sensitizing its faculty and students to the social responsibilities envisaged through certain unique programs as detailed below.

Training on Family Doctor System:

In the context of globalization where health care delivery is highly complex and expensive (Yenepoya Deemed to be University) considered introduction of family doctor system as a most effective method through which the country can ensure all the families are covered under the health care delivery system. Accordingly the University took a strategic decision to train its students on family doctor system. The Centre for Rural Health Care Development in collaboration with Community Medicine and Public Health Dentistry has designed a program where clusters of families in the outreach areas are assigned to group of students who will look after the health care requirements of these families thereby creating a mindset conducive to become family doctors of tomorrow.

Yen Health Cards:

The patients referred by the personnel engaged in the family doctor systems and the patients referred through the aforesaid projects for advanced investigations and treatments are all provided with the Yen Health and Arogya cards which will enable them to avail these facilities either free or at heavily subsidized rates.

Door to Door Health Care Delivery Program:

University has established 29 Medical Sub centers and 09 Dental rural centers in the rural and outreach areas in and around the campus The 29 sub centers cover the entire population of the 10 Panchayats and these centers concentrate on primary health care for those who require investigations and treatment beyond the primary care and are referred to Yenepoya Medical College Hospital. While implementing this program, it was noticed that many of the villagers were illiterate and under-privileged and did not understand the necessity of utilizing the services available for their health needs. They

approach these centers only when they fall ill. To tide over this problem, the RHCDC came out with a program of Door to Door health care delivery where doctors, medical students and para-medicals from different groups make house to house visits, identify the public health problems faced by them and help them to solve these problems. This program of health care delivery has been widely appreciated.

Yen Health Care on Wheels:

In spite of establishing 38 sub centers, we could identify some outreach areas where centers could not be established. For these areas the University conceived the idea of health care on wheels and titled it as ‘Yen Health Care on wheels’. Ambulances and dedicated vehicles are specifically designed, furnished and equipped to function as mobile medical and dental outpatient departments where groups of medical/dental personals constituting of doctors, students, nurses and paramedics are posted on these mobile clinics, to visit these outreach areas and conduct clinics regularly.

Routine activities: The routine activities conducted include Health screening camps, detection Camps, Immunization Camps, Treatment Camps, Blood donation camps etc. In last three years several such programs have been organized by the departments of Community Medicine, The Centre for Rural Health Care and Development and Department of Medical Social Work.

Health Awareness Talks:

Health awareness programs are conducted at community level regarding various communicable diseases and methods of prevention regarding water and food borne disease like gastroenteritis, infective hepatitis, mosquito borne diseases like Dengue, Chickengunia, Malaria, Filaria, etc. Emphasis is given with regard to the relationship between personal hygiene, environmental hygiene etc, with the maintenance of health.

School health education programs are also conducted to ensure that school children, parents and teachers are aware of good practices in maintaining the physical and mental health of children in a proper way.

School & College Health Checkups:

School health check-up programs are being done in different schools in order to prevent malnutrition, underweight, anemia etc. if any. Blood grouping & Blood sugar checkup camps: Blood grouping and blood check up camps are being done in different schools and colleges and in the sub centres. Major Camps: Outreach activities like special medical camps in association with local organization, self help groups etc. are being done in all the sub centres. Immunization Programs: Pulse Polio immunization is given for under 5 year children during pulse polio immunization days. The aforesaid activities have created remarkable impact on the social output of students and they themselves take the initiatives to constitute and organize their own program called ‘Samraksha’. Samraksha: mobilizes funds through various activities (e.g., Art for Charity) and helping hand is extended to the suffering patients from the less privileged sections of the society.

Dental trauma awareness programs

These programs are conducted in schools around Mangalore where a large number of locals along with students have benefitted (Auto rickshaw driver, bus drivers, social workers, physical trainers, play home staff etc.). Blood check up camps for detection of

Anemia, Diabetes, Malaria and Filaria with facilities for routine blood investigation and blood grouping; School health check up program for early detection of diseases and timely treatment and prevention of diseases through well planned immunization program are contributing to build and strengthen interface between the University and the neighborhood community.

Smile Train Program:

University in collaboration with Smile Train, USA and Germany has established a dedicated Centre for the Craniofacial Anomalies. These anomalies which include cleft lip and palate and spectrum of complex craniofacial anomalies have always been a treatment challenge where various specialists namely, Oral and Maxillofacial Surgeons, Plastic Surgeons, Neurosurgeons, ENT Surgeons, Ophthalmologists, were to be coordinated depending on the nature of Anomaly. This could be solved to a great extent through the emergence of a dedicated specialty namely craniofacial surgery. YU has been fortunate enough to have well qualified and competent team of specialists in these complex interdisciplinary areas for thousands of unfortunate patients at a totally free of cost with the help of Smile Train. The details of the Centre are furnished in Annexure No. 1

The Centre has already come out with a post- doctoral program in this interdisciplinary area for surgeons from different specialties like oral and maxillofacial surgery, plastic surgery, ENT etc. to specialize in the surgery of acquired and congenital craniofacial anomalies. Institution also actively participates in the programs of national importance namely pulse polio program, breast feeding program, vector borne disease control program, HIV/AIDS program, blindness control program, nutrition programs etc.

Institution also observes all the important days and weeks outlined by World Health Organization, Union of India, Govt. of Karnataka and University Grants Commission. All Institutions have extensively collaborated with various government departments for specific purposes which include:

 Yenepoya Medical College Hospital(YMCH) collaborates with the District Health and Family Welfare Department in organizing health care education and also in epidemic control in the designated Taluks of Dakshina Kannada District depending on need.  YMCH extends services to the public through ICTC (Integrated Counseling & Testing Centre) for voluntary blood testing for detection of HIV which is established by Karnataka State Aids Prevention Society.  Directly Observed Treatment Short-course (DOTS) Centre under Revised National TB Control Program (RNTCP) of Government of India is also functioning at YMCH which provides free testing and dispensing of drugs for treatment of Tuberculosis.  With a vision to make cataract free zone YMCH with financial grant from District Blindness Control Society is undertaking cataract screening and surgery camps in the 12 PHCs in Mangalore and Bantwal Taluks allotted by the DBCS of Dakshina Kannada.  To further women and child health YMCH has joined hands with Nirmala Charitable Hospital, Ullal by posting specialists and PGs in OBG and disciplines on regular basis.  YMCH collaborates with the District Child Welfare Committee of Dakshina Kannada (CWC) in conducting health checkup program at fixed intervals in the six Orphanages affiliated CWC in Mangalore Taluk.  YMCH collaborate with the District Health and Family Welfare Department in organizing monthly health check-up clinics in Community Health Centre Moodbidri for the families affected from Endosulphan spray.  YMC collaborates with the District Health and Family Welfare Department of Dakshina Kannada for conducting MBBS rural posting in Kotekar and Kurnad PHCs.

 YMCH collaborate with the District Health and Family Welfare Department in compulsory immunization, pulse polio, family planning and control of communicable diseases.

Medical Social Work Department in the hospital is actively involved in extension activities like conducting awareness programs, medical camps, screening camps, immunization camps, treatment camps, blood donation camps etc.

Community Medicine Department also conducts awareness programs, screening camps, immunization camps, treatment camps etc. apart from rehabilitation initiatives and research activities mentioned above. For these activities in the rural areas the department has dedicated rural centres and an urban health centre specifically for the sake of the less privileged poor in the society.

Public Health Dentistry Department has 9 peripheral centres to coordinate their extension activities in the rural and outreach areas. These centres also demonstrate its responsibility of bringing up the new generation with appropriate oral health through camps conducted in the nursery and primary level schools.

The Department of Pedodontics also joins with Department of Public Health Dentistry in carrying out these activities directed towards maintaining the oral health among school going children. Departments of Public Health Dentistry and Paedodontics have conducted school screening camps and other oral health care programs. Community Health Nursing: Community and family based Nursing Care is a new area being introduced in the nursing. Accordingly, they also join the University’s endeavor to take health care profession and health care delivery to the community at a family level. Awareness camps, rehabilitation camps, road shows, runs etc. are being conducted.

Details of social surveys, research or extension work undertaken by the institution to ensure social justice and empower the underprivileged and the most vulnerable sections of society:

Most of the departments under the University conduct survey (social) based research as part of community engagement and extension of medical education from the hospital environment to the community environment. Community Medicine Department has initiated above twenty research projects related to epidemiological aspects of various diseases. Various departments in the Nursing College have taken up Survey based research projects as shown below.

Mechanism for tracking the students’ involvement in various social movements/ activities that promotes citizenship roles: Student involvement in various community and educational based service activities is closely monitored by the faculty. University has a system of conveying departmental meetings where the curriculum is critically reviewed and the student participation in

community based activities is discussed in detail. Participation in activities that promote citizenship roles among students is part of the curriculum and attendance is mandatory. To monitor the student involvement in community based activities organized by the Centre for Rural Health care and Development, MSW Dept., NSS and Youth Red Cross a very effective mentorship program is in place where the students are divided into small groups and it is the responsibility of the mentor faculty to ensure adequate participation of the students in all social movements and activities that promote citizenship roles. In general we found an overwhelming participation of students in implementing a spectrum of projects being implemented either as part of curriculum or as a part of beyond the curriculum projects.

To enable the students to play citizenship roles, the University observes several national and international days like World environmental day, No tobacco day, World health days etc. In connection with these days, street plays, skit, walkathon, marathon, quiz, debate etc. are conducted and add-on courses are offered to mould each student through personality development, including sessions on behavioral and social skill, so as to enable him/her to play his/her roles as a good citizen of this country.

The University has a dedicated Centre for Environmental Studies to sensitize the students to the emerging threats to the environment and to prevent them. Add on course is offered in this regard as prescribed by the UGC.

Involvement of the community in its outreach activities and contribution to community development by the University:

Wide spectrums of community based activities have been initiated to take healthcare education and health care delivery to the rural and outreach areas and thereby contribute to community development.

Rural Health Care and Development Centre, Dept. of Medical Social Work, National Service Scheme Unit, Youth Red Cross, play a major role to ensure the involvement of the community in these outreach activities. The starting of 29 sub-centres in outreach areas, 09 dental peripheral centres, initiation of house to house health care delivery programme, initiation of health care on wheels programme, initiation of the training programme in family doctor system etc., stands eloquent testimony to the endeavors made by the University to ensure the involvement of the community in the outreach areas. The following activities are being carried out on a regular basis in the vicinity of the institution, surrounding areas of Dakshina Kannada and also neighboring districts such as Hassan, Chickmagalur, Shimoga, Davanagere, Chitradurga and Kasargod districts by the respective departments/ Institutions. 1. Department of Community Medicine:

The following activities are conducted:  Razia Rural Health Care and Training Centre at Harekala is functioning under the supervision of department of Community Medicine of YMCH. This hospital is catering to the health needs of about 50,000 populations. Inpatient and outpatient facility, dental and physiotherapy services are made available to the public through this hospital. 750 families are covered under 100% free treatment facility at YMC Hospital in collaboration with Hydrose Haji Charitable Trust Harekala. Weekly clinics,

health education, implementation of national programmes like pulse polio, malaria control, TB control, sanitation in selected clusters are conducted through this hospital.  YMC has taken up an innovative pogramme of village adoption for total health care. Accordingly Kotepura ward of Ullal Municipality has been adopted where a sub centre under RHCD provides free health checkup facility for the community. Health education, immunization and school counseling services are also conducted. Dept. of Community Medicine supervises and monitors this programme by deputing its faculty, MBBS students and CRIs.  Regular health check up in the two orphanages affiliated to Child Welfare Committee (CWC) of Dakshina Kannada.  Urban Health Centre at Bunder, Mangalore catering to the health needs of Bunder, Kudroli, and Bengare population through outpatient services, health camps, health education and implementation of national programs.  Rural postings of 4th, 6th, and 7th term MBBS students through Family Care Programme.  Postings of PGs to conduct weekly clinics at Balepuni Satellite Centre.

Department of Public Health Dentistry: Dept. Public health dentistry of Yenepoya Dental College which is the key link between the college and the community has been undertaking number of dental health care programs since the establishment of the YDC. Key programmes are:-  Dental checkup and treatment camps in the community with support from local service organizations.  School dental screening camps.  Dental education program through talks, video shows, posters and exhibitions.  Rural Dental Satellite Centers with support from NGOs and Institutions.

Department of Community Health Nursing: The department administers rural postings of students in the selected villages at regular intervals. Health education, health screening camps and celebration of national programs are being conducted through interdepartmental coordination. Yenepoya Physiotherapy College: YPC plays a key role in extending physiotherapy activities in the village community through rural postings of students in the selected villages and shelter homes; health education, disability screening camps, free distribution of rehabilitative aides and celebration of national days with the involvement of students, interns and PGs. The YPC runs physiotherapy clinics at Olavinahalli Social Service Centre for the benefit of the inmates of Old Age Home.

Rural health care and Development Centre (RHCDC) Yenepoya (Deemed to University) is implementing a separate project titled RHCDP in the vicinity of the campus. The following activities are conducted in the selected village clusters:

 Sub centers – regular health screening clinics  Coordinating family focused health care by MBBS and BDS students  Health education camps  Celebration of national events  Implementation of national programs like pulse polio, malaria control, TB control, leprosy eradication etc.  School health program - health check up, counseling, health talks  Health cards for unprivileged populations  Conduct research studies on topics of significance  Linkages with local governance bodies, NGOs and associations for issue based collaboration.

Department of Medical Social Work:  Coordination of camps – Medical, Dental, Ophthalmology, blood donation, diabetes detection in the semi urban and rural areas ongoing basis.  Coordination of weekly clinics at Balepuni Satellite Centre  Coordination of monthly health check up clinics in collaboration with District Family Welfare and Family Planning department of Dakshina Kannada and CHC Mudbidri for the Endosulphan affected families.  Coordination of Yen-Sahayog initiative of YMCH to extend health care on free basis to Old Age homes, Psycho-Social Rehabilitation Centres and Residential schools. At present 41 NGOs, 5 shelter homes & residential schools, 8 CWC Orphanages, 17 Patient Information Guidance Centres in Dakshina Kannada, Uttara Kannada, Kasargod and Kannur districts.  Implementation of Blindness Control Programme in the 12 PHCs allotted by the District Blindness Control Society of District Health and Family Welfare department.  Coordination of regular health check up in the four orphanages affiliated to Child Welfare Committee (CWC) of Dakshina Kannada.  Rehabilitation support to needy patients and clients in the community through mobilizing local resources.  Assistance in epidemic control and health education programs conducted by colleges/departments of YU.  Assistance in popularizing various patient friendly health programmes like Yen- Arogya in the community with the intention of reaching out to vulnerable sections in the society.

Department of Ophthalmology:  Coordination of Blindness Control Program in the 12 PHCs allotted by the District Blindness Control Society of District Health and Family Welfare department.  Observance of days like World Sight Day, National Glaucoma Awareness week, National Eye Donation etc.

 Blindness screening for eye screening and diabetic retinopathy camps in the community.  School Children Eye Screening camps  Health Education.  A MoU has been signed with DBCS and the Government supports this activity through reimburses cost of surgeries.

National Service Scheme Unit:

NSS has taken up national integration as a priority area and motivates the students to actively take part in the national integration camps. NSS also conducts cultural programs in association with various national and international voluntary organizations like South Indian Cultural Centre, Folklore Academy, where artists from other parts of India come out with their native form of arts. NSS unit also organizes programs focused in the areas of cancer awareness, health check up camps, Youth exchange programs, self defense programs, observation of important days and a National Symposium on Cancer prevention and health promotion and early detection. Hundreds of students participated in various programs over the year from the State and other parts of the country.

These activities are widely covered by the press and media due to its innovative nature and also greatly benefitted the society in terms of focused efforts towards health promotion and disease prevention. The activities of NSS is duly recognized by the Government and Non Government Agencies and following awards have been received for the same from the State Government:- The details of programs carried out during the last three years are enclosed as Annexure No.4.

 KARA Seva: A student initiative for early detection of common diseases in school children by medical undergraduates for screening the students in Government Schools who do not have access to quality health care. During the course of study the Students came across young children with conditions which could easily have been prevented if they had access to the right kind of medical attention. The condition of poor health of the students was reflected in their attendance in school and hence this activity was taken up for screening and prevention. KARA SEVA conducts health check-ups for the school children, talks on Hand washing techniques are demonstrated, talks on Menstrual Hygiene, General hygiene, Good touch and bad touch are given. Management of medical emergencies are delivered. Recycling and reusing the various wastes to create social responsibility is also explained to the students. For the period 2017-18 the students have visited 18 schools and conducted the screening. The total numbers of students screened are 2326 out of which 347 students have been treated in our hospital.

The various activities also include:

DHRITI: Recreational activities are conducted at orphanages and old age homes in the months of March to May

EPIPHANY: A activity where waste papers are collected from the teaching, non teaching staff and students of the University and used for recycling.

A Memorandum of Understanding has been entered by Kara Seva and Rural Health Care and Development Centre of Yenepoya (Deemed to be University). Financial assistance of Rs.10, 000/- per month is being given by the University.

SOCIAL-JUSTICE: Substance Free Campus: University has declared the campus as totally substance free and created a dedicated centre namely, Centre for Substance Abuse Prevention. The centre has conducted various awareness programs against various substances like alcohol, tobacco and drugs. Placards are displayed in the campus with slogans of preventing tobacco and alcohol in the campus and this is being strictly implemented with the help of mentors and faculty.

Institutional alignment itself with the annual themes/programs of WHO/ICMR/UGC: University has made it a regular practice to observe various days on relevant health themes like World Aids Day, World Health Day, Mental Health Day and Week, No Tobacco Day, Doctors Day, Yoga Day, International Flag day, World Nursing Day, Breast feeding week, Nutrition week etc. There is a theme associated with each day and week. University arranges panel discussions and invited lectures on these themes and takes up the challenges in the themes for further follow-up. Several students and faculty projects are undertaken in the thematic areas outlined by ICMR/UGC.

Role of the institution in the following extension activities  Community outreach health programs for prevention, detection, screening, management of diseases and rehabilitation by cost effective interventions:

The routine programs consisting of awareness creation program, screening/detection camps, treatment camps, immunization camps, and rehabilitation endeavors and unique programs like door to door health care delivery program, health care on wheels, immunization program, training program on family doctor system, Health Card distribution, are meant for prevention, detection, screening, management of diseases

and rehabilitation totally free of cost or provided at highly subsidized rates. These services can be described as cost effective.

 Awareness creation regarding potable water supply, sanitation and nutrition.  Awareness creation regarding water-borne and air-borne communicable diseases.  Awareness creation regarding non-communicable diseases - cardiovascular diseases, diabetes, cancer, mental health, accident and trauma, etc.  Awareness creation regarding the role of healthy life styles and physical exercise for promotion of health and prevention of diseases.  Awareness creation has always been an integral part of our extension activities carried out by different agencies of the University.  Awareness creation regarding AYUSH Systems of medicines in general and / or any system of medicine in particular.  Complementary and alternative medicine.  Pharmaco economic evaluation in drug utilization.

Yenepoya (Deemed to be University) has been so far running academic programs on modern medicine, nursing and allied health sciences. But, now the University has considered the possibility of diversifying the areas of specialization to the systems of Alternative medicine as well. Accordingly, University has approved Yenepoya Homeopathic and Medical College Hospital and Yenepoya Ayurveda College and Hospital at its extended campus located at Naringana Village, Kinya. Several awareness programs have been conducted for our faculty and students on the alternative systems of medicine. In addition to awareness creation, our academic departments and University research center has been conducting some integrated research programs. A separate faculty of Ayurveda and faculty of Homeopathy have been created to facilitate the establishment of AYUSH courses and Hospital.

Hospital information system covers entire hospital including the pharmacy. Periodic evaluation of the drug utilization from Pharmaco-economic point of view is undertaken to make the treatment cost effective. A drug policy including antibiotic policy has been evolved by the hospital administration and a drug formulary has been created with cost effective and high quality brands of all important molecules. The department of Hospital Administration makes a systematic review on the judicious utilization of the drugs using the data available through the Hospital Information Systems periodically.

 Participation in national programs like Family Welfare, Mother and Child Welfare, Population Control, Immunization, HIV/AIDS, Blindness control, Malaria, Tuberculosis, School Health, anti-tobacco campaigns, oral health care, etc. and Promotion of mental health and prevention of substance abuse.

University has taken extra care to extend the medical education and health care delivery to the community and the family level. In that process importance is given to implement National Health Programs like Family Welfare, Mother and Child Welfare etc. In fact the unique programs initiated as beyond the syllabus academic activity is a training program on the Family Doctor System to groom the next generation of health care providers well trained on family doctor system, which is the only method of ensuring health care delivery and welfare at the family level including mother and child welfare. University centres like centre for Rural Health Care and Development and Department of Medical Social Work along with the academic departments like ophthalmology, dermatology, OBG, General Medicine, Chest Medicine etc. all play their due roles in the implementation of National Health Programs like Blindness control, HIV AIDS, Malaria, TB, and Population Control etc. Rural Health Care & Development Centre and Dept. of MSW along with Pediatrics and Pedodontics conduct regular dental and general health check-ups in different schools, destitute homes, orphanages etc.

Anti-tobacco campaigns is one the main program adopted by the Center for Prevention of Substance Abuse. University has established 9 dental peripheral health care centers apart from various camps conducted in other areas as mentioned. As recognition to our commitment towards oral health care the Head of the Department of Public Health Dentistry was chosen by the Govt. of Karnataka as the special officer to evolve a comprehensive oral health policy for Govt. of Karnataka.

Mental health remains to be one of the major challenges to be addressed in the field of health care delivery. While the indicators related to the physical health have all shown improvement, mental health has been deteriorated. While physical health problems like infant mortality, maternal mortality, problems due to infectious diseases etc. are faced by developing countries, mental health issues are the burning problems in the developed world. University has already recognized the increased incidences of mental health problems in the society and has evolved different programs and personnel to handle the additional burden in addition to the functioning of the mandatory department of Psychiatry. The department of Psychiatry has a clinical psychology branch that handles patients with a spectrum of mental health problems below the level of psychiatric illness. The department of Pediatrics also has a Child Psychology wing headed by a highly acclaimed faculty in psychology. All these units conduct several awareness programs apart from offering counseling sessions in consultation with qualified psychiatrist in the psychiatry department. Already the department has PG and doctoral programs in Psychiatry and related areas. Now the department has come out with a new post-doctoral certificate program on Psychiatric rehabilitation in collaboration with NIMHANS Bangalore.

It has been observed that one of the worst problems that the present generation faces is the mental health problems associated with the substance abuse and addiction. The peaceful atmosphere of the educational institutions is often adversely affected by a small group of students who are habituated or addicted to the substances, including

alcohol and drugs. Understanding the need of the time, University took a policy decision to create a dedicated centre to create awareness and thereby to prevent substance abuse. The Center for Prevention of Substance Abuse also conducts several awareness programs. The Center has declared the University campus as substance free campus.

Environmental Consciousness: University has been meticulously orienting our students to become environment conscious through various programs organized by University Centre for Environmental studies. Accordingly several awareness programs on Global Warming, Waste disposal including solid waste disposal, liquid waste disposal, and bio-medical waste disposal. University has elaborate facility to handle this waste and minimize the impact. The Liquid wastes are treated through STP and ETP and the effluent water after treatment is used for Gardening and Flushing purposes. The solid waste, after outsourcing the biomedical waste for disposal, all other waste including organic waste including food waste, garbage, dry leaves are subjected to vermi-composting and the compost is distributed for agriculture purposes. In this way we have already proved to our students that nothing can be considered as waste and the so called waste if properly collected and treated can be recycled effectively. Special classes are arranged by the Centre for Environmental Studies on all these aspects. The centre also conducts green auditing of the campus systematically collecting the spectrum of flora and fauna in the campus in which the students take an active role.

The Student Welfare Department and the NSS units conduct special program on national integration, value system, human rights etc. apart from conducting various cultural programs to strengthen the affective domain of the student with deliberate intention of molding them into ideal and good citizens of tomorrow.

To create intimacy and affection to the suffering patients, from the second year onwards specific number of beds are allotted to each student who prepares his case for

presentation before the faculty and at the same time groom them into matured health care provider with the qualities of an ideal citizen/doctor. All such activities are planned and executed with wholehearted cooperation of faculty and other staff.

 Adoption of population in the geographical area for total health care: As an institution deemed to be University situated in the Village panchayath, University has taken up the responsibility of reaching a target of health for all in the geographical areas surrounding the University by 2020. To achieve this target at the inception itself University has created a dedicated centre that established 38 sub-centres in these areas and adopted 12 villages for total health care.

 Research or extension work to reach out to marginalized populations. All the extension activities are carried out by various units of the University namely, Centre for Rural Health Care and Development, Department of Medical Social Work, NSS, Community Departments, YRC etc. are aimed at marginalized populations living in the rural and outreach areas. Now the Centre for Rural Health Care and Development has come out with various community based research programs on communicable and non- communicable diseases in collaboration with the Community Medicine department of the University.

Participation of faculty members in community health awareness programs: Faculty plays the most important role as resource persons and organizers in the various types of community health awareness programs regularly.

Institutional participation in National program for prevention and control of diseases: Yenepoya (Deemed to be University) through its constituent institutions takes active participation in the implementation of national programs for prevention and control of

diseases. This is done mainly through awareness generation programs and extending services through opening of government supported patient support units in the YMCH. Such units include:

DOTS: is a community program of prevention and treatment of Tuberculosis. The unit is managed by the Department of Pulmonary Medicine. The patients are being referred from Primary Health Centers in and around Mangalore.

District Tuberculosis Center- Collaborates with Dept. of Pulmonary Medicine for the implementation of the RNTCP, Govt. of India. Students are regularly posted to the RNTCP unit that provides exposure to a spectrum of Tuberculosis related problems like MDRTB and XDRTB.

Integrated Counseling and Testing Centre: (ICTC) Under the supervision of District Health and family Care Officer is functioning in the Dept. of OBG providing testing and counseling services for prevention of HIV transmission from mother to child. ICTC also conducts awareness program for the general public.

Immunization clinics: Departments of Pediatrics and Community Medicine conduct free immunization clinics (DPT, BCG, Polio etc.) both in the tertiary care hospital as well as in the rural and urban health centers. Extension of services during epidemic outbreak: Yenepoya Medical College is an active collaborator with District Health and Family Welfare Department striving for the control of diseases during epidemic outbreaks (dengue, malaria, chikungunya etc.) in the district. Health education, blood smear test, clinics are part of such interventions.

Blindness control: Dept. of Ophthalmology with support from Dept. of MSW conducts regular eye screening camps in the remote villages as part of national blindness control program. Recognizing this service District Blindness Control Society has allotted 12 PHCs to YMCH for organizing eye screening camps with support from the respective PHCs and to perform cataract surgeries on patients identified through these camps in the Yenepoya Medical College and Hospital tertiary care hospital.

In addition to this awareness is created on importance of eye donation during the eye donation awareness fortnight observed from August 25th to September 8th every year.

OTHER INITIATIVES: - YEN MITHRA

Objective of the practice: Yen Mithra is a program designed to implement effective health care delivery to the remote and un-accessed areas. The participants of this initiative are interested volunteers (Teachers, Panchayat members, Health guides, Self Help group leaders of the particular locality) spread over several hundred Kilo meters from the main hospital. These volunteers disseminate information about the health care facilities and benefits to the respective regions. They create awareness through hand bills, posters and banners. Currently we have 200 Yen Mithras in 12 districts spread across Kerala and Karnataka. The Practice: The practice is to identify Yen Mithras in their own areas spread over 12 Districts in Kerala and Karnataka covering almost 3 to 5 lakh population. These Mithras refer the patients who need hospital care through their communication channels to the Doctors directly as we all as the co-ordinating authorities in the Hospital who also guide the patients regarding the facilities available, Doctor expertise and arrange for their travel plans. The outcome of this practice is becoming phenomenal and currently on any given day more than 1000 people are referred to the outpatient department of the hospital. This step has been extremely useful for the hospital as well as for the students

who are exposed to adverse case studies and get hands on expertise which is an essential component in health care institution. The patients are also happy in terms of shorter stay, free food, free bedding as well as extreme care in terms of standard as hospital is accredited with NABH and NABL. Hence this has proved to be a successful model in terms of extending health care to outreach areas

Evidence of success: In the last one year 6,381 patients have been referred by Yen Mithras. Most of the patients referred are from low economic background and are referred from North Karnataka i.e Davanagere, Bellary, Haveri, Chithradurga, Shivamogga and Malnad districts like Hassana, Coorg.

The results indicate that such kind of initiatives which involves different methodologies of reaching to the patient would help in improving the public health targets and motivating people to avail best possible treatment in order to reduce the disease burden. All these efforts are complimented through various programs listed above.

Dedicated communication channels have been opened on regular basis and also the Yen Mithras are invited once in a year to the main hospital where they are provided with training regarding the communication, follow up and completion of the task. The details of YEN MITHRA practice as well as extension activity carried out to support such initiative and number of people benefitted due to such practice is detailed as Annexure No.5

Yenepoya Medical College Hospital is a 950 bedded multi disciplinary teaching hospital, offering diagnostic and treatment facilities both in general and super specialty services to vast majority of population living in Karnataka and northern Kerala regions.

The Centre for Craniofacial Anomalies is much emphasized and focused in treating craniofacial deformities, comprehensively, visa viz., a multi disciplinary approach.

The Centre is equipped to provide comprehensive care for the variety of complex, craniofacial deformities that includes congenital, developmental and post traumatic. The treatment for cleft lip and palate is provided free of cost by Yenepoya Management under University Social Responsibility.

The Centre also provides ‘Cosmetic Surgery’ in view of the growing demand amongst the younger generation. The formidable team of the Centre includes Maxillofacial Surgeons, Orthodontists, Speech Pathologist, Paediatritions, Anesthesiologists and Neuro Surgeons. Due to the core competency of this Centre it attracts patients from distant areas of Andhra Pradesh, Kerala and Karnataka.

The Centre keenly participates in academic activities and has completed four studies and there are seven studies in the pipeline. The activity of the Centre is remarkably significant, that it has been awarded National best awards for Paper presentation since inception. We are soon going to launch a foundation – "Zulekha Cleft Foundation” to serve better.

HIGHLIGHTS OF THE CENTRE:  Invented and patented Max Gym, a new approach to correct mid face deficiencies in growing individuals.  Developed an innovative method of doing Naso Alveolar Moulding.  The Centre runs a fellowship programme in “Cleft and Craniofacial Surgery”

Cancer Awareness Outreach Programme for NSS Volunteers of Dakshina Kannada, Udupi & Kodagu Districts

Introduction:

Cancer is an important public health problem in India. The incidence and prevalence of cancer is rapidly increasing in both developing and developed countries. Approximately 9.5 lakh Indian population suffering from cancer, out of which around 3.3 lakh males are suffering from lung cancer, oral cavity, Oesophagus, stomach and around 6.2 lakh females are suffering from breast, cervical, oral & Oesophageal cancer. The most common cancers reported in India are breast and cervical cancers in females and lung and oral cancers in males. Over 62,000 women died in India due to cervical cancer in 2015 latest estimates by the ICMR. In 2008 cervical cancer was the most common cause of death due to disease among Indian Women. However 2012 report by WHO shows breast cancer upstaging cervical cancer as the most common in Indian women. Public awareness of cancer prevention can play its vital role in early diagnosis and treatment. In view of this, University has undertaken Cancer Awareness Outreach Programme for the community through different organization like National Service Scheme (NSS), Nursing students and expanding towards Anganwadi workers focusing on awareness among rural population for early cancer detection with the leadership of Honorable Vice Chancellor Dr.M.Vijaykumar Onco-Surgeon. In the light of present knowledge early detection and prompt treatment of early cancer and pre cancers conditions provide the best possible protection against cancer for the individual and community. Objectives:

 To create awareness about the risk factors and early clinical features of oral, cervical and breast and general caners.  To inform people about the importance of early detection of cancer.  To create awareness about facilities regarding cancer screening and treatment.  To educate them about existing social security scheme with respect to cancer treatment.

Evolution of the Cancer Awareness Outreach Programme:

Phase1: Planning of the programme –  At the 1st meeting we had discussion on how to go about the programme. Members suggested different organization like NSS, Lions Club, Inner wheel, Rotary club etc. At the beginning we had mainly concentrated on training NSS Programme officers and volunteers of Dakshina Kannada, Udupi and Kodagu Districts with the aim that they are the ideal people to promote cancer awareness for the rural population through their special camp programme. Our talk on Cancer Awareness equips and empowers trainees building their knowledge, skills and confidence to talk about the cancer for the rural population.

Phase 2: Formulation of the Committee under Yenepoya University

 Officially on 19th of August 2015 we formed committee, as per the norms of Yenepoya University .Committee consist of a Programme Chief Patron, Patron, Chairman, Convenor, Coordinators and members. NSS Programme Officers were appointed to establish a cancer awareness and early diagnosis work stream, in order to improve population outcome from cancer locally and to make cancer more of a priority for those working in community health.

Phase 3: Identification of organization:  In each Meeting all the members of the CAOP have given different suggestions for conducting the programme. The suggestions were to conduct awareness for NSS Volunteers, NSS Programme Officers, Different NGO’s, Rotary Inner wheels, Lions Club, Rotary Clubs organizations and Industrial Workers, Youth Organization & Rural Community.

Phase 4: Development of training Module:  On 25th August we had conducted meeting with Honourable Vice Chancellor who had suggested to prepare module for Breast, Cervical, and Oral & on General Cancer accordingly the task has been divided by the team and worked on it.

Phase 5: Critical review, test run validation of module etc.  On 10th September we had presented the module of all the four cancers and also test trial done with the presence of housekeeping staffs and accepted their suggestions and modified. Critical review done by the experts.

Phase 6: Translation of module into different local languages  On 28TH September In the meeting content of the module finalized and planned to edit in a different languages like tulu, Konkani, Beary, Malayalum, Kannada and Hindi and in future to distribute the CD to all the programme officers those who have participated in the training programme. Through this Information on CD NSS Volunteers give awareness to the community. As per our plan CD contains short film on cancer awareness, dignitaries messages, Message from the celebrities and the ppt on breast cervical, oral and general cancers in different languages

Phase 7: Launching of the CAOP  The Yenepoya University Cancer Awareness Outreach Programme launched on October 1st 2015 at Indoor Auditorium Yenepoya University Campus and organized programme for NSS Programme officers of Dakshina Kannada Kodagu and Udupi District.
