An exciting opportunity has arisen for a motivated and enthusiastic Band 5 nurse with previous experience to join the Barra Nursing Team. If you would like to work on an island that’s interesting, rewarding and challenging, with lots of opportunities to develop your career, then the Barra Nursing Team has plenty to offer. This is an excellent opportunity to become a member of an integrated team, working within the hospital (5 beds), treatment room and community setting. We are working closely with Social Care, providing patient-centred care to the community of Barra and , and moving towards a new ‘Community Hub’ planned for the island. The Isle of Barra is the smallest of the islands in the archipelago of the Western Isles and is renowned for its idyllic ‘Caribbean-like’ beaches, sporting activities, sense of community and the Gaelic culture. The island has the only beach in the UK, which provides an excellent air service twice per day to the mainland, an exhilarating daily ferry service from Oban on the mainland, and four sailings per day to the adjacent island.

The ideal candidate will be a registered nurse with a current NMC registration, have evidence of Continuing Professional Development and be in receipt of a current full driving licence.

The successful applicant will be required to register with the PVG (Protecting Vulnerable Groups) Scheme. This post is not eligible for relocation expenses.

1 JOB IDENTIFICATION

JOB TITLE: Registered Nurse DEPARTMENT (S): St. Brendan’s, Barra JOB HOLDER REFERENCE: NO. OF JOB HOLDERS

2 PURPOSE

Responsible for the assessment of individual care needs and the development, implementation and evaluation of programmes of care for those patients allocated. To carry out all relevant forms of care in predominantly the hospital and where necessary community setting as directed by patient activity/need. Postholder can as workload and demand dictates, deliver care in hospital and or community.

To occasionally take charge of the ward and from time to time and the community caseload and occasionally deputise in the absence of the SCN or C/N who has continuing responsibility.

Must be willing and able to work flexibly in both hospital and community setting in response to patient caseload.

To supervise junior staff (HCA’s) and to demonstrate techniques to qualified and unqualified staff and carers, including learners.

To be the patient’s Named Nurse.

To participate in the out of hours (1600 – 0800) on call service on a rotational basis.

To act as Nurse Mentor to student nurses and Nurse Preceptor for newly qualified nursing staff.

Too be willing to participate in and undertake clinical supervision.

Identify and access best practice statements to contribute to the assessment, planning and implementation of patient care.

To work as part of an integrated nursing team and with the wider multi-disciplinary team.

To ensure that nursing practice is person centred and evidence based.

4. MAIN DUTIES AND RESPONSIBILITIES OF THE POST. KNOWLEDGE TRAINING AND EXPERIENCE.

Registered Nurse, Level 1, on the Nursing and Midwifery Council, NMC.

The post-holder will be required to continuously update his/her knowledge of nursing developments, medical techniques and treatments and is required to provide evidence of Continuing Professional Development to meet to NMC requirements in order to maintain Registration. Have the ability to provide care to a variety of care groups e.g. Minor injuries, Care of the elderly, Palliative Care, Accidents and Emergencies. A good understanding of the specific requirements of nursing people in their own homes and in care home settings. Willing to carry out Single Shared Assessments to all care groups.

ANALYTICAL AND JUDGEMENTAL SKILLS. Requires an ability to interpret, patient observations, recognise anomalies in patients conditions.

Be able to accurately carry out patient orientated risk assessments, and then to implement and evaluate specific interventions to manage identified risks e.g. using the nutrition risk score to ensure appropriate levels of nutrition ( and other risk assessment tools). Initiates appropriate investigations and treatments, including onward communication with other professionals where appropriate, in response to clinical assessment and allocated patient needs. Ability to assess, identify and plan for allocated patients’ care pathway from the point of admission to caseload discharge. Have the ability to give information to patients/carers regarding the effects and side effects of prescribed medication and to report these to staff. To carry out nurse prescribing role within limits of own training and competencies.

PLANNING AND ORGANISATIONAL SKILLS. To prioritise and manage patient caseload, with the ability to re-organise schedule in accordance to workload variations. To organise the work for clinical area when occasionallyin charge. To plan, organise and deploy staff according to competencies and workload in order to meet patient needs. Ability to collate all relevant patient information and to ensure such details are timeously transmitted e.g. on transfer to hospital or other setting or on hospital discharge. To participate in multi-disciplinary case conferences and discharge planning meetings with other professional groups/agencies, including family and carers. PHYSICAL SKILLS. The post-holder is expected to have a high degree of dexterity, hand to eye co-ordination, in order to accurately perform nursing tasks, e.g. intra-venous therapy, calculating and preparation of drug dosages (e.g. insulin) operating medical devices etc. RESPONSIBILITY FOR PATIENT/CLIENT CARE. To be the Patient’s Named Nurse and be responsible for assessing, developing, implementing, evaluating and documenting all forms of nursing care given to allocated patients. Always encourage patient participation in appropriate aspects of care and to involve the patient in the planning of their own care, e.g. empowering patients/carers. Always act as patient advocate and to act in accordance with the NMC’s Code of Professional Practice that ensures the integrity of the nurse/patient relationship and that patients are empowered to exercise informed consent. As the Named Nurse to be responsible for patient referral to appropriate clinicians, i.e. other members of local multi-disciplinary team. Gives clinical advice ( e.g. stoma care, infection control) to patient, and where appropriate to relatives/carers, which includes aspects of health promotions. The nurse will know the therapeutic uses of the medicine to be administered to patients, it’s normal side- effects, precautions and contra-indications. The nurse will be aware of the patient’s care-plan and ensure that the appropriate medical/nursing intervention is carried out, in accordance with the needs expressed by the patient and documented in patient records. COMMUNICATION AND RELATIONSHIP SKILLS Inform and support patients, and where appropriate, relatives/carers in relation to medical investigations, results and lifestyle behaviours that contribute to health. Have the ability to convey distressing and unpleasant information with empathy and sensitivity to patients and where appropriate, relatives/carers. Use tact, reassurance and sensitivity when necessary to overcome barriers to understanding, e.g. distressing situations such as death, dying and bereavement. Display a positive attitude to diverse cultures and facilitate forms of communication that embraces the philosophy of social inclusion. Participate in supporting colleagues whenever required. Attend and participate in meetings and disseminate information to staff at ward/department level.

RESPONSIBILITY FOR POLICIES/SERVICE DEVELOPMENT. Follow approved policies in own role and clinical area and comments on policies developing or under review.

RESPONSIBILITY FOR FINALCIAL AND PHYSICAL RESOURCES. Always exercise economical use of departmental resources e.g. incontinence products and other sundry items and raise awareness of the need to do this within hospital, the home or care home setting. Ensure maintenance and regular checking of equipment and reporting of faults. Ensure all equipment has been checked prior distribution to patient. To always demonstrate economic utilisation of equipment, services, resources and facilities. RESPONSIBILITY FOR HUMAN RESOURCES. Is required to advise and demonstrate practice to new members of the nursing workforce in mentoring, coaching and training students, new staff, junior staff ect. To allocate and supervise work delegated to peers and junior staff when on duty. Occasionally take charge of the nursing team in the absence of the team SCN and C/N. To always promote an environment that provides reflection, learning and growth for students, other registered nurses and healthcare assistants and to participate in ward learning programmes.

RESPONSIBILITY FOR INFORMATION AND RESOURCES. To always maintain patient records and documentation e.g. care plans, charts, investigations, test results etc. To provide activity/caseload information as required.

RESPONSIBILITY FOR RESEARCH AMD DEVELOPMENT. Participate in the data collection of clinical audits relating to good practice guidelines national standard, local audit programmes etc.

FREEDOM TO ACT. Accountable to NMC for own professional practice. Able to act within established parameters, e.g. acknowledgement of personal competencies, extended scope of practice which are clearly defined by the NMC’s Code of Professional Conduct and the Western Isles NHS Board’s operational policies and protocols. Manages the prioritisation and delivery of care and associated decisions related to allocated patients. SUPPORTING EVIDENCE. PHYSICAL DEMANDS OF THE JOB. The ability to use mechanical hoists and other equipment safely and effectively in the movement of patients in hospital and other care settings during every working day. There will be occasions where the post-holder will have to manoeuvre patients and equipment in confined areas within the home setting, e.g. to transfer from one room to another. There will be occasions when the post-holders will have heavy patients and equipment without the recourse to hoist during emergency situations, e.g. cardiac arrest; patient falls where the environment prohibits the use of hoists.

MENTAL EFFORT. The post-holder will be required, during every working day, to prioritise caseload and work demands in the face of:- 1. Unpredictable work patterns. 2. Changing patient dependency levels 3. Interruptions such as telephone enquiries, demands of other patients and relatives/carers, demands from other staff such as doctors, peers etc. 4. Occasional emergency situations such as responding to A&E presentations.

Periods of concentration are often required to facilitate the physical examination of patients and the carrying out of patient assessments using verbal and non-verbal language. Concentration may be particularly challenging when the patient is distressed and or is experiencing difficulties in co-

operating with instructions and having difficulties communicating, e.g. post stroke, head injury, toxic confusional states associated with endocrine disorders, infection etc.

Concentrating is also required to:-  Carry out patient observations (e.g. wound care assessment, assessments of levels of consciousness etc.) in stressful environments e.g. during staff shortages, busy workloads etc.  Observe unpredictable patient behaviours that may present risks such as potential of falls.  Manage the unpredictability associated with people (patients/members of the public) who are exhibiting behavioural difficulties and who are unknown to staff.  Carrying out intricate and or personal interventions e.g. catheterisation, wound care.  Nursing non compliant/concordant patients.

WORKING CONDITIONS. Daily exposure to changing atmospheric conditions e.g. moving between homes with differing temperatures and travelling in inclement weather conditions. Daily contact and exposure to body fluids such as faecal matter, urine, blood, sputum, soiled linen, infective material etc. Organising and carrying out decontamination of infected areas and equipment. The post-holder may be expected to manage occasional exposure to aggressive and abusive patients, relatives/carers, members of the public and members of staff. Driving on various road conditions. The Western Isles Health Board employs over one thousand members of staff, excluding GPs and Dentists. There are three main hospitals situated in the Western Isles:

Transport Information
