We are a fully accredited, mixed billing medical practice located in Willetton, Perth, providing general medical services to the local community. Herald Avenue Family Practice was established in November 2000 in purpose built premises at 7 Herald Avenue, Willetton. The practice has onsite parking and is within easy access to nearby pharmacy, phlebotomy, imaging and dental services. We are fully accredited with AGPAL (Australian General Practice Accreditation Limited), who oversee our commitment to safe and quality health care through compliance with standards set by the RACGP (Royal Australian College of General Practitioners). Minor Surgery: Repair of lacerations, removal of skin lesions, corneal foreign bodies and management of uncomplicated fractures. Our instruments undergo full cleaning and sterilisation procedures in accordance with Australian Standards. Work Related and Motor Vehicle Accidents: Management of injuries, liaison with rehabilitation providers and employers. Women’s Health Nurse: Breast checks, pap smears and sexual health. Appointment Information Standard consultations average 10-15 minutes. If your appointment is likely to take longer than usual, please advise reception staff so we can allocate adequate time for a longer appointment with your doctor. Whilst we endeavour to avoid running late, please note that delays may sometimes be unavoidable due to emergencies, interruptions and complex medical problems. We thank you for your understanding. To decrease waiting, we advise patients not to arrive prior to their appointment time, unless you are a new patient who require registration to our practice. If you are unable to attend your appointment, we appreciate your courtesy in contacting the practice as soon as possible to cancel your appointment. Unfortunately, patients who repeatedly fail to attend appointments without giving reasonable notice of cancellation will incur a non-attendance fee. Fees Herald Avenue Family Practice is a mixed billing clinic. Please note that fees charged and the availability of bulk billing are at the discretion of the treating doctor. Please call and our reception staff will be able to assist you. Our current private fee schedule (for payments provided on the day) • Level B Consultation $75 (standard consult) We accept cash and most credit cards. • Level C Consultation $125 (long consult) EFTPOS facilities are available. • Emergency level B Consultation $95 (fitted-in) The treating doctor determines the consultation based on the complexity of your consultation, whether extra services have been provided, and the specific circumstances of your presentation. Rebates are available through Medicare for most services. Consultations not eligible for a Medicare rebate, such as insurance, travel, diving and employment medicals also attract a GST of 10%. Workers Compensation and Motor Vehicle Claims It is a condition of the practice that all fees and charges associated with Work Cover and Motor Vehicle consultations and treatments are the responsibility of the patient and are paid upfront on the day. Fees are at the discretion of the doctor but in general such attendances are charged at applicable AMA rates. A receipt for services rendered will be provided. It is then your responsibility to liaise with your insurance provider for any reimbursement of costs that may be due to you. Please note that you may be out of pocket a gap fee as the rebate you receive may be less than that charged by the doctor, or if your insurance claim is rejected you will not be entitled to any insurance rebate (but you may be able to submit a claim to Medicare instead). Pathology Collection Blood collection services are available on site through SJOG Pathology. St John of God Pathology operates privately and independently. Any pathology enquiries including account enquiries need to be addressed with the St John of God collection staff member on duty. Operating hours are 8.00am-1.30pm and 2.00pm-4.00pm Monday to Friday. Page | 2 Herald Avenue Family Practice Results Please be advised that it is the responsibility of the patient to follow-up on the results of any tests ordered by the doctor. As a general rule patients are advised to make a follow-up appointment to discuss test results with their doctor unless otherwise advised, or if unsure to ring the practice for further instruction. Usually allow 3 days for the results to be checked by the doctor (longer if over the weekend). The only information given by phone regarding tests will be a relayed comment from the requesting doctor. Reception staff are NOT permitted to provide figures or interpretations of results (except for INR results as arranged by the doctor). If you wish to receive copies of test results please discuss with your doctor during your consultation and he or she can arrange for the copy to be sent directly to you when completing the request form. Copies are only provided at this time or during the follow-up consult with your doctor. Whilst the practice has follow-up systems in place, these are not always foolproof, therefore your assistance and vigilance in following up results will minimise the risk of adverse events in your health care. Telephone Calls Our reception staff can assist with most enquiries. Please understand they will need to ask you for information to ascertain the nature of the call and assess the urgency of the matter. Emergency medical matters will be referred to an available medical staff member immediately. Our doctors prefer not to be interrupted during consultations to take calls, as this can be intrusive for all concerned. To avoid this, calls may be placed on hold or patients advised to ring later when staff have been able to seek advice between consultations. It is important to understand that it is preferable to schedule an appointment to discuss medical problems with the doctor in person so that they can provide you with the best possible care. Referrals to Specialists Prior to arranging an appointment with a specialist, please ensure you have a current referral. This will normally require a consultation with the referring doctor who will choose the specialist they feel is the most appropriate for your medical condition and locality. Due to legal requirements, referrals cannot be backdated. A fee applies for repeat referrals supplied without consultation, or for lost referrals. Please advise the doctor if you later decide that you do not wish to see the specialist as advised. Repeat Prescriptions For your safety, a consultation is usually required for issuing of repeat prescriptions. Prescriptions issued without consultation will typically incur a fee. The fee charged is not rebatable through Medicare. Medical staff would appreciate 24 hours notice for the provision of repeat prescriptions. Privacy & Confidentiality This practice has developed a privacy policy that complies with the privacy legislation and the National Privacy Principles (NPPs). Please ask our staff if you wish to see a copy of our privacy policy. To assist us in maintaining accurate records please notify staff of any changes to your details as soon as possible. If any information we hold is thought to be incorrect please draw our attention to the error and we will amend accordingly. Your medical record is a confidential document. It is practice policy to maintain security of personal health information at all times and to ensure that this is available only to authorised members of staff.
