While its introduction into service was controversial, it quickly found its way into the hearts and minds of Australians, and none more so than the men and women of Boeing. Over the past 15 years, Boeing and its people have developed a special bond with this remarkable aircraft. From the commencement of our first F-111 contract, the Avionics Upgrade Program, on 15 January 1995 through to its farewell flight, Boeing has worked in close partnership with the RAAF, industry partners and the Commonwealth of Australia to ensure its technology, weaponry and airframe remain cutting edge. For Boeing, the retirement of the F-111 signals the end of a very proud period in our company’s history. It has been a source of tremendous opportunity and great pride for our employees. In reflecting on Boeing’s history with the F-111, there are many proud moments. We have performed significant repairs, modifications and upgrades that enhanced the fleet’s lethality and survivability, Dennis Muilenburg and we worked as partners to ensure it remained a President & Chief Executive Officer formidable supersonic long-range strike aircraft for Boeing Defense, Space & Security Air Force. Most of all, as the F-111 flies into the history books, we salute the men and women of the RAAF who have operated and maintained this iconic Australian aircraft. It has been our privilege and honour to support our RAAF customer, and to work with our friends and allies to preserve and promote freedom around the globe. Thank you for all that you have done. iv Rarely has the acquisition of one aircraft type had such a far-reaching impact on a fighting force as has the F-111 with the Royal Australian Air Force. So as the aircraft goes out of service in December 2010, it is with much pleasure that I introduce this book. The F-111 has been an incredible success story, and the RAAF has had a love affair with the ‘Pig’ for nearly 40 years—as has the Australian public. It has been a familiar crowd pleaser at air shows and its signature ‘dump and burn’ routine will be sadly missed. There can be little doubt that the F-111 will find its place in history for its contribution to Australian defence as the cornerstone of our strategy of deterrence. That such a complex weapon system could remain relevant is a testament to the many hundreds of men and women who flew, maintained, updated and supported the F-111 fleet over the four decades it was in service. Despite the F-111’s success, there is also much that the RAAF has learnt from operating it - both in the air and on the ground. Perhaps the most significant was the deseal/reseal program which led the RAAF Air Marshal Mark Binskin, AO to change the way it managed its people and its Chief of Air Force maintenance practices. As well as the RAAF’s highly skilled technicians, I single out the role that defence industry has played, particularly over the last 20 years. Without that support, the aircraft would have retired at least 10 years before its time. I extend my personal thanks, and those of the Air Force F-111 community, to Boeing for their partnership with us on this remarkable aircraft. I particularly want to commend the company’s sponsorship of this book.     “I have the Fokker in sight!” After some delays caused by tropical cyclone “Gita” the first of our two Fokker F27 Friendship aircraft arrived from New Zealand on Monday 27 February. Now registered VH-EWH, the Friendship departed Auckland on Sunday 26 February, stopping to refuel at Norfolk Island before proceeding to Coolangatta to clear customs. The intention was to proceed to Wollongong that same day, but bad weather in the Illawarra dictated an overnight stay at Coolangatta. The second aircraft will be ferried sometime in the next few months. In Australian service the Fokker F27 Friendship was operated from the late 1950’s by many of the domestic and regional airlines. Some of the Australian airlines which operated the type include Trans Australia Airlines, Ansett ANA, East West, Queensland Airlines, Bill Peach’s Aircruising Australia and Airlines of NSW. The type was also operated by the Department of Civil Aviation and CSIRO. The Fokker F27 was developed during the early 1950’s and was intended to compete with or replace piston engine aircraft such as the DC-3. It uses Rolls Royce Dart turboprop engines, is pressurised and, in typical configuration seats around 50 passengers. Our F27s were most recently in service with a company that provided services to New Zealand Post and our Kiwi readers would be well familiar with the sight and unique sounds of the F27s as they made their way around the mail runs. Convair CV-440 progress The return to flight project for our Convair CV-440 made significant progress this month with completion of the new engine installation.

    ATTITUDES TO SAFETY AND ORGANISATIONAL CULTURE IN AUSTRALIAN MILITARY AVIATION B. T. FALCONER A thesis submitted in fulfilment Of the requirements for the degree of Doctor of Philosophy Department of Aviation University of New South Wales June, 2006 ABSTRACT This thesis describes original research that examines the extent to which organisational culture, and psychosocial aspects specifically, relate to individuals’ ‘normal’ performance within Australian Defence Force (ADF) aviation. The primary rationale for the research relates to the ‘safety record’ of ADF aviation, whereby more than fifty ‘peace time’ fatalities have occurred in ADF aviation accidents since 1990 and many of these have links to organisational culture attributes. The secondary rationale relates to a more general perspective: previous research identifies human functioning in military aviation – more than any other aviation domain – as being dependent upon psychosocial attributes including interpersonal collaboration, communication and coordination. However, the depth to which such qualities impact the safety of a sociotechnical system remains substantially uncharted. The methodology of this research has been developed to fit the context of ADF aviation, in contrast to applying pre-existing ‘universal’ methods (e.g., Hofstede’s cultural dimensions) developed without reference to the cultural attributes of specific organisations. More generally, though equally important, this research is further innovative in that it responds to the challenges posed by the need to address systemic aspects of safety confronting aviation and numerous other sociotechnical industries. This thesis firstly examines both scientific and Australian military literature on organisational behaviour, culture and human factors. Subsequently, it describes the design and implementation of a new 45-item questionnaire – the Australian Defence Force Aviation Questionnaire (ADFAQ).
