SHOEBOX​® ​ Calibration Service Instructions

Your calibration package will include​: ✓ Shipping with ✓ SHOEBOX​® ​ calibration service instructions ✓ Calibrated transducers and cables ✓ Calibrated microphone (if applicable) ✓ with calibration certificate ✓ Updating SHOEBOX Transducers Guide (and/or Microphone Guide)

Packing your expired items 1. Remove the contents of the . Set aside the s​ hipping carton​, and b​ ubble wrap​. 2. Remove the ​new set of calibrated transducers​, your n​ ewly calibrated microphone (if applicable), ​ and the ​calibration certificate envelope, ​ and set these aside. 3. Take your old transducers and wrap the cable in a small bundle, and place the cable bundle between the ear cushions. 4. Take the clear plastic or bubble wrap your new hardware came in, and use them to seal your old transducers (seal your microphone as well, if applicable). 5. Place the bag or bubble wrap containing the expired items into the shipping carton. 6. Fold the bubble wrap over the bag and close the box.

IMPORTANT​: If your old transducers may be contaminated with potentially infectious agents, please alert us at [email protected]​​ for special instructions

Shipping 1. Refer to the shipping guides we emailed to you for detailed instructions on how to return ship your items to us via either FedEx or UPS. 2. Once completed, email ​[email protected]​ with the package’s tracking number to confirm you have sent the package to us.

To set up your newly calibrated SHOEBOX system: 1. Remove the calibration certificate from the envelope labeled “Calibration Certificate” and file your new calibration certificate for future reference. 2. Follow the “Updating SHOEBOX Transducers Guide” (and the “Updating SHOEBOX Microphone Guide” if applicable) we provided to install your new hardware. Note:​ Please do not remove the QR code from the transducer cable. Your transducers cannot be identified without it.

If you are using the commercial invoice we have provided for you, use this checklist to ensure that your commercial invoice is filled out correctly.

❏ Exporter Address​: Ensure the address you are shipping from is entered here ❏ Ship Date:​ Enter the full date of the day you plan on shipping the item ❏ Air Waybill Number/Tracking number:​ Enter the tracking number on the that is being shipped ❏ Fedex: 12-digit code (numbers only) ❏ UPS: 18-digit code (numbers and letters) ❏ Quantity:​ Enter the amount of items you are shipping ❏ Total Quantity​: Enter the total amount of all the items you’re shipping, and the number of ❏ Weight:​ List the weight of each item (0.9 lbs / 0.4 kg per transducer, 0.22 lbs / 0.1kg per microphone, 1lb / 0.454 kg per iPad) ❏ Country of manufacture:​ Ensure each item’s country of manufacture is entered (For reference, each item’s country of manufacture is listed below) ❏ Unit Value/Total Value​: Ensure the value of every item is listed at $100 USD ❏ Signature:​ Confirm that all 3 copies of the commercial invoices have been signed ❏ Date​: Make sure you include today’s date next to your signature at the bottom of the page
