If you wish different classes at different trials, please clearly describe (such as Open A on Sat AM, Open B on Sunday). Check in which trials your dog is entered, and the specific special awards. Awards are entries in the REGULAR OBEDIENCE TRIAL CLASSES ONLY. Castro Valley Blvd #404 Completing CDX Castro Valley, CA 94552 Completing UD Working Therapy Dog CLOSING DATE FOR ENTRIES: WEDNESDAY, DECEMBER 28, 2011, at 12:00 NOON, or when limits are reached prior to December 28, 2011, after which time entries will not be accepted, cancelled, substituted, or changed. ‘ SAN FRANCISCO DTC - SATURDAY AM Obedience + Rally Working Therapy Dog The Obedience entry fees shown above include an AKC Recording Fee of $.50 for first entry of each dog entered in any Licensed or Member Club Show, Obedience Trial or Tracking Test plus an AKC Event Service Fee of $3.00 for every entry (first and additional) in the Senior Handler (62 years & over) recordable single entry classes. These fees are collected by the Show-giving Club and paid to the American Kennel Club. (AKC Rules Earning First Leg in Utility A Applying to Dog Shows, Chapter 11, Section 2, effective January 1, 2008). Each first Rally entry fee of a dog includes an AKC Recording Fee of $3.50. Each additional Rally entry fee for same dog includes $3.00 Recording Fee. ‘ SANTA CLARA DTC - SUNDAY Rescue Dog Senior Handler (60 years & over) CREDIT CARD ENTRIES VIA FAX OR EMAIL -- See the Credit Card Form at the back of the online version of the Therapy Dog Premium List. Include the Credit Cardholder’s name, Mastercard or Visa number, Expiration date, Security code, Cardholder’s signature, Address and Telephone number. No AmEx accepted. Faxed and emailed entries (using a PDF file) MUST INCLUDE THE AGREEMENT printed alongside the Official AKC Entry Form (Hint: print landscape Be sure to include your dog’s AKC titles on the Official AKC Entry Form so the remaining special orientation on the paper) and must contain your signature. THERE IS AN ADDITIONAL CHARGE OF $5.00 PER DOG prizes can be determined from the dog’s titles. PER TRIALFOR CREDIT CARD ENTRIES. Please mail this form with your Official AKC Entry form, or give to the Trial Secretary’s desk on the day of the Trial. Special Award Forms must be on file BEFORE your class starts. NOTE – If you use INFODOG (MB-F) for the convenience of online entry, we remind you that they close entries 2 ½ days earlier for non-MBF-superintended events. This is because they will print your entry, issue a check, and Fed-Ex overnight across the country. You are charged at least $15 for this service and your entry may be delayed in arriving.
