A palliative and end-of-life model of care for Illawarra Shoalhaven November 2017 The South Eastern NSW Primary Health Network (COORDINARE) seeks to increase the efficiency and effectiveness of medical services for patients across the Illawarra Shoalhaven and Southern NSW region, particularly those at risk of poor health outcomes. COORDINARE works directly with General Practitioners (GPs), primary and secondary health care providers and hospitals to improve and better coordinate care for patients across the network.
