Welcome . We want to make your transition to Fallon as smooth as possible. Here are some tips that will help you get the care you need as easy as 1-2-3. 1. Fill out your enrollment form. • List your primary care provider (PCP), your spouse’s and your children’s PCPs as well. • Make sure your PCP is in your plan network by using Fallon’s online “Find a doctor” tool. • Once you’ve picked a PCP, call Customer Service so we can enter your PCP into our system. • Or, go to myFallon.org, log into our secure member portal and click on the link to change your PCP. 2. Check out our online prescription drug list. • Also called a “formulary”, our online drug list will help you determine if your prescriptions are covered, what tier they are in, or if they require prior authorization from Fallon. • Save money by using our mail-order pharmacy program. 3. Call us if you already have a doctor’s appointment scheduled. • We’ll help make sure your doctor is in your plan network and that the medical services you need are covered under your plan. Don’t forget: Tell your providers your health insurance has changed, and give them your new insurance information. This will help you avoid billing problems. FY 2020 Hospital Emergency Department Visit Data RELEASE NOTES Background These release notes provide information for users of the FY 2020 Emergency Department Visit Data (EDD). The data includes emergency visits from Massachusetts acute care hospitals for the period of October 1, 2019 – September 30, 2020. The Center for Health Information and Analysis (CHIA) has made minimal changes to the FY 2020 data structure/ elements. Users of the data should consult the Fiscal Year 2020 Documentation Manual for further details. Data Notes North Shore Medical Center – Union Hospital Campus (Org Id 3) permanently closed on November 3, 2019. Union Hospital based services were consolidated to the North Shore Medical Center - Salem Hospital facility (Org Id 116). Emergency Department Visit data is only reported for the applicable FY20 Quarter 1 timeframe. Cambridge Health Alliance closed their Emergency Department at Somerville Hospital (Org Id 143) on April 30, 2020. Emergency Department Visit data is only reported for the first three quarters of FY20. Steward Norwood Hospital (Org Id 41) temporarily closed on June 29, 2020 due to flooding. Emergency Department Visit data is only reported for the first three quarters of FY20. FY 2020 – Provider Specific Notes Prior to releasing the FY 2020 EDD, CHIA provides the hospitals with a profile of the data submitted. Providers may resubmit data or may provide written feedback to CHIA. Below is a summary of some of the key feedback received. Signature Healthcare Brockton Hospital (Org Id 25): o Noted discrepancies with Patient Homeless Status for all quarters of FY20. Due to a mapping issue, no patients were identified as homeless. -
