直付医院列表 Direct Billing Providers List PLEASE MAKE APPOINTMENTS 直付指南 Direct Billing Guidelines 1. 请带上您的保险卡及含有照片的有效身份证明。不能使用复印件。 Please bring your insurance card along with a valid photo ID. Photocopies will not be accepted. 2. 处方药必需有本人领取。家属/亲戚/朋友不可以代表您去取处方药物。 Prescription drugs must be picked up in person. Dependents/Relatives/Friends may not pick up your prescription drugs on your behalf. 3. 务必记得在您离开医院之前确认就诊金额并在理赔表上签字。 Please remember to sign the claim form to confirm expenses before leaving the hospital. 如果您在直付医院就诊的过程中遇到了任何麻烦，请与我们的客服中心联系：021‐6187‐0330. Direct billing providers merely provide a convenient service for members by sending bills to us for direct settlement. It is not their responsibility to determine whether or not certain treatments are covered under your health plan. If you have questions about coverage/benefits, please call MSH China at 021‐6187‐0330. In some cases, a direct billing claim may not be fully covered, due to reasons such as: the treatment is excluded under your policy, or you have exceeded the maximum benefit for the policy year. In such a case, our Collections Department will contact you to ask that you reimburse us for the expenses if we have already paid the hospital on your behalf. Please refer to the Policy for your exact geographic coverage and insurance benefit. FRED™ Flow Re-direction Endoluminal Device The Flow Re-Direction Endoluminal Device (FRED) System is indicated for use in the internal carotid artery from the petrous segment to the terminus for the endovascular treatment of adult patients (22 years of age or older) with wide-necked (neck width ≥ 4 mm or dome-to-neck ratio < 2) saccular or fusiform intracranial aneurysms arising from a parent vessel with a diameter ≥ 2.0 mm and ≤ 5.0 mm. Use of the FRED System is contraindicated under these circumstances: Patients in whom anticoagulant, anti-platelet therapy, or thrombolytic drugs are contraindicated. Patients with known hypersensitivity to metal such as nickel-titanium and metal jewelry. Patients with anatomy that does not permit passage or deployment of the FRED System. Patients with an active bacterial infection. Patients with a pre-existing stent in place at the target aneurysm. Patients in whom the parent vessel size does not fall within the indicated range. Patients who have not received dual anti-platelet agents prior to the procedure. For complete indications, contraindications, potential complications, warnings, precautions, and instructions, see instructions for use (IFU provided in the device). HEADWAY Microcatheter Indications for Use: The HEADWAY Microcatheter is intended for general intravascular use, including the peripheral, coronary and neurovasculature for the infusion of diagnostic agents, such as contrast media, and therapeutic agents, such as occlusion coils.
