The aim of the guide is to help identify community services available beyond leaving hospital. Community stroke services are varied and access to them is dependent on which area of Nottinghamshire you live in and in which Clinical Commissioning Group your General Practitioner/ Doctor is situated. The East Midlands Academic Health Science Network has produced this guide for the benefit of stroke survivors, families, friends and carers living in South Nottinghamshire (Rushcliffe, Nottingham North & East and Nottingham West). 2 WHY I MIGHT NEED COMMUNITY SERVICES AFTER LEAVING HOSPITAL There is research evidence to show that stroke survivors benefit from stroke specialist rehabilitation delivered by a stroke specialist multidisciplinary team. This is particularly true for ‘Early Supported Discharge’ (ESD) services. ESD services facilitate early transfer of care from hospital to home and provide intensive rehabilitation in the home environment, to aid better and faster recovery. There is also evidence to show that extending community stroke specialist rehabilitation beyond the length of an ESD service, for stroke survivors who need it, has proved beneficial. 3 STROKE PATHWAY ILLUSTRATION This diagram provides an overview of a ‘typical’ stroke care pathway. The term ‘pathway’ refers to a system of stroke services as recommended by research evidence. A ‘typical’ journey experienced by a stroke survivor starts with admission to a hospital (referred to as acute stroke services) then continues through community based rehabilitation and social care services. These services are provided by Clinical Commissioning Groups or local authorities and are delivered by a variety of service providers. The diagram was drawn by Ossie Newell who is a stroke survivor and was created by the East Midlands Academic Health Science Network Stroke Programme. 4 EXAMPLES OF COMMON STROKE CARE PATHWAYS The pathway of care experienced by each stroke survivor will be different, based on their particular needs and what services are available in the area they live. The flowcharts below give examples of some common pathways – these are also ones that research evidence says are most effective. If further rehabilitation is required, you may spend some time on a rehabilitation ward. Before being discharged from hospital, you should be provided with information about any community services you have been referred to and therefore should expect to receive. This guide is designed to help with this process. An Early Supported Discharge service is delivered by the Stroke Outreach Service located at Nottingham University Hospital NHS Trust on Berman Ward (Tel: 0115 969 1169). Following this a Community Stroke Rehabilitation service is provided by the Community Stroke Service located at Stapleford Care Centre, Church Street, Stapleford, Nottingham, NG9 8DB (Tel: 0115 883 5060). Both services provide specialist rehabilitation in your own home. 10 A Complex Stroke Outreach Service is also available depending on individual patient needs and for those who do not meet the criteria required to access the Early Supported Discharge or Community Stroke Rehabilitation services, but who still have outstanding rehabilitation goals. The team delivers therapy at home or in your place of residence. This service is provided by Nottingham University Hospitals NHS Trust, Rehabilitation Unit, City Hospital, Nottingham, NG5 1PB (Tel: 0115 9691169 ext 56922). In order to be eligible to receive these services you must be registered with a South Nottinghamshire GP and meet certain requirements specified by the services. FLOW CHART OF THE PATIENT JOURNEY FOR STROKE SURVIVORS LIVING IN THE SOUTH NOTTINGHAMSHIRE Hospital Acute Ward Hospital Complex Stroke Rehabilitation Wards Outreach Service Stroke Outreach Service Community Stroke Rehabilitation Service 11 Additional services that are available: Speech and Language therapy is provided by Nottinghamshire Healthcare Trust, County Health Partnerships, Referral Admin Team, Stapleford Care Centre, Church Street, Stapleford, Nottinghamshire, NG9 8DB (Tel: 0115 883 5187). Stroke patients may be referred to this service depending on individual needs. An outpatient rehabilitation unit for complex fallers, neurological patients (including stroke) and elderly rehabilitation is provided by Nottingham University Hospitals NHS Trust, 1st Floor South Corridor, City Hospital, Nottingham NG5 1PB. Stroke patients may be referred to this service de- pending on individual needs.     Landowner Declaration Register This is maintained under Section 31A of the Highways Act 1980 and Section 15B(1) of the Commons Act 2006.   • Case Studies of Schemes We Have Designed in the East Midlands
    Case studies of schemes we have designed in the East Midlands. Ashby Road, Coalville. We worked closely with North West Leicestershire’s Urban Designers to develop 37 homes following the principles of ‘Building for Life’. The scheme has produced a layout with courtyards and terraces that is in keeping with the traditional homes in Coalville, but re-interpreted in a contemporary way. Daybrook Street, Sherwood, Nottingham. A small infill scheme set within a 3 storey, dense, urban environment. We designed 9 houses and 3 flats to form a mews courtyard. The developer Waters Homes were very pleased with the results which sold off plan. Heather, Leicestershire. This scheme of 11 bungalows and houses has been designed to reflect the traditional farmyard and terraced cottages that surround the area. It was rated A+ under North West Leicestershire’s ‘Ourplace’ scheme that rates design quality of new homes and neighbourhoods. Duffield Court, Bestwood, Nottingham. This derelict site in the heart of an existing community was tendered in a competition by Nottingham City Council, with NCHA and Pelham Architects winning with their proposal for a striking crescent form to the houses. The site is steeply sloping which we addressed with split level house designs. Squires Avenue, Bulwell, Nottingham. Set within a preserved wooded area with ancient oaks, we worked with the local tree officer to limit tree felling to the minimum. A palette of bricks and cladding was chosen to help bring out the beauty of the existing trees Squires Avenue, Bulwell, Nottingham. Perry Road, Nottingham.
