The mother received prenatal care throughout her entire pregnancy, which was complicated by preeclampsia, gestational diabetes, and premature rupture of membranes. He was stable at the time of transfer. Past Medical History: None Medications/Allergies: Ampicillin, gentamycin, and acetaminophen. No known drug allergies. Family History: No family history of skin disorders. Parents are non-consanguineous. Physical Exam: General: neonate appropriate for gestational age, with reactive normal cry. Skin: scalp and body with firm platelike skin interrupted by deep fissures. HEENT: severe ectropion, severe eclabium, and hypoplastic malformed pinna with hyperkeratotic skin filling the external auditory canals. GU: rudimentary penis and scrotum, epispadias, and undescended testicles. Extremities: hypoplastic fingers and toes with mitten-like casing and distal cyanosis, hemorrhage, and erosion. Laboratory Data: The following were abnormal or positive: Hemoglobin 7.9 g/dL [13.5-22.0 g/dL] MCV 160.1 fL [88-120 fL] Platelets 108 k/uL [200-400 k/uL] Calcium 7.1 mg/dL [8.5-10.5 mg/dL] Potassium 3.1 mEq/L [3.5-5.0 mEq/dL] The following were normal or negative: Blood cultures, white blood cell count, BUN, and creatinine. Diagnosis: Harlequin ichthyosis Treatment and Course: The patient was placed in a high humidity chamber (90-95%) in reverse isolation. Petrolatum ointment was applied to his skin every two hours under loosely applied plastic wrap. He was closely followed for fluid and electrolyte imbalances by the primary NICU team. The prophylactic antibiotics were discontinued on day 5 of life. The ophthalmology service recommended lubricating ophthalmic drops and erythromycin ophthalmic ointment for management of his ectropion, but on day 10 of life he developed mucopurulent discharge with conjunctival injection. A conjunctival culture revealed multi- resistant Staphylococcus epidermidis and Escherichia coli. The erythromycin ointment was discontinued, and a triple antibiotic ointment and gatifloxicin drops were initiated with resolution of the discharge. ______________________________________________________________________________ 6 Chicago Dermatological Society______________________________________ Case # 1 continued By day 10 of life, much of the encasement on his trunk had sloughed and transitioned to diffuse erythema with clinical improvement in his ectropion and eclabium. The mitten like casing of his fingers and toes slowly sloughed with improvement in perfusion to the digits. He failed to gain weight, and his formula feeds were increased four-fold with supplemental nasogastric tube feeds. Given the natural sloughing of the plaques, systemic retinoids were not initiated. On day 29 of life, he developed few pustules on the right cheek, which were positive for hyphae and budding yeast on KOH examination. Nystatin cream was initiated with improvement of the lesions. Discussion: Harlequin ichthyosis (HI) is a rare, severe congenital ichthyosis presenting with ectropion, eclabium, flattened ears, and large plate-like scales with deep fissures affecting the entire body. The prognosis is very poor, with few neonates surviving beyond the first few weeks of life. HI is an autosomal recessive disease due to a deletion mutation in the ABCA12 gene, which encodes an ATP binding cassette protein that transports glucosylceramides from the Golgi apparatus into lamellar bodies. A missence mutation in the ABCA12 gene results in type 2 lamellar ichthyosis. Because few to no lipids are delivered to the stratum corneum, the epidermis compensates with an intense, hyperplastic response leading to thick plate-like encasement. This defect also results in a defective skin barrier, placing the neonates at increased risk of fluid loss, electrolyte imbalances, hypothermia, and sepsis. Of the few neonates who survive, they most commonly progress to a severe congenital ichthyosiform erythroderma. The ATP binding cassettes (ABCs) are a large group of proteins that transport different substrates across cellular membranes and are divided into seven subfamilies, A-G. The ABCA transporters function as cellular lipid-transporting organelles. ABCA transporter gene mutations are associated with disorders of lipid metabolism, including Tangier disease (ABCA1) and Stargardt disease (ABCA4), as well as neonatal surfactant deficiency, which has been associated with ABCA3 deficiency. Prior to 2001, only seven reported children with HI survived beyond one year of life. All had been on a systemic retinoid (isotretinoin in one, etretinate in five, and acitretin in one) for short or intermittent periods. Systemic retinoids have been shown to help slough the thick adherent plaques within weeks, as well as improve ectropion and eclabium. Anecdotal experience suggests that systemic retinoids may help prevent ischemia and autoamputation of the digits. Peroxisome proliferators-activated receptors (PPARs) are a potential therapy on the horizon for HI. PPARs have been shown to stimulate corneocyte envelope formation, stimulate epidermal lipid synthesis, accelerate LB secretion, and improve epidermal permeability barrier homeostasis. Recently, it has been shown that PPAR-γ induces ABCA12 mRNA expression in normal cultured keratinocytes, and may improve epidermal permeability barrier homeostasis in HI patients with residual ABCA12 expression. He had completed 20 months of a two-year regimen of multidrug therapy including dapsone, rifampin and clofazamine prior to his visit. He denied any neurological symptoms. The new tender lesions were intermittent and were mainly on his legs, flanks and earlobes. Past Medical History: Borderline lepromatous leprosy Medications/Allergies: As above Family History: None Review of Systems: Intermittent fever, cough, and occasional wrist and ankle pain associated with the lesional flares Physical Exam: 07/04 Calves, left earlobe and abdomen: erythematous nodules Flanks: erythematous to violaceous reticulated patches 10/06 Trunk and extremities: slate-gray to brown patches 01/08 Earlobes: erythematous papules Thighs and legs: erythematous plaques Laboratory Data: The following were normal or negative (throughout 4 years): Complete blood count with differential, basic metabolic panel, liver enzymes

It can be a challenging differential diagnosis in dermatology practice due to several overlapping features with rheumatologic disorders. Patients with leprosy can develop reactive states as a result of immune complex-mediated inflammatory processes, leading to the appearance of additional cutaneous lesions that may further complicate the clinical picture. We describe a case of a woman presenting with a long history of a recurrent bullous rash with chronic ulcers, with an evolution of vasculitic diagnoses, who was later determined to have lepromatous leprosy with reactive erythema nodosum leprosum (ENL). Kalb, M.D. Phone: (716) 630-1102 Fax: (716) 633-6507 Department of Dermatology 325 Essjay Road Williamsville, New York 14221 ERYTHEMA NODOSUM (EN) Erythema Nodosum (EN) is a relatively uncommon reaction in the skin consisting of red shiny tender nodular lesions most commonly found on the legs. The erythema refers to the red color that is seen at the surface of the skin. The nodosum refers to the fact that these lesions feel like bumps or nodules underneath the skin surface. Erythema nodosum is a reaction pattern occurring in the subcutaneous tissue and fat. It may be preceded by a low grade fever, fatigue, and joint pains. In about half of the cases, there is an internal condition occurring in the body which contributes to its formation. These include the use of certain medications, certain infections, and a number of other less likely causes. In order to identify cause for the erythema nodosum, it may be necessary to perform blood and laboratory tests. At times, however, the cause cannot be determined and erythema nodosum is considered to be idiopathic in nature. Erythema nodosum often runs an acute course lasting for a brief period from weeks to months. In a small percentage of cases, it can be a more chronic condition. The red nodules are usually confined to the lower legs, but can develop anywhere there is fat under the skin including the thighs, arms, and trunk. Initially, the lesions are more red and inflamed in appearance, but with time become more bruise like.

Two patients with sudden onset of myelopathy associated with Systemic Lupus Erythematosus (Erythema Nodosum) are described. Pertinent literature is extensively reviewed and these two new patients are added to previously reported 26 patients. Key words: Myelopathy; Meningoencephalomyelopathy; Systemic lupus erythematosus. NEUROLOGICAL manifestations of systemic lupus erythematosus (SLE) have only recently been emphasised although they were mentioned by Kaposi in 1875, who observed stupor and coma as terminal manifestations of the disease. But focal neurological abnormalities were first reported by Osler (1903) and since then there have been several reports in the literature. Most commonly reported entities have been acute organic brain syndrome, seizures and cerebrovascular disorders. Chorea, Guillain Barre syndrome, subarachnoid haemorrhage, peripheral neuro­ pathy and cranial nerve palsies associated with SLE have also been reported on a few occasions. Myelopathy, however, has not received adequate emphasis as a complication of SLE. Fisher and Gilmour reported the first case of flaccid paraplegia in a female with SLE in 1939. Since then only 25 additional cases have been reported in the medical literature over the past 38 years. Twenty cases have been described in sufficient detail and six briefly, to permit a meaningful review of the spinal cord involvement in this disease. We feel that there are probably many more unreported cases of myelopathy associated with SLE.

    DRUG ERUPTIONS http://www.aocd.org A drug eruption is an adverse skin reaction to a drug. Many medications can cause reactions, especially antimicrobial agents, sulfa drugs, NSAIDs, chemotherapy agents, anticonvulsants, and psychotropic drugs. Drug eruptions can imitate a variety of other skin conditions and therefore should be considered in any patient taking medications or that has changed medications. The onset of drug eruptions is usually within 2 weeks of beginning a new drug or within days if it is due to re-exposure to a certain drug. Itching is the most common symptom. Drug eruptions occur in approximately 2-5% of hospitalized patients and in greater than 1% of the outpatient population. Adverse reactions to drugs are more prevalent in women, in the elderly, and in immunocompromised patients. Drug eruptions may be immunologically or non-immunologically mediated. There are 4 types of immunologically mediated reactions, with Type IV being the most common. Type I is immunoglobulin-E dependent and can result in anaphylaxis, angioedema, and urticaria. Type II is cytotoxic and can result in purpura. Type III reactions are immune complex reactions which can result in vasculitis and type IV is a delayed-type reaction which results in contact dermatitis and photoallergic reactions. This is important as different medications are associated with different types of reactions. For example, insulin is related with type I reactions whereas penicillin, cephalosporins, and sulfonamides cause type II reactions. Quinines and salicylates can cause type III reactions and topical medications such as neomycin can cause type IV reactions. The most common drugs that may potentially cause drug eruptions include amoxicillin, trimethoprim sulfamethoxazole, ampicillin, penicillin, cephalosporins, quinidine and gentamicin sulfate.

Most reports have shown that skin lesions are found in 10–15% of IBD cases, although this depends on the definition of skin lesions. The representative skin lesions in patients with IBD are erythema nodosum, pyoderma gangrenosum, Sweet’s syndrome, and so on. These lesions are often associated with IBD progression, and intestinal lesions in particular require appropriate treat- ment. Recently, another clinical issue regarding skin lesions in patients with IBD, a so-called paradoxical reaction, during the treatment with anti-tumor necrosis factor (TNF)-α agents has emerged. These reactions are termed paradoxical reactions because the skin lesions sometimes resemble psoriasis, although the anti-TNF-α agents have been historically used to treat psoriasis. Since infectious exanthema- tous diseases range from mild infections that disappear naturally to severe infectious diseases, focus on and basic knowledge of these diseases is very important. But, these include non-infectious diseases, so that comprehensive knowledge of these oth- er diseases is required. Usually, early diagnostic testing for a febrile illness with a rash is inefficient. For clinical diagnosis of diseases accompanied by skin rash and fever, a complete history must be taken, including recent travel, contact with animals, medications, and exposure to forests and other natural environments. In addition, time of onset of symptoms and the character- istics of the rash itself (morphology, location, distribution) could be helpful in the clinical diagnosis. It is also critical to under- stand the patient’s history of specific underlying diseases. However, diagnostic basic tests could be helpful in diagnosis if they are repeated and the clinical course is monitored. Generally, skin rashes are nonspecific and self-limited. Therefore, it could be clinically meaningful as a characteristic diagnostic finding in a very small subset of specific diseases. Presumably, it is a hypersensitivity reaction and may occur in association with several systemic diseases or drug therapies. Letrozole, an aromatase inhibitor, is a commonly used drug to treat hormone-sensitive breast cancer. There have been a few cases of aromatase inhibitor induced vasculitis. Presentation of Case: We report the case of a 73-years old woman with relapsing erythema nodosuma few weeks after start of treatment with Letrozole, because of hormone-sensitive breast cancer. The patient was presented with painful erythematous nodules located on the lower limbs and reactive synovitisof theankles. We excluded systemic autoimmune disease, sarcoidosis or tuberculosis; evaluated ANA-antibodies/negative, urine analysis/negative/chest X-ray/normal, TB- spot/negative. Discussion: We accepted that erythema nodosum is associated with the letrozole therapy. It was stopped and we started systemic corticosteroids. After 2 months there were no more erythema nodosum manifestations and we tapered the steroid dose. Conclusion: There are few cases of necrotizing or leucocytoclasticvasculitis induced by aromatase inhibitors. Letrozoleis often used drug for treatment of hormone-sensitive breast cancer, so with this case we would like to report this side effect of Letrozole as rare cause for erythema nodosum.
