Stool consistency: • There appears to be consistent separation between the 3mg treatment and placebo arm CDF curves at the meaningful threshold intersection with the x-axis (stool consistency endpoint change score) in both Studies 00 and 03. • Study 00 showed about a 26% separation between 3mg and placebo at the 1.4 stool consistency threshold (2-point improvement in PGA constipation severity anchor from Study 10). • Study 03 showed about a 24% separation between 3mg and placebo at the 1.4 stool consistency threshold (2-point improvement in PGA constipation severity anchor from Study 10). With regard to straining, the fourth pre-specified secondary endpoint, this reviewer has concluded the following (see Appendices E-H [CDF plots]): Straining: • It is important to note that the applicant did not conduct cognitive interviews with CIC patients to support the 5-point straining item and response options that were used in the two phase 3 studies. However, the same five response options used in the 5-point straining scale have been used in other therapeutic areas and cognitively tested with patients concluding that the response options are generally well-understood and meaningful to patients, with the exception of the “very severe” response option, which some patients believe is redundant and not meaningfully different from “severe.” 6 Reference ID: 4017868 Clinical Outcome Assessment Review Sarrit M. See Section 4 (Content Validity) for how patients, who reported experiencing severe or very severe straining, defined severe straining. • The 5-point straining scale was not included in the end of phase 2 Study 10 (an 11-point numeric rating scale [NRS] for straining was included, rather than the 5-point straining scale that was used in the phase 3 trials, Studies 00 and 03); therefore, the Study 10 anchor scales could not be used to establish a responder definition. o However, based on examining cross-validation anchor thresholds (obtained from one phase 3 study and applied to the other), it appears that the 2-point improvement in PGA constipation severity anchor is meaningful to patients and corresponded with about a 1-point improvement in straining score (i.e., matching up with a -1.1 point straining score change in Study 00 and a -0.9 point straining score change in Study 03 at the median score on the PGA constipation severity anchor scale): o The improvement threshold of a 1-point improvement in straining score showed a separation between 3mg treatment and placebo arms of about 15% in Study 00 and 13% in Study 03. • Based on qualitative research with patients in other disease areas, a one-point improvement from “very severe” to “severe” may not be a meaningful improvement for patients. That said, there were approximately 10% of patients in both studies in both the 3mg and placebo arms who reported that they were experiencing “very severe” straining at baseline. These discoveries have also been instrumental in increasing our understanding of the basic mechanisms that control the electrolyte and water balance in the gut, kidney, and urinary tracts under normal conditions and in disease. Herein, we review the evolution of genes of the guanylin family and STa genes from bacteria to fish and mammals. We also describe new developments and perspectives regarding these novel bacterial compounds and peptide hormones that act in electrolyte and water balance. The available data point toward new therapeutic perspectives for pathological features such as functional gastrointestinal disorders associated with constipation, colorectal cancer, cystic fibrosis, asthma, hypertension, gastrointestinal barrier function damage associated with enteropathy, enteric infection, malnutrition, satiety, food preferences, obesity, metabolic syndrome, and effects on behavior and brain disorders such as attention deficit, hyperactivity disorder, and schizophrenia. Research Article Intestinal enteroendocrine cells are critical to central regulation of caloric consumption, since they activate hypothalamic circuits that decrease appetite and thereby restrict meal size by secreting hormones in response to nutrients in the gut. Although guanylyl cyclase and downstream cGMP are essential regulators of centrally regulated feeding behavior in invertebrates, the role of this primordial signaling mechanism in mammalian appetite regulation has eluded definition. In intestinal epithelial cells, guanylyl cyclase 2C (GUCY2C) is a transmembrane receptor that makes cGMP in response to the paracrine hormones guanylin and uroguanylin, which regulate epithelial cell dynamics along the crypt-villus axis. Here, we show that silencing of GUCY2C in mice disrupts satiation, resulting in hyperphagia and subsequent obesity and metabolic syndrome. This defined an appetite-regulating uroguanylin-GUCY2C endocrine axis, which we confirmed by showing that nutrient intake induces intestinal prouroguanylin secretion into the circulation. The prohormone signal is selectively decoded in the hypothalamus by proteolytic liberation of uroguanylin, inducing GUCY2C signaling and consequent activation of downstream anorexigenic pathways. Thus, evolutionary diversification of primitive guanylyl cyclase signaling pathways allows GUCY2C to coordinate endocrine regulation of central food acquisition pathways with paracrine control of intestinal homeostasis. Executive Summary Irritable bowel syndrome (IBS) is a chronic functional gastrointestinal disorder characterised by intestinal pain or discomfort together with alteration of bowel habit, abdominal distension, bloating, constipation or diarrhoea1. Symptoms usually wax and wane for many years, often resulting in reduced quality of life and work productivity. The pathophysiology of IBS is incompletely understood. Despite affecting 5 to 20% of the Western population, no medicines are authorised in the European Union (EU) specifically for the treatment of IBS. Patients and prescribers are limited to general symptomatic treatments such as laxatives, antidiarrhoeals and antispasmodics, which are recommended in current guidelines but on the basis of weak evidence. Alternatively, they may use unapproved treatments such as antidepressants and non-absorbable antibiotics when lifestyle modifications such as reducing stress, altering diet or psychological interventions prove ineffective. In September 2012, the European Medicines Agency’s Committee for Medicinal Products for Human Use (CHMP) recommended the authorisation of Constella (linaclotide) to offer a new option for adults with moderate to severe IBS with constipation (IBS-C), a common subtype of the disease.

Here, we tested CNS administration of two GC-C agonists and found no significant reduction of food intake. We also carefully phenotyped mice lacking the GC-C receptor and found them to have normal body weight, adiposity and glucose tolerance. Interestingly, uroguanylin knockout mice had a small but significant increase in body weight and adiposity that was accompanied by glucose intolerance. Our data indicate that the modest effects of uroguanylin on energy and glucose homeostasis are not centrally mediated by central GC-C receptors.

Uroguanylin (UGN) is one such cGMP regulating peptide, but its effect on RAAS has not been studied. On the basis of background information from other cGMP regulating factors like atrial natriuretic peptide (ANP), as well as the phenotype of UGN knockout mice, we formulated the hypothesis that UGN negatively regulates renin expression and/or secretion. The microarray results were consistent with our hypothesis that UGN inhibits renin mRNA expression but we could not validate this by Real-Time PCR. A possible reason behind this discrepancy could be the remarkable effect of psychosocial stress on renin expression, a parameter not considered during our microarray experiment. Future studies could be designed to explore the possible negative role of UGN on renin secretion.
