Annual Report 2015-16

Organizational activities:

In accordance with the Rules of the Academy, a report on the general concerns of the Academy, the income and expenditure of the preceding year along with the estimates for the year, and the prosperity or otherwise of the Academy is to be presented to the General Body at its Annual Meeting.

Annual Meeting:

The 55th Annual Meeting was held on 16th, 17th and 18th October, 2015 at the All India Institute of Medical Sciences, Patna (Bihar). The Convocation of the Academy was held on 17th October, 2015 evening.

His Excellence Dr. Ram Nath Kovind, Governor of Bihar, was the Chief Guest.

Dr. Mukund Sadashiv Joshi, President, NAMS in his Presidential address, welcomed the distinguished Guests, Fellows and Members of the Academy. The full text of the Presidential address is given as Annexure I.

17 Lifetime Achievement Award

Lifetime Achievement Award of the National Academy of Medical Sciences (India) for the year 2014 was presented posthumously to Late Dr. C.S. Bhaskaran, FAMS; President, NAMS (at his residence to his wife). Dr. Bhaskaran was an outstanding teacher, excellent research worker and an able administrator in recognition of his proven track record of professional excellence and subject expertise of high order, his qualities of equanimity academic eminence, and professional excellence.

24 NAMS 2007 Amritsar Award

25 Meetings of the Council

The Credential Committee considered 165 biomedical scientists for election to Fellowship. Of these, 82 were new nominations (58 direct and 24 advancement) and 83 were carried forward nominations (47 direct and 36 advancement) for the year 2015. Membership of the Academy under Regulation V – The candidates who have passed the examination conducted by the National Board of Examinations and have been duly proposed for Membership (MNAMS). Regulation V provides as under:-

“Those candidates who pass the examination conducted by the National Board of Examinations will individually submit an application for admission as Member of the National Academy of Medical Sciences, duly proposed by at least one Fellow of the Academy certifying the character and conduct of the candidates. Subject to the approval of the Council of the National Academy of Medical Sciences, the candidate will be admitted as Member after paying one time Life Subscription fee of Rs.7,000/- (inclusive of admission fee of Rs.1,000/-) as may be determined from time to time, and after executing the Bond of Obligation.”

Accordingly, the application forms were issued by the Academy to the candidates on their request.

54 Golden Jubilee Commemoration Award Lecture

On the recommendations of the Credentials Committee and with the approval of the Council, the youngest biomedical scientists elected as a Fellows during the year is invited to deliver Golden Jubilee Commemoration Award Lecture at the Annual Conference of the Academy.

Dr. Ashish Suri, FAMS Professor, Department of Neurosurgery, All India Institute of Medical Seciences, New Delhi was the youngest biomedical scientist elected as a Fellow during the year 2014. He delivered Golden Jubilee Commemoration Award Lecture on 18th October, 2015 at All India Institute of Medical Sciences, Patna. The Topic of his Lecture was “Simulation Based Skills Training in Neurosurgery and Contemporary Surgical Practices”.

Lifetime Achievement Award

The Council at its meeting held on 4th August, 2015 under Rule 34(d) also approved the conferment of Lifetime Achievement Award for the year 2015 on Air Marshal (Retd.) Dr. M.S. Boparai, FAMS; in recognition of his life-long devotion to the cause of integrity in outstanding professional excellence and leadership in the field of Ophthalmology and Medical Education, with qualities of equanimity and academic eminence, have proven track record of excellence and subject expertise of a high order. He has served the Academy with distinction as Council Member, Emeritus Professor and Members of several standing Committees.

Maintenance of the Buildings

The expenditure incurred on the maintenance of the administrative building is shared by the NAMS and the NBE on 50:50 basis. The jobs of house keeping and security have been entrusted on contract basis.

The following repair work of the Academy’s were done in four phases as per details given below:-

55 1. Repair of damaged ceiling, waterproofing of terrace 2. Providing & laying Over head Tank, Chajja and Grit wash 3. Repair/Replacement of second floor damaged G.I. and Sewage pipe lines at second floor 4. Renovation of first Floor oiletsT & linkages of all sewage pipe lines etc.

The tenders for work mentioned at S.No. 1, 2 & 4 were called by the Academy and work was awarded to the shortlisted contractors. The work has been successfully accomplished. The work, related to No. 3 above, was done by the National Board of Examinations as communicated by them.

The Audio System has been installed in the Auditorium building and is operational.

The Air-conditioners Plant starter, which is fitted at the ground floor, has now been connected with the A.C. Plant and the operation of the A.C. Plant has become smooth for its routine operation (On/Off), as and when required.

56 Publication of Annals

1. Report on Annals of National Academy of Medical Sciences (India)

NAMS Center for Research in Medical Education at AIIMS Jodhpur was given additional responsibility to look after publication the Official Journal of Academy - Annals of the National Academy of Medical Sciences (India). At Jodhpur we have been able to publish seven issues of the Annals in 2 years. These are:

1. Golden Jubilee Lectures, 2013; 2. Sleep Medicine, 2013; 3. Research in Medical Education, 2015; 4. January- June issue 2014; 5. July- December 2014; 6. June- Sept 2015; 7. October- December 2015.

The next issue of 2016 (Volume 52 No. 1) is now ready for printing and distribution.

During the financial year from 1st April 2015 - 31st March 2016, three issues of Annals have been published.

An informal meeting was held on 23 January, 2015 at 10.30 am at the NAMS Camp Office. Agenda of the meeting was discussed over a period of time during teleconferences and e-mails. A decision was taken to ensure the continuity of the publication of the Annals and it was decided that there will be no change in the size, format, and of the front page colour etc. from January, 2015 issue.

Printing of Annals of NAMS : Approximate cost of Annals including cost of printing, cost of envelopes, postal charges etc has been fixed now.

2. “According to the rule, the term of Editor and Editorial Board is 3 years. The Editorial Board was constituted in 2007 and 57 approved in the Council meeting held on 12 July, 2008.” The last issue of the Annals was published in October, 2010 on “Diabetes Mellitus” since then the same Editorial Board has continued but no single volume of Annals has been produced. Prof J S Bajaj was authorized to constitute a new Editorial Board for the Annals of NAMS. The new Editorial Board which was presented in the meeting of the Council on 4 October, 2014 was endorsed and become effective from first January, 2015, have printed in the first issue of the Annals of January, 2015 and circulated. Dr. Kuldeep Singh has been appointed as an Associate Editor from July 2015.

3. All the future issues of Annals of NAMS which will be printed at AIIMS, Jodhpur will be put on the website of National Academy of Medical Sciences by the NAMS Centre for Research in Medical Education, Jodhpur as http://annals-nams.in within 3 days of its distribution without involving any extra cost.

4. The NAMS Centre for Research in Medical Education established at AIIMS, Jodhpur, will make efforts for indexing of Annals if the requisite continuity of publication is maintained. There will be no cost involved. If we achieve this target, Annals of NAMS will be on PUBMED (international indexing system). The requirement to provide contact details of all the Members of the Editorial Board including their postal address, mobile numbers, e-mail etc. This will not be in a public domain but is the pre-requirement of the organization.

5. Till the time PUBMED indexing can be achieved, the NAMS Center will explore indexing in several other open indexing systems which will widen the visibility of the journal. We have got following achievements:

a. Google analytics verification done, our journal ranks 2nd while searching in google and is 2nd only after official NAMS website. b. Google scholar is showing in citation index. c. Dataverse project at Harvard has established a link with our journal and all articles will be extracted to it’s database. d. d. The WorldCat is indexing and having repository of our articles

Efforts are being initiated by the NAMS Centre for Research in Medical Education, Jodhpur, for the online submission of manuscripts while the Emeritus Editor will continue assigned responsibilities including editorial corrections and review all the manuscripts for any pertinent comments made by any Fellow of the Academy and for that the email addresses and affiliations of all Reviewers, Fellows and Members will be obtained. Back issues in ‘pdf’ format will also be uploaded that will help in pubmed indexing.

On the occasion of the 55th Annual Conference of the National Academy of Medical Sciences (India) – NAMSCON 2015 from 16th to 18th October, 2015, it gives me very great pleasure to welcome all delegates, invitees and participants at All India Institute of Medical Sciences, Patna, the land of Patliputra where great rulers ruled the world. The National Academy of Medical Sciences was established with the primary objective: ‘the cultivation of scientific knowledge and its application to human welfare’. I am glad that the seed of progress of the Academy over the years has grown to the extent of fulfilling of this objective through applied, basic and community health research by large number of Indian scientists, some of them are honoured by Fellowship of this Academy itself and that number is gradually growing. It is a matter of great satisfaction that the Academy is doing excellent work to achieve its objectives. The NAMS Annual Conference is preceded by a CME programme on “Maternal & Child Health: Upto and Beyond 2015”, that will provide a great opportunity for fruitful deliberations and will enhance the knowledge of young minds in this field and will provide an opportunity to interact with one another and with experts in this field. The Academy recognizes outstanding achievements of scientists by conferring Fellowships, Memberships, Orations and Awards and I heartily wish to congratulate all the new Members and Fellows who would be receiving scrolls during the convocation ceremony. My salutation to all of them and my heartiest good wishes to others who I firmly believe shall be recognized in the future. Guests, Director, Faculty, Students and employees assembled on the occasion of the 55th National Conference of the National Academy of Medical Sciences Hosted by AIIMS Patna, I welcome you to Bihar. The National Academy of Medical Sciences was inaugurated by the then Prime Minister of India, Pandit Jawaharlal Nehru, on 21st April, 1961. Since then they have provided Fellowship and Membership to many Doctors on the basis of excelling in Academic in their profession. My best wishes and congratulations to those being conferred Fellowships and Memberships this year. They also help organize such conferences to solve the regional problems. I hope that in this conference too deliberations will help solve regionally relevant Health problems in Bihar. The All India Institute of Medical Sciences was envisioned to correct the regional imbalances in Health and Medical Education in India. How will the benefits of modern science reach the last man in the queue, at his door step at affordable cost is the mandate of such Institutions to be dedicated to the Nation in sufficient quantity. With this mandate this 3 year old baby Institution is already Unique as compared to other medical teaching Institutions of the country in many ways. You can have an idea of how from their publication AINA 12, 13 and 14 in which the news reports describing the difference have been presented. How would the simple, safe, low cost and effective treatment be delivered to the remotely placed last man have been tackled by them in many ways? They helped train the Quick Medical Response Team being stationed and networked in remote areas by the Disaster Management Department of the Government of Bihar. They have a virtual vidyalay for the common man who wants to learn to respond correctly to medical emergencies or disaster at their Tele-medical Education site http://www.sankatmochannagrik.in. They have devised an application called Sankat which facilitates sending a pre-coded message by a common man such as an Auto driver through a mobile which identifies the location of the disaster or emergency ona Google Map on the mobile screen of the First responder ambulance, Fire

62 Tenderer or police so that the nearest help can reach without delay. There Chattisa is a remotely placed computer-savy youngster who gets credible free consultation from an AIIMS specialist through video conferencing through net saving the remotely placed patient at least Rs.2,000 from a trip to Patna to get the same consultation. I have been told that since its inception by the then Health Minister of Bihar, Shri Ashwani Choubey on January 23, 2013 the program had provided consultation to 87000 patients and saved the trip of 50000 that is Rs.10 crores of the poor man’s money on Health. Their micro-mapping of the AES JE 2012 epidemic strengthened the hypothesis of it being caused by an Entero Feces borne water transmitted Virus along the North banks of Saryu and Ganga. Using the information the State Health Department by an intensive water contamination control and detection, retrieval and SOP based management was able to decrease the incidence of the disease in Muzzafarpur from above 1300 last year to less than a 100 this year. Besides these patients attending the Institutes OPD can register themselves through their mobiles avoiding large queues. All their Data including images, video, text, and numbers are stored on the net from the point of creation and can be retrieved through mobile at multiple distant points, used and updated. Being digital from the very beginning the data can be used for Research and creation of Good Clinical Practice and Policy Guidelines. They have done 2000 major surgeries like cochlear implant and joint replacements in their 4 modern theatres without mortality since March 2014. Soon they will have 3 more theatres to reduce waiting lists and with installation of CT Scan and Blood Bank in near future their emergency will start. I have been told that their teaching techniques include e learning and are very modern and developed. Their ability to create powerful Friends like RMRI, UNICEF, PATH, WHO, State Governments Disaster and Health Departments, IIT and IIM of Bihar and NAMS nationally and network with them will help everyone fight the unending war against Death and Disease. I wish you success in being useful to the Nation, World and Humanity and stop here. Jai Hind

During the year 2015-2016 meeting of the CME Programme Committee was held on 10th July, 2015.

The Committee, time to time, co-opts Fellows to attend the meeting, depending on the expert advice required based on the topics of discussion and CME proposals received.

Extra-mural CME Programmes: These CME Programmes are run under the aegis of CME Programme Committee of the Academy. To improve the quality and contents of the Extramural CME Programmes funded by the NAMS, proposals received for funding are technically reviewed in the first instance by a subject expert who is also a Fellow of the NAMS. This practice has been followed since July 2004, and the reviewers so designated would evaluate the proposal and identify lacunae, if any, in the programme contents. The reviewers may suggest revision/ modification to be incorporated in the programme. These are communicated and nearly all such suggestions are invariably accepted by the organizers and only after the CME Scientific Programme are revised that the CME proposals processed for the grant-in-aid. A Fellow of the Academy is 67 designated as Observer to attend and evaluate the extra-mural CME Programme. The Academy provides TA/DA and Honorarium to Fellows who are nominated as Observers to attend, review and submit report on the CME Programmes.

Out of the CME proposals received from various medical institutions/ professional bodies in the country, the Academy has sanctioned partial financial assistance for organizing Nine Extramural CME Programmes during the financial year April 1, 2015 to March 31, 2016.

The details of the Extra-mural CME Programmes funded during April 1, 2015 to March 31, 2016 are placed as Annexure III.

The total expenditure sanctioned on Extramural CME Programmes during April 1, 2015 to March 31, 2016 is Rs.5,34,835 lakh (Rupees Five lakhs thirty four thousand eight hundred and thirty five only)

Intramural CME Programmes: These Symposia/CME Programmes are run under the aegis of Academic Committee of the Academy. The Academic Committee identifies, from time to time, topics of National and academic relevance for funding as Intramural CME Programmes. Just like for the extramural CMEs the Academy provides TA/DA and Honorarium to Fellows who attend and submit evaluation report on the CME programmes as NAMS Observers.

The Academy has funded 5 Intra-mural CME/Symposia during the year 2015-16.

Main Objectives of the CME Programme

The main Objective of the Workshop were:

1. To create a Nation-wide awareness on the system for patient safety reporting; 2. To support regulatory agencies in decision making process on the use of medication; 3. To collaborate with other national ADR reporting centers for the exchange of information and data management; 4. To generate evidence based information on safety of medicines.

Highlights of the Observer’s (Dr. V. S. Mathur, FAMS) Report

Adverse Drug Reaction Monitoring and Pharmacovigilance needs active participation of both pharmacologists and clinicians, as the latter participate by way of prescribing the medicines and the former participate by managing the ADR reporting centre in the medical college hospital in the overall patient care activities. Besides, the SHKM faculty, guest speakers were invited from the neighbouring medical institutions. The CME was attended by around 120 doctors, nurses, pharmacists from the medical colleges from Rohtak, Greater Noida, Aligarh, Hapur, Faridabad, Meerut and Muzaffarnagar. The CME cum Workshop started

75 at 9.30 am and closed at 6 pm with a lunch break of 30 minutes. A well designed scientific programme with expert speakers, who spoke on various aspects of Adverse Drug Reactions (ADR) was the highlight of the CME. The speakers through power point presentations dwelt on their respective topics. Each presentation was followed by questions- answers and comments by the audience. The programme included a demonstration cum hands-on exercise on as how to fill the CDSCO-ADR Form. All efforts were made to have interactive sessions and didactic lectures were avoided. The speakers were able to highlight the goals of the CME: (i) increased awareness of possible ADR and making a note in patients file by doctors, nurses, pharmacists and even patients reporting any untoward reaction to prescribed medicines, (ii) evidence- based selection of medicines and to remember that medicines are a double-edged swords and all drugs are potentially toxic when misused or taken without following doctor’s prescribing directions, resulting into non-compliance, (iii) recording all suspected ADRs in the CDSCO Form, (iv) post-marketing surveillance (PMS) employing all means to detection of known and so far unknown harmful effects, (v) identifying the problem areas in Mewat where there are a large number of patients on anti- tuberculosis drugs, e.g. INH, Rifampicin, Pyrazinamide and Ethambutol, PMS is immediately needed and the health care personnel be mobilized to study these problems. The CME was a well organized, much needed workshop in a remote and backward region of Haryana. The government of Haryana in this remote area here has not only provided a medical college but also a ultra-modern hospital with all specialties and super- specialities, well qualified and skilled consultant staff and infra-structure made me and everyone feel proud of all the praise and worthy efforts of the State government.

2. CME Programme on “Theory and Practice of Epidemiology, Biostatistics and Demography” held on 6th to 7th July, 2015 at St. John’s Research Institute, St. John’s National Academy of Health Sciences, St. John’s Medical College, Bangalore.

Highlights of the Observer’s (Dr. A. V. Kurpad, FAMS) Report

The program was held in a well lit seminar room with ample seating space for all the participants. There were sufficient computing facilities provided with laptops, internet connection and access to statistical package. All participants were involved in the discussion, raised questions during the lectures and actively participated in the practical sessions. All the presentations were shared with the participants before the session. All sessions were aided with power-point presentations with clarity and supported with discussions involving the participants. Sufficient audiovisual aids were available. All course material, practical questions data, pre-test, post-test session evaluations were all available on the e-learning platform (TYRO). All discussions were supported with appropriate examples from the clinical and non-clinical subjects.

3. CME Programme on “Gynecon 2015: Critical Care in Obstetrics & Gynecology” held on 25th – 26th September, 2015 at the Armed Forces Medical College, Pune.

77 Main Objectives of the CME Programme

The main objectives of the CME were :

1. To educate gynaecologists and residents, on the subject of intrauterine invasive procedures for diagnosing foetal chromosomal abnormalities. 2. To educate about ultrasound features of foetal malformations and give an insight into pathophysiology of intrauterine foetal problems and to acquaint with latest advances in foetal therapy. 3. The CME program shall focus on setting protocols for managing critical maternal conditions like haemorrhage, sepsis, hypertension and thrombotic conditions.

Highlights of the Observer’s (Dr. Kamal Buckshee, FAMS) Report

An overview of scope and advances in Foetal Medicine was presented. This was followed by video presentation of various intrauterine procedures, viz. amniocentesis, chorionic villous sampling, cordocentesis, intrauterine transfusion, foetal reduction and foetoscopy. Ultrasound: An elaborate demonstration of nucal thickness, nasal bone, foetal anomalies and dopper flow (normal and abnormal doppler flow of uterine and umbilical arteries, ductus & cerebral arteriolar flow) was done. Role of MRI and its correlation with respect to foetal anomalies was highlighted. Dr. Arun kinare’s presentation was excellent with respect to its clarity, content, simplicity, information, update and was evidence based. It was a good session.

Session 1 on Medical Emergencies in Obstetrics: Peripartum Cardiomyopathy and Jaundice in Pregnancy that has a high maternal and foetal mortality were discussed. Lectures on these difficult topics highlighted the pitfalls in diagnosis and thus suboptimum management. All these aspects were well covered. The postgraduate quiz was excellent. Everybody enjoyed it thoroughly. Free paper session for postgraduates & young gynaecologists rating was 2.75/5. Only few papers were of quality presentation. Session on gynaecological emergencies: were well covered to a great extent. The inaugural function was fabulous. Session

78 3 on Obstetrics/Anesthetist nightmares was too good. he planning of the CME programme, the topics and faculty selected was very good. The academic deliberations and conduct of the workshop was very apt and informative. The ambience of the symposium was exceptional. The participants were very enthusiastic and remained present most of the time. They seemed to have enjoyed the workshop on foetal medicine & sessions on medical emergencies in obstetrics & gynaecology. There was a good interaction by the consultants; however; the interaction by PGs, young residents during the workshop and scientific sessions on emergencies was not much.

4. CME Programme on “Ultrasonography: A Futuristic Vision in Teaching and Learning Anatomy” during the National Conference of Anatomical Society of India held on 20th November, 2015 at King’s George’s Medical University, UP, Lucknow.

Main Objectives of the CME Programme

Ultrasonography (USG) has emerged as an indispensable diagnostic tool as it is convenient, economic, non-invasive as well as efficient. Therefore, the medical students should be exposed to this basic tool right from the beginning. Incorporation of USG for teaching, learning living anatomy would make the subject interesting and clinically relevant. This CME would therefore help in sensitizing the faculty of Anatomy to use this modality for teaching and research purposes.

It was very a well organized CME. The participants and organizers were very enthusiastic. The resource faculty were experts in the field of ultrasonography and discussed the ultrasonograhic anatomy of abdominal viscera, musculoskeletal system and vascular anatomy of limbs, ultrasound of neonate brain and correlation of embryology with sonographic findings where relevant. The sessions were open for interactive discussion after each presentation. A state-of-the-art audio- visual aid was available and worked efficiently all the time which made all sessions very enjoyable, informative and interactive.

Sharma, FAMS) Report Overall the workshop was scientifically sound and quite beneficial to the participants for implementing principle of GCPs in the practice of Clinical Trials and Research. Eminent faculty and resource persons form acdemia, industry funding agency (ICMR) and regulatory agencies delivered their lectures in the workshop. Topics on ethics in research, responsibilities of different 80 stakeholders, ethical requirement of clinical trails, ICH-GCP, Indian GCP guidelines and lots of important topics were covered in the workshop. The lectures were thought-provoking and most of the topics covered will benefit MD/DM/Ph.D/MSc students in their respective exam preparation. Overall the workshop was beneficial and informative for the participants.

6. CME Programme on ‘Pharmacovigilance’ held on 17th December, 2015 at Saurashtra University, Rajkot.

The CME programme was organized along with main 48th Annual meet of the Indian Pharmacological Society (IPS). This CME is expected to bridge research of experimental pharmacologists with those involved in the applied aspects the medical and pharmaceutical sciences, allowing them to interact and cultivate interdisciplinary research spirit, specially in the area of pharmacovigilance.

Highlights of the Observer’s (Dr. R. Balaraman, FAMS) Report:

The one-day CME Programme on Pharmacovigilance as a part of IPSCON 2015 was an overwhelming success. Numbers of participants were more than 50. Many of them are clinicians, persons working in

81 pharmaceutical industries and the PG students of medical and Pharmacy colleges. The objective of the symposium was fulfilled due to the excellent talks given by several stalwarts in the field of Pharmacovigilance and Adverse Drug Reaction Monitoring activities. Initial lecture was on the introduction of Pharmacovigilance practice and its scope in India (PvPI). Second speaker spoke on the role of clinicians in implementing effective pharmacovigilance practice in hospitals and private clinics. There were talks on under reporting of ADRs in implementing the pharmacovigilance programme in India, role of National Co-ordinating Centre in Pharmacovigilance Program in India and ICH guideline and regulatory aspects on PVP1. Finally there was a talk on terminologies for coding ADRs and Drug Informations. There was a practical session on operation of VigiFlow software and hands-on data entry on UPPSALA Monitoring Centre for ADR Reporting through internet. There was a group exercise followed by pannel discussion on ADR reporting. The programme was well organized and a grand success.

Highlights of the Observer’s (Dr. Nancy Malla, FAMS) Report

The proposed CME scientific program held as part of “11th National Conference of Society of Indian Human and Animal Mycologists (SIHAM)” was well organized. Registered participants were 199, mainly postgraduate students and junior faculty members from different Medical Colleges in the country. The main objective of the programe was to disseminate awareness and knowledge of fungal infections and inculcate the ability to chalk-out the future research strategies, among the participants. The

83 CME scientific program was conducted in three sessions, including fungal infections in ICUs, therapeutic drug monitoring and antifungal resistance. Eminent faculty, scientists and senior research experts working on various areas and aspects of fungal infections delivered good quality lectures and shared their research experiences. The topics were quite relevant, keeping in view the public health problem, morbidity and mortality rates due to pathogenic fungal infections. Problem of antifungal resistance in clinical settings and therapeutic drug monitoring to improve outcome were amply discussed. The deliberations were interactive and active discussions were held at the end of each lecture, ably guided by the Chair-persons. The participants were quite enthusiastic, as was evident with their presence in the sessions and active participation in discussions.

9. CME Programme on the “Role of Clinical Pharmacologists in Antimicrobial Stewardship Programs” held on March 29, 2016 at Lecture Theater, PGIMER., Chandigarh.”

Highlights of the Observer’s (Dr. Nancy Malla, FAMS) Report

The CME academic programe was well planned and the topics deliberated were quite relevant to the theme. The 96 participants, mainly postgraduate students from different Medical Colleges in the country attended the academic-sessions. The main objective of the program was to disseminate awareness, knowledge and need to Antimicrobial Stewardship Initiative among the participants. The program started with the Pre-Test Question exercise by the participants followed by lectures, workshop and Post-Test Question exercise (MCQs). Eminent faculty, scientists and senior experts

84 presented basic and advanced strategies for implementing the program and highlighted the need for joint collaboration of the microbiologists, pharmacologists and clinicians for its ultimate success. Experts from the industry emphasized the role of industry in this endeavor. Developing research protocols for improving relevant practice; designing, conducting and reporting prospective audits along with optimizing dose regimens for antimicrobial agents were amply discussed. The lectures were followed by group workshops on case studies for applying principles of Evidence- Based Medicine to rational prescription of antimicrobials, which were ably guided by the faculty. The workshop session concluded with the presentation of given Case Exercises/Case Studies by a lead person of the group and thus, at the end, participants jointly discussed all the given Case Studies. The deliberations were interactive and active discussions were held in between, as well as at the end of each lecture. The participants were quite enthusiastic, as was evident with their presence in the sessions and active participation in discussions. At the end of the program, participants attempted Post-Test MCQs.

87 The Medical Scientists’ Exchange Programme (Health Manpower Development)

One of the activities promoted by the Academy under Continuing Medical Education Programme in the area of Health Manpower Development is the Exchange of Medical Scientists’ possessing positions at junior and middle levels.

The Academy provides funding to junior and middle level specialists/ scientists to go to well established centers of excellence and acquire new/ newer skills. Under the scheme, the selected nominees are eligible for reimbursement of traveling expenses (limited to actual II class AC two-tier rail fare) and D.A. @ Rs. 300/- per diem during the training period subject to maximum of Rs. 5,000/-.

One Hundred Ninety-Eight (198) Medical Scientists/Teachers have been selected advance training upto the year 2016 and have successfully completed their proposed training programmes after getting the grant-in- aid from the NAMS.

Highlights of the Observer’s (Dr. Umesh Kapil, FAMS) Report:

The objectives of the Symposium were fulfilled. The organizers have provided well documented Learning Resource Material. There was a total gathering of more than 236 (144 from outside and 92 inside) and more then 200 participants were present at azny time during the programme. There were interactive sessions as well as practical demonstration. There were interactive sessions and small group learning activities. The participants were provided practical demonstration/hands-on-training. Presentation by the Course Faculty was excellent and very informative. The Audiovisual aids used during the Symposium were of excellent quality. The information provided was evidence-based. Delegates 89 were impressed with the Quiz Competition, training on detection of food adulterant and scientific deliberations. They participated enthusiastically in all sessions and had good interaction with facilitators and speakers. The dissemination about the symposium should be done more widely so that participants from the different parts of the country could have attended the CME.

2. NAMS Regional Symposium on : “Nursing Care of Patients with Diabetes Mellitus Type-II” held on April 27, 2015 at Sri Guru Ram Das Institute of Medical Sciences & Research, Amritsar

1) to determine and understand how lifestyle factors that impact on diabetes management relate to strategies for patient education; 2) to complete an assessment of a patient with diabetes and implement an evidence-based patient care plan; 3) to identify a person with metabolic syndrome, pre-diabetes or issues with glycaemic control; 4) to develop an appropriate nursing care plan to prevent escalation of disease; 5) to enlist the strategies of optimum nutrition, feet care and self- care management in diabetes.

Highlights of the Observer’s (Dr. Ravinder Goswami, FAMS) Report:

The ambience of the Symposium and the enthusiasm of the participants were good. The objective of the Symposium was to educate the Nursing Personals with state-of-the-art practical knowledge required for managing a patient with type 2 diabetes and its possible complication at their end. The objectives of the Symposium were fulfilled up to 90% as outlined by the Convenor of the Symposium. However, the programme was over packed with lectures. As a result there was less scope of interactive session

90 with the participants. Besides, the participants found it difficult to concentrate throughout the duration of programme. I felt 10 hours stretch appeared too lengthy for such programme. It can be reduced to eight hours. The organizers provide a well-documented learning resource material to the participants. Number of participants registered was 300 and were present throughout the programme. The programme consists of didactic lectures with very little interactive lectures. The participants were not provided practical demonstration/hands-on training. The presentation by the course faculty was of a very good but there was scope of improvement by incorporating more medical personals. Audiovisual aids use during the CME Programme was of Medium Quality. The size of the Projector was less than ideal. The information provided was evidence-based.

3. NAMS National Symposium on “Maternal and Child Health: On and Beyond 2015” held on October 16, 2015 at All India Institute of Medical Sciences, Patna

91 Highlights of the Observer’s (Dr. Prema Ramachandran, FAMS) Report:

The objectives of the Symposium were fulfilled. There were excellent interactive sessions in which students participated vigorously and their questions were insightful and expressed their concern. A well-documented Learning Resource Material (LRM) was provided to the participants. 160 participants were registered; out of which 80-100 were in the morning session and 50-75 in the evening session. Each lectures was followed by discussion, there were two interactive sessions. Number of the participants were predominantly undergraduate and the theme was Maternal and Child Health: On and Beyond 2015. Presentations focused on the National policies, programmes, improvement in access to reviews and outcomes vis-a-vis MDGS. The presentations were of excellent qualities. They took into an account that majority of participants were undergraduate students and experienced fundamental concepts. The Audiovisual aids used during the Symposium were very good. The Academic content i.e. the state of art information provided was very good. Information provided was evidence based.

4. Initial Expert Group Meeting to evolve Guidelines on “PMR (Prevention, Management and Rehabilitation) in Stroke – A Multi-professional Team Approach” held on the World Stroke Day, October 29, 2015, at the Prof JS Bajaj Centre for Multi- professional Education, in Smt Kamla Raheja Auditorium, NAMS, New Delhi

Organizing Officer: Dr. Sanjay Wadhwa, FAMS, Professor, Dept. of Physical Medicine and Rehabilitation, AIIMS, New Delhi.

The main objective of the Expert Group Meeting was to frame Guidelines on “PMR (Prevention, Management and Rehabilitation) in Stroke – A Multi-professional Team Approach”.

The Expert Group Meeting was organized by Dr Sanjay Wadhwa

92 in the Smt Kamla Raheja Auditorium at NAMS, New Delhi to commemorate the World Stroke Day. This was the first Academic Programme held at the Prof JS Bajaj Centre for Multi-professional Education at NAMS. The programme was very well designed and the meeting was very well organized. The programme included Experts from the areas of Neurology, Radiodiagnosis, Public Health, Pharmacology, Nutrition, Nursing, Orthotics, Medico- Social Works, and Physical Medicine & Rehabilitation. A number of Posters printed by Dr Sanjay Wadhwa and related to World Stroke Day were displayed prominently at the venue.

The Meeting was inaugurated by Dr. Mukund S. Joshi, President, NAMS at 1 p.m. on 29th October, 2015 by lighting the lamp and delivering the inaugural address. Dr (Mrs) SS Deshmukh, Chairperson, Finance Committee, NAMS, Dr Prema Ramachandran, Chairperson, CME Programme Committee, NAMS, Prof. R.K. Chadda, Member of NAMS Council and Dr Deep N Srivastava, Honorary Secretary, NAMS also joined in Lamp lighting. The Meeting was blessed by Prof. JS Bajaj, Emeritus President, NAMS and Chairman, Academic Council, NAMS with his message of congratulations to Dr Wadhwa and best wishes for the success of this event. The Meeting concluded at 6 p.m.

The efforts put in organizing this first Academic Activity as an Expert- Group Meeting were appreciated by all the Expert participants. This activity was also displayed prominently as an important activity in India, on the official website of the global organization, World Stroke Campaign (http://www.worldstrokecampaign.org/ india-2015/1091-india-new-delhi-prevention-management-and- rehabilitation-in-stroke-a-multiprofessional-team-approach.html).

The Resource Persons included Experts from AIIMS and Safdarjang Hospital, New Delhi, representing important areas related to Stroke Prevention, Management, and Rehabilitation. The presentations made by Experts during the programme were of very high quality and evidence-based information was provided to enable the Expert Group to finalize the guidelines and make useful recommendations. 93 It was decided that Dr Sanjay Wadhwa should follow up this initial Meeting with holding of a NAMS Sponsored National-Level Academic Programme on Stroke, for wider consultation and participation at an appropriate time when feasible.

Main Objectives of the CME Programme:

At the end of the symposium the participants will be able to: i) review the national and state achievement in terms of Millennium Goals ii) Discuss the proposed health and nutrition related target in SDG and how these could be achieved by optimally utilizing the strengths of the Indian scenario and trying the overcome the existing constraints.

Highlights of the Observer’s (Dr. K.K. Sharma, FAMS) Report:

A total of 114 persons attended the Symposium in Delhi. Besides, an unknown number of participants attended the Symposium at AIIMS, Delhi and AIIMS, Jodhpur as the proceedings of the Symposium were telecast live by interactive video-conferencing to these institutions. Participants in all these centres took an active part in the symposium not only by listening the deliberations but also by posing questions for clarifications, etc to the respective speakers after each talk. A large number of those who had registered at the venue of the symposium in Delhi had expressed, at the time of registration, that they are not very familiar with the MDGs and the issues in SDGs or India’s progress in all areas of goals. Most of the registered participants expressed that this was the first symposium they were attending where these issues are being discussed. 94 In view of this, they had indicated their inability to complete the pre- symposium questionnaire. All participants were very enthusiastic about attending the programme and listened all lectures with rapt attention and asked very pertinent questions and sought clarification on a number of issues. Organizers fulfilled the objectives outlines in the CME Programme proposal. Organizers tried to fulfill the set- out objectives in the best possible way they could by designing the Scientific Programme in the manner that all areas as per objectives were covered and in case there were some lacunae and some points which remain uncovered were covered by the Convener. This was conveyed by Dr. Prema Ramachandran, in her concluding remark during the final discussion on the Transition from MDGs to SDGs: India’s Challenges and Opportunities.

A well organized souvenir-cum-background paper was provided to each delegate as folder, which gave all details of the CME Programme. At any time till the end, around more than 100 participants remained present in the Symposium. The Scientific Programme consisted of didactic lectures and a number of epidemiological case scenarios were included in the presentation of each speaker. Full opportunity was provided to participants for interaction with the Resource Persons. However, there were no small group learning activities as this was not required as per Objectives of the Symposium. All the Resource Persons presented authoritative data supported by the State and National level epidemiological evidences as included in the Government reports on the proceedings of the current status of MDGs and SDGs. All audio-visual aids were excellent not only in the Auditorium where the scientific deliberations were held but also the quality of telecasted deliberations and questions-answers sessions all through the programme. All the speakers and resource persons who were chosen by the Organizers are Government of India dignitaries, Ministry officials and Academician involved in conducting and evaluation the MDGs and SDGs proceedings in the country.

1. Summary of Lectures delivered by Dr. T.D. Chugh, Emeritus Professor, NAMS

(i) Tropical Fevers, on 27th July, 2015 at IAMM.

Tropical fevers are prevalent in tropics and subtropics, often seasonal (during rainy or post rainy season) and frequently vector borne. India has a huge burden of these infections caused by bacteria, parasites and viruses. The most common are rickettsial infections, viral (dengue, chikungunya, influenza, enteroviruses), malaria, enteric fever and leptospirosis. Clinical manifestations are varied: fever with no localising signs or fevers with rash, ARDS, encephalopathy, acute renal failure, jaundice or multiorgan failure. Laboratory diagnosis and management is done using a syndromic approach. Prevention and control is a public health issue.

(ii) Antimicrobial Stewardship (WHO Fellowship Training Programme on Antibiotic Resistance Testing, Quality Assurance and Surveillance), on 3-8th August, 2015

I made the presentation through two case studies” a case of uncomplicated enteric fever and a case of MRSA bacteraemia in a case of ESRD on haemodialysis. The session was interactive for 2 hours.

Emphasis on microbiology reports “Accurate, Timely and Affordable” and use of antimicrobials with a sound knowledge of Pk and Pd of drugs.

Interaction of microbiologists and pharmacologists with clinicians is essential. It must be on regular basis.

101 iii) Patient Safet, on 16 August, 2015 at SGT University, Gurgaon

Spoke briefly about Shri GuruGobind Singh ji: the spiritual master, his moral philosophy, human values, ethics and the relevance to practice of medicine today.

Medical errors, adverse drug events and unsafe injections are a huge burden in India. “Complications generate revenue” is unacceptable.

Leadership in Medicine is the Need of the day.

iv) Microbes and Non – Communicable Diseases on 20 September, 2015 at Cuttack, Odisha

Professor Bikram Dass was not only a scientist and teacher of microbiology but also a political revolutionary and Man of Letters who wrote poetry, fiction and novels.

Human – Microbe Mutualism is not only related to infectious diseases but microbes are known to cause cancer: Burkitt’s lymphoma, cervical cancer, hepatocellular carcinoma, gastric carcinoma and MALT etc and various life-style diseases (metabolic diseases, obesity, diabetes mellitus, cardiovascular diseases)

The role of gut microbiome, probiotics and faecal microbiota transplantation was discussed.

v) Immunodiagnosis of Infectious Diseases on 19th November, 2015 at AMBICON

Various in – vitro immunological techniques have been used in the practice of infectious diseases. These are precipitation, complement fixation, agglutination, neutralisation, immune-electrophoresis, counter immunoelectrophoresis, chemiluminescence and more recent are western blotting, rapid immunochromatographic tests,

102 immunofluorescence, immunohistochemistry and immune- electron microscopy. Their techniques mechanistic and clinical implications in various diseases: AIDS, hepatitis A,B,C,E; Brucellosis, enteric fever, leptospirosis, syphilis, candidiasis, aspergillosis and auto immune diseases were discussed with sensitivity and specificity.

vi) Biomarkers and Infectious Diseases on 20th November, 2015 at AMBICON

Biomarkers are important in infectious diseases for diagnosis, prognosis, severity, progress of the disease, differentiation of bacterial vs non bacterial diseases, sepsis, multiorgan failure, response to therapy, adverse events and are a useful guide to antimicrobial therapy in Antibiotic Stewardship Program.

There are a large numbers of biomarkers currently used in infectious disease practice: CRP, PCT, IL, TNF α. Use of combination of cost effective biomarkers with serial estimations is the best approach. IL- 6, 8 are the most promising early markers. Combination of IL-6 and CRP is an excellent approach. PCT has an excellent sensitivity and specificity and is best used to rule out rather than rule in diagnosis of sepsis and is good to reduce the duration of antibiotic therapy. Point of care tests is again a new and useful approach.

vii) Antimicrobial Resistance: A public health challenge on 9th February, 2016 at VMMC, Delhi

Emergence and burden of antimicrobial resistance in the world and in India. The presentation focussed on “Way Forward”. Accuracy and quality of Microbiology services (poor quality and transport of patients specimen), lack of interaction of microbiologists and pharmacologists with clinicians were discussed in detail. Persuasive and restrictive intervention for use of antibiotics was discussed. Sustained campaigns with public and students and chemists were highlighted.

During the year 2015-16, a sum of Rs. 36,97,000/- as Life Membership from Fellows and Members has been received. Besides, an income of Rs. 30,88,511/- has been received on account of interest on the fixed deposits.

ACCOUNTS

The audit of accounts for the year 2015-16 has since been completed by the Chartered Accountant. The Council at its Meeting held on 17th September, 2016 has also approved the Statement of Accounts. These are now recommended for adoption by the General Body.

Report on the financial statements

We have audited the accompanying financial statement of NATIONAL ACADEMY OF MEDICAL SCIENCE (INDIA) (“The Academy”) which comprises the Balance Sheet as at 31st March, 2016 and the Income and Expenditure Account for the year then ended, and a summary of significant accounting policies and other explanatory information.

Management’s responsibility for the financial statements

Management is responsible for the preparation of these financial statements that give a true and fair view of the financial position, financial performance of the Academy in accordance with the Accounting Standards issued by the Institute of Chartered Accountants of India to the extend applicable to the Academy being the charitable institution. This responsibility includes the design, implementation and maintenance of internal control relevant to the preparation and presentation of the financial statements that give a true and fair view and are free from material misstatement, whether due to fraud or error.

Auditor’s responsibility

Our responsibility is to express an opinion on these financial statements based on our audit. We conducted our audit in accordance with the Standards on Auditing issued by the Institute of Chartered Accountants of India. Those Standards require that we comply with ethical requirements and plan and perform the audit to obtain reasonable assurance about whether the financial statements are free from material misstatement.

An audit involves performing procedures to obtain audit evidence about the amounts and disclosures in the financial statements. The procedures selected depend on the auditor’s judgement, including the assessment of 111 the risks of material misstatement of the financial statements, whether due to fraud or error. In making those risk assessments, the auditor considers internal control relevant to the Academy preparation and fair presentation fo the financial statements in order design audit procedures that are appropriate in the circumstances. An audit also includes evaluating the appropriateness of accounting policies used and the reasonableness of the accounting estimates made by management, as well as evaluating the overall presentation of the financial statements. We believe that the audit evidence we have obtained is sufficient and appropriate to provide a basis for our audit opinion.

Opinion

In our opinion and to the best of our information and according to the explanations given to us, the financial statements give a true and fair view in conformity with the accounting principles generally accepted in india :- a) In the case of the Balance Sheet, of the state of affairs of the Academy as at 31st March, 2015. b) In the case of the income and Expenditure account, of the excess of Income over expenditure for the year ended on that date.

A) Significant Accounting Policies 1. Accounting Convention The financial statements have been prepared under the historical cost conventionn on cash basis.

2. Revenue Recognition Revenue, donation, grants are recorded when received. Expenses are recorded when paid.

3. Fixed Assets Fixed Assets are recorded at Original Cost.

4. Depreciation No depreciation is being charged on fixed assests.

5. Grants Grants are recorded in accounts as per specific direction of donor. The government grant received for CME Programme (Plan) is in recurring in nature and the same is earmarked fund. The income earned on that fund while the amount is in bank directly added to the fund and expenses incurred reduce from that fund. The Non-Plan government grant received is in recurring in nature. The Non- Plan grant (net of capital expenditure) credit to income and expenditure account and capital expenditure amount transfer to capital expenditure amount transfer to capital assets grant fund.

B) NOTES FORMING PART OF ACCOUNTS 6. During the year, 100% of Life membership fee received from members credited to Capital Fund. 7. The corpus fund received by Academy for orations and awards has 147 been earmarked for that particular funds as per direction of the doner and kept that money separately in bank under term deposit is added to that fund and expenses incurred reduced from that fund. 8. The Academy has two buildings known as old building and new building Smt. Kamla Raheja Autitorium and J.S.B. Center for Multi- Professional Education. The Academy and National Board of Examination (NBE) are sharing the accommodation of old Building in accordance with the terms and conditions which has approved by Ministry of Health and Family Welfare, Government of India. The Academy is incurring all maintenance expenses and 50% of those expenses are recovered from NBE and reduced from maintenance expenses. 9. The construction of new building Smt. Kamla Raheja Auditorium and J.S.B. Center for Multi-Professional Education has been completed. The Academy and Smt. Kamla Raheja Foundation have incurred expenses on construction of that building. The expenses incurred by the Academy on that building has been capitalized in the books of accounts in the respective head and expenses incurred by the Smt. Kamla Raheja Foundation are pending for approval with Finance Committee and will be considered in accordance with direction and approval of Finance Committee. 10. The Physical verification of fixed assets its reconciliation with books of accounts is pending.

11. Previous year’s figures have been regrouped whereever necessary.

6. NAMS Scientific Symposium:

Every year, during the NAMS Annual Conference, a Scientific Symposium is organized on a topic of great relevance to the health care needs of the Country. The Theme of the NAMS Scientific Symposium on 22nd October, 2016 during the Annual Conference at AIIMS, Raipur (Chhattisgarh) is ‘AYUSH: Need of Integrated Medicine’.

7. Lifetime Achievement Award

The Council at its meeting held on 17th July, 2016 approved the conferment of Lifetime Achievement Award under Rule 34(d) on Dr. Snehalata Deshmukh, FAMS for the year 2016 in recognition of proven track record of professional excellence with subject expertise of high order. Being Chairperson, Finance Committee of NAMS at present, she has served the Academy in different capacities with distinction and having life-long devotion to the cause of integrity in professional practice and ethics in medical education, and her leadership role both in the professional associations in India and the National Academy of Medical Sciences, she deserves Lifetime Achievement Award.

Golden Jubilee Commemoration Award Lecture On the recommendations of the Credentials Committee and with the approval of the Council, the youngest biomedical scientists elected as a Fellows during the year is invited to deliver Golden Jubilee Commemoration Award Lecture at the Annual Conference of the Academy.

Dr. Jugal Kishore, FAMS, Professor and Head, Department of Community Medicine, Vardhman Mahavir Medical College, was the youngest biomedical scientist elected as a Fellow during the year 2015. He will deliver the Golden Jubilee Commemoration Lecture in the Annual Conference at AIIMS, Raipur, Chhattisgarh on 23rd October, 2016. The Topic of his Lecture is ‘‘Essential skills in postgraduate medical curriculum of Community Medicine’’.
