FLYING LESSONS: CREW RESOURCE MANAGEMENT IN HEALTHCARE Authored by: Colin Konschak, FACHE, CEO Blake Smith, Principal www.divurgent.com © Divurgent 2017-2018 INTRODUCTION Patient safety has come under closer scrutiny in recent years and experts agree that human factors are major contributors to adverse events in healthcare. In its1999 landmark report, “To Err is Human: Building a Safer Health Care System,” the Institute of Medicine (IOM) stated that anywhere from 44,000 to 98,000 patients die each year as a result of medical errors at an estimated annual cost of $29 billion.i Medical errors, two-thirds of which are preventable, are the eighth leading cause of death in the U.S. each year.ii As medicine relies on increasingly complex technologies, this problem will only intensify. For example, The New York Times recently conducted a study that found an alarming number of injuries related to radiation therapy. The injuries, which should have been avoidable, were caused by faulty programming, poor safety procedures, software flaws that should have been recognized, and inadequate staffing and training.iii Like medicine, aviation relies on teams of experts working together to carry out complex procedures that, if not executed properly, can have serious consequences. If 98,000 people died in plane crashes each year, it would be the equivalent of a fatal jet crash – 268 passengers – every day. Airlines, however, have taken steps to prevent such catastrophes and health care providers are taking note. Democratic Consolidation and Capital Flight in Latin America
ABSTRACT Title of Dissertation: FLIGHT OR FLIGHT? DEMOCRATIC CONSOLIDATION AND CAPITAL FLIGHT IN LATIN AMERICA Daniel Scott Owens, Doctor of Philosophy, 2017 Dissertation directed by: Dr. Virginia Haufler, Department of Government and Politics Since 1980, developing countries lost US$16.3 trillion dollars as a result of capital flight (Kar 2016) representing a major threat to international development efforts. This dissertation investigates why some democracies in the developing world experience much more capital flight than others. Using the experiences of Latin America democracies, the fundamental reasons for flight lie in the failure of these countries to consolidate their democracies. As a result of their failure to consolidate, they are highly vulnerable to popular mobilization by excluded groups demanding redistribution, which has the effect of increasing perceptions of political risk among asset holders and incurring flight. In an area of the world where wealth, income, and power is chronically unequal, my central argument posits a causal sequence that begins with mass mobilization by social movements directed towards new redistributive public policies and in opposition to pro-market democratically elected governments. Typically, as mass mobilization strengthens, Leftist parties embrace the aims of popular movements whose electoral support subsequently increases to levels that allow them to form governments committed to redistribution. Under these conditions, as mobilization and support for the Left strengthened, asset holders’ perceptions of risk increase significantly, leading to capital flight. Using a mixed methods research design combining quantitative analysis with qualitative case studies I present empirical evidence to support my argument. For the quantitative analysis, regression analysis was applied to a cross-sectional time series dataset for 18 democracies. -

Acknowledgements February 2015 In 2012, Washburn Center for Children, Kente Circle, and Centro collaborated on a grant proposal to obtain funding from the Hennepin County Children’s Mental Health Collaborative to help the agencies improve cultural competence in services to various client populations, including Spanish-speaking families. These funds allowed Washburn Center’s existing Spanish-speaking Provider Group to build connections with over 60 bilingual, culturally responsive mental health providers from numerous Twin Cities mental health agencies and private practices. This expanded group, called the Hennepin County Spanish-speaking Provider Consortium, meets six times a year for population-specific trainings, clinical and language peer consultation, and resource sharing. Under the grant, Washburn Center’s Spanish-speaking Provider Group agreed to compile a clinical language guide, meant to capture and expand on our group’s “¿Cómo se dice…?” conversations. -
