Combined gallium and bone scan studies has sensitivity of 81-90% and specificity of 69-100% 2.4 White blood cells (WBC) scan 2.4.1 This is sensitive and specific for bone infection and particularly useful in violated bone. -

On direct questioning, she reported that she had suVered from back pain for four years. The thoracolumbar radiograph four years earlier showed T6/7 vertebral collapse, mild scoliosis, and degenerative change of the lumbar spine (fig 1); but other investigations at that time including the eryth- rocyte sedimentation rate (ESR) and protein electophoresis were normal. Bone mineral density then was 0.914 g/cm2 (T score = −2.4) at the lumbar spine, 0.776 g/cm2 (T score = −1.8) at the right femoral neck and 0.738 g/cm2 (T score = −1.7) at the left femoral neck. She was given cyclical etidronate after this vertebral collapse as she had suVered a previous fragility fracture of the left wrist. On admission, she was afebrile, but general examination was remarkable for pallor, dental http://ard.bmj.com/ caries, and cellulitis of the left leg. A pansysto- lic murmur was heard at the cardiac apex on auscultation; there were no other signs of bac- terial endocarditis. Physicians involved in the care of osteomyelitis patients. Introduction . Osteomyelitis is inflammation of the bone marrow which results from infection. It can progress to osteonecrosis, bone destruction, and septic arthritis. Osteomyelitis is an important cause of permanent disability worldwide. Bimodal age distribution with incidence peaking in children under 5-years-old and adults over 50-years-old. Introduction . Etiology: . Hematogenous spread . Predominant etiology of infection in children. Less common in adults, however usually leads to vertebral osteomyelitis when it occurs. Contiguous spread . Infection originating from soft tissues and joints. Usually coincides with vascular insufficiency, such as patients with diabetes mellitus or peripheral vascular disease. Most commonly the lower extremities. Direct inoculation . Direct seeding of organism into the bone usually from open fractures, surgery, or puncture wounds. Introduction . Imaging diagnosis . Radiography . Low sensitivity and specificity for detecting acute osteomyelitis. Approximately 80% of patients within 1-2 weeks of infection have normal radiograph. Bone marrow edema is the earliest pathological feature and NOT well visualized on radiography. Useful as first-line imaging to exclude other differentials (i.e. fracture) and to assess progression from priors. Nuclear Medicine . Sensitivity of a 3-phase bisphosphonate-linked technetium bone scan is greater than that of radiography for early osteomyelitis. Diagnostic sensitivity of approximately 50-85%. -

ONDRA, M.D. Department of Neurological Surgery, The Feinberg School of Medicine and McGaw Medical Center, Northwestern University, Chicago, Illinois Pyogenic vertebral discitis and osteomyelitis (PVDO) has become an increasing problem for the spine surgeon. Despite recent advances in medical care and improved diagnostic neuroimaging, PVDO remains a major cause of ill- ness and death in the elderly population. Infection of the spinal column often presents insidiously; however, if not treat- ed appropriately and in a timely manner it can lead to severe neurological impairment, systemic septicemia, and pro- gressive spinal deformity. In this paper the authors review the epidemiological and pathophysiological features and the clinical presentation of PVDO. Conventional medical therapy is described, with a particular focus on the methods of diagnosis. Surgical strategies for PVDO are then presented based on the literature and according to the practice of the senior author (S.L.O.), with an emphasis placed on structural considerations, implant selection, and techniques for aug- menting vascular tissue to the site of infection. Reavill, DVM, Dipl ABVP (Avian and Reptile & Amphibian) Dipl ACVP Zoo/Exotic Pathology Service, 2825 KOVR Drive, West Sacramento, CA 95605 USA ABSTRACT This paper will cover the diseases of the musculoskeletal system of reptiles. It is not a comprehensive review. More in-depth disease descriptions can be found in the articles and reference books listed in the literature cited. The paper is divided by etiologic disease categories involving the musculoskeletal system. A short discussion of the disease conditions is found in each section. For the purposes of this paper, chelonian will refer to all of the shelled reptiles. The term “tortoise” will be used to describe the large land animals that have stout elephantine feet and “turtle” for those from an aquatic environment. Skeletal Muscle Congenital Muscular dystrophy is described in variety of lizards and snakes. The lesion is characterized by irregular atrophy, with myofibers lost and replaced by fat. In a series of evaluations, no sex bias or species-specific findings were noted.44 Non-Inflammatory Nutritional Myopathy Muscular degeneration and necrosis are relatively common conditions in reptiles. This most frequently occurs from catabolic states prior to death. Subacute disease with decreased food consumption will result in a loss of heart fat, atrophic coelomic fat pads, followed by muscle wasting. Reptiles with chronic disease often have severe loss of body condition characterized by reduced abaxial muscle mass in snakes, girth of the tail in lizards, and sunken eyes in all reptiles.30 Cachectic myopathy in marine turtles has been associated with chronic bacterial and parasitic infections. -

CLASSIFICATION Osteomyelitis may be divided into two major categories based upon the pathogenesis of infection: (1) hematogenous osteomyelitis and (2) nonhematogenous osteomyelitis, which develops adjacent to a contiguous focus of infection or via direct inoculation of infection into the bone [1-3].
