Arborway Corp. hereby certifies the waste shall be in conformance with analytical data and other specifications provided to Franklin Pumping Service, Inc., prior to pick-up.

To the extent the waste does not conform, Arborway Corp. shall indemnify and hold Franklin Pumping Service, Inc., harmless from all liability and damages arising therefrom and Franklin Pumping Service, Inc., shall be released from all its obligations under this agreement.

Arborway Corp. is solely responsible for the appropriate DOT and EPA markings, labels, and construction specifications of waste containers. U I Arborway Corp. Quote #1472 Continued I-'Under Massachusetts "Right to Know" legislation, Franklin Pumping Service, Inc., requests that all generators supply Material Safety Data S Sheets for any aforementioned waste before any waste will be transported. Terms: Net 15 days from invoice date, with a 1.5% finance charge per month on overdue invoices.

In conjunction with the repiping work at the bulk terminal, an 8" by 8" 5/16" plate was welded over the damaged spot on the floor of tank 104, which was noted in the ultrasonic test report done on the tank after it was cleaned.

The scope of the inspection was to determine through visual I and instrument inspection the integrity of the coating applied to the plates. In addition, areas found to be cracked, delaminated or I containing holidays were marked for reference or repair, if required.

II. TEST PROCEDURES

A Tinker & Rasor Model 11/1 Holiday Sponge Test Instrument was

used to inspect the coating. When connected to the steel plates,

the instrument generates a potential difference between the surface

of the coating and the steel underlayment.

Holidays, cracks or other defects in the integrity of the

coating will cause of D.C. current discharge when the sponge wand

of the instrument is brought in contact. The defect area is then

marked for visual inspection.

A print of the tank bottom using manways and product piping,

as reference, shows those areas where defect signals were generated.

III. TEST RESULTS

The majority of the tank bottom coating tested to be in good

condition. Defect signals generated in other areas were, for the

most part, caused by residual sandblast grit and blasted metal chips

not cleaned prior to coating application.

Defect signals were also generated on some of the riveted seams where the points of the rivets been exposed through the coating.

At the time of the survey, there was no evidence of corrosion activity through the coating.

INTRODUCTION

At the request of Mr. Richard T. Horan, Sr., President of Arborway Corporation, a Preliminary Site Assessment and a Phase I Investigation of the property located at 3593-3651 Washington Street in Jamaica Plain, MA has been completed by Web Engineering Associates, Inc. for the purpose of conforming with the provisions of 310 CMR 40.00, the Massachusetts Contingency Plan (MCP).

On October 28, 1988, personnel from the Emergency Response Branch of the Massachusetts Department of Environmental Protection investigated a report of a release of #2 fuel oil at the above referenced location.

A letter dated April 10, 1989 (Appendix F) from the Massachusetts Department of Environmental Protection stated that the Department had determined that a release of #2 fuel oil had occurred at this site, and that a Notice of Responsibility pursuant to M.G.L.

- 3 - Ch.21E was issued and accepted by Mr. Richard T. Horan on behalf of Arborway Corporation of 3593-3651 Washington Street, Jamaica Plain, MA 02139. Subsequently, Web Engineering Associates, Inc. of Cohasset, MA, a professional environmental consulting firm has been retained by Arborway Corporation, to satisfy the conditions outlined in the Notice of Responsibility letter.

Scone of Work To accomplish the objective of the study, the following work was performed. 1. Records Search - A records search was conducted at the Department of Environmental Protection (DEP), Northeast Region office, and various local agencies including but not limited to the Boston Fire Department, Boston Assessors'

Office, City of Boston Office of Environmental Affairs, and the Suffolk County Registry of Deeds in Boston, Massachusetts. 2. A detailed visual study of the site was conducted by Web Engineering personnel.

3. Three test borings (MW-1, MW-2, MW-3) were performed on the site and groundwater monitoring wells were installed to better characterize the environmental quality of the site. Representative soil and groundwater samples were collected. 4. Soil samples from the test borings were screened for the presence of volatile organic compounds using a photoionization detector.

Site Description and History The site is located at 3593-3651 Washington Street in Jamaica Plain, Massachusetts. A Locus Plan is presented in Appendix A.

- 4 - Appendix B contains Boston Redevelopment Authority (BRA) sheet 15N, 8E which identifies the site as two parcels of land; parcel 2617 and parcel 2617-1, and shows property boundaries, shape and abutting properties. Parcel 2617 is 81,304 square feet in size and parcel 2617-1 is 42,745 square feet in size. Both parcels feature frontage on Washington Street, and are abutted by the MBTA Orange Line at the rear of the property.

According to the Total Site Profile Report (contained in Appendix D) prepared by Environmental Research, Inc. of Allston, MA, the site has been utilized by various owners since the turn of the century for a variety of different enterprises and operations, including use as a bulk storage facility by the Gulf Oil Company. In 1981, Arborway Corporation developed the site S for a radio dispatch facility and as office and warehouse space for Contractor's Equipment and the Boston Water and Sewer Commission. Appendix C contains a site sketch which shows the size and location of these structures.

The site is currently situated in an M-1 manufacturing zone and is located in the Charles River Drainage Basin (subbasin CH-131) within a Zone C area of minimal flooding.

The DEP Water Sources Overlay indicated no potable water supplies within one mile of the site vicinity and no major hazardous material spills were referenced in the DEP records for the site or immediate area.

- 5 - Field Investigation The Field Investigation of the subject site was conducted by Edward J. Warner and Lorrimer Armstrong, Jr., of this office on September 29, 1989. The site was traversed on foot in order to identify any surficial evidence of contamination. None were found.

Test Borings Test borings are located as shown in Appendix C. The borings were situated in strategic areas where migration of contaminates might occur.

Test borings were performed by Soil Exploration Corporation of S Leominster, Massachusetts with a hollow stem auger, driven on a track mounted drill rig. Three borings were completed to depths of 27 feet in MW-1, 19 feet in MW-2, and 20 feet in MW-3, below grade into the water table.

The borings were continuously observed and logged by an on-site geologist. The exploration logs enclosed in this report are based on field logs. Disturbed but representative soil samples were obtained by using the Standard Penetration Procedure in accordance with ASTM:DI587. The test and sampling method consists of driving a standard 2 inch outside diameter split barrel sampler a distance of 18 inches into the soil with a 140 pound hammer free falling a distance of 30 inches. The blow count provides a measure of the relative density of granular soils or relative consistency of cohesive soils.

- 6 - The various types of soil and depth where soils or I characteristics of the soils changed are indicated on the boring i logs. The soils were classified using the Unified Soil Classification System included in Appendix E. The depth indicated on the logs where soil conditions changed may represent gradational changes between soil types in the field. If changes j occurred between sample intervals in the boring they were interpreted. I The Quality Assurance section in Appendix H describes the I drilling operations and the procedures used to maintain the integrity of the monitoring well.

S GeoloqV of the Site and Vicinity During the construction of monitoring wells MW-1, MW-2, and MW-3, 3 sands of differing densities and compositions ranging from loose-coarse sand with gravel to dense-fine silty sand with gravel were encountered in all three borings (Appendix E).

According to a USGS Bulletin titled t"Glacial Geology of the Mystic Lakes - Fresh Pond Area, Massachusetts", by W.R. Hansen, I 1956, the texture of the unconsolidated deposits or overburden of i the site and vicinity is mainly comprised of glacial till or drift, a geological term used to describe debris deposited by glaciers or by associated streams and lakes. Due to the confidence (hardness and tightness) of glacial till, these types 3of sediments can act as an aquitard, thus restricting groundwater movement. The exact age of these deposits is uncertain but they 3 are most likely glacial or post glacial in age. -7- According to a USGS map titled "Bedrock Geologic Map of Massachusetts" the type of bedrock underlying the overburden is comprised of Roxbury Conglomerate. Roxbury Conglomerate is a detrital sedimentary rock composed of cemented gravels, and is formed from deposits laid down directly by glacier ice. The large particles in a conglomerate are usually rock fragments and the finer particles are usually minerals derived from pre-existing rocks, especially quartz, a hard and chemically resistant mineral. Conglomerate is known to be very impermeable and can also act as an aquitard, restricting groundwater movement.

Due to the tight configuration of the soil and the known properties of the underlying bedrock (which has not been ) encountered at this site), it is unlikely there would be penetration of contaminates to a confined aquifer.

Monitoring Wells Monitoring Wells installed in each of the test borings on the site were constructed of schedule 40 #10 slotted PVC well screen placed at the bottom of each test boring. Details of the monitoring wells are included in Appendix E. On October 6, 1989 the depth of the water table in each well was measured relative to the top of the well casing. The corresponding groundwater surface elevation was calculated relative to an arbitrary benchmark. This data indicates a north to northwesterly gradient of groundwater flow. The approximate groundwater flow direction is shown in Appendix C.

- 8 - * Photoionization Screening - Appendix G i Soil samples obtained from the test borings were screened for the presence of volatile organic compounds using a Photovac Tip II j photoionization analyzer equipped with a 10.6 eV lamp probe.

Most of the light permanent gases such as ambient gases, hydrogen, helium, etc. have ionization potentials of 12 eV or I more. Volatile organic compounds generally have ionization potentials below 10 eV. The photoionization readings are shown I in the margins of the boring logs. The Photovac screening is a i measure of the total contaminants and does not distinguish I between different components. Groundwater Analyses i Groundwater samples recovered from monitoring wells MW-1, MW-2, and MW-3 were subject to laboratory analyses for EPA Method 624 volatile organic compounds and Standard Method 503 B/E petroleum hydrocarbon oil and grease (TPH). Samples were taken on I October 6, 1989. Analyses for volatile organic compounds (EPA i 624) indicated no detectable (ND) contamination in monitoring wells MW-I and MW-2. Monitoring well MW-3 showed a concentration Sof 6.0 ppb (parts per billion) for the compound trans 1,2 dichloroethene. Analyses for petroleum hydrocarbon oil and grease (TPH) indicate the presence of TPH in the following concentrations; 1.87 ppm (parts per million) in MW-1, 1.78 ppm in MW-2, and 2.02 ppm in MW-3.

On October 25, 1989 additional groundwater samples were retrieved I , and subjected to laboratory analysis for EPA Method Priority Pollutant Metals (13). The results of the tests are as follows:

Soil Analyses Soil samples obtained from the test borings of monitoring wells MW-1, MW-2, and MW-3 were subjected to laboratory analyses for EPA Methods 8240 volatile organic compounds, TPH (IR), total petroleum hydrocarbon oil and grease, total RCRA (8) metals,

EP toxicity RCRA 8 metals, and hexavalent chromium. Analyses for volatile organic compounds (8240) for monitoring wells MW-i and MW-3 indicated no detectable (ND) contamination, although the detection limits are high. (The high detected levels are the result of the presence of petroleum products.) Monitoring well MW-2 showed a concentration of 450 ppb of total xylenes from 0'-2' of depth. Analyses for total petroleum hydrocarbon oil and grease (TPH (IR)) show elevated levels of product from soil samples retrieved from all three borings. The results of the test are as follows:

Analyses for EP toxicity RCRA 8 Metals and for hexavalent chromium, show levels below EPA primary drinking water standards. All analytical laboratory results are in Appendix H.

Environmental Assessment and Opinion The environmental quality of the property located at .3593-3651 Washington Street in Jamaica Plain, Massachusetts was assessed by means of a field investigation including; subsurface exploration, photoionization of soils, limited chemical analysis of groundwater and soil samples, investigation of records at local and state agencies, and a review of the site history.

Photoionization of soil samples detected volatile organic compounds present in all three test borings above threshold limits (Appendix G). Chemical analyses of groundwater and soil samples indicated that a release of oil and hazardous materials has occurred which has impacted the near surface soils and groundwater quality. Soil and groundwater analyses also confirm the presence of toxic metals (chromium and lead) within the soil -11 - and groundwater matrices.

This investigation addresses the limited nature of the Phase I Site Assessment. A conclusive statement relative to the public health, safety, and the environment cannot be made. No free product has been observed on the site and there is no apparent immediate threat to public health, safety, or the environment. Due to the limited nature of this investigation, certain unknowns remain regarding the off site migration of the contaminates. No municipal water supply will be affected by this release.

S27 General The site is known as 3593-3645 Washington Street, and is located in Jamaica Plain, Masssachuselts. The site is identifiable on Boston Redevelopment Auhorty (BRA) Sheet 15N, 8E. According to the City Engineer's Office, the site consists of two parcels of land; I Parcel 2617 at 3593-3607 Washington Street, and Parcel 2617-1 at 3615-3645 Washington Street Parcel 2617 is 81,304 square feet in size, Parcel 2617-1 is 42,745 square feet in plan Sdirnension.

The subject property is a former fuel oil buk storage facility, and is presently occupied by the I. Boston Waterand Sewer Comrrmission and a moving company. The majorty of buildings on site were erected during the 1950s and 1960s, while the property was utilized by Gut Oil Corporation. 3593-3645 Washington Streetfeatures frontage on Washington Street. The ste is served with all public utilities, including water, sewer, and electricity.

Parcels 2617 and 2617-1 are located in an M-1 manufacturing zone. Flood Insurance Rate Maps reference the site as being located within a Zone C area of minimal flooding. 1

According to the City of Boston Assessors Office, the current Owner of Record is Arborway Corporation, Mass Corp, 3593 Washington Street, Jamaica Plain, MA 02130.

SoLth: Parcel 3219 - Formerly 3651 Washington Street

The history of this property was researched back to the turn of the twentieth century, when records of this period indicated that the subject property consisted of three Iracts of land. The portion of the property now known as 3593 Washington Street was a 56,620 square foot residential parcel owned by Mary EMorton, 3607 Washington Street was a 41,046 square foot residential parcel owned by James V. Pool, and 3645 Washington Street consisted of vacant land. Surrounding properties consisted of railroad track land and other parcels owned by the Boston Providence Railroad to the south and west, residential properties along Burnett Street to the north, and on the east side of Washington Street, the car yard for the Boston Elevated Railway Company.

By the early 1920s, the residential property at 3593 Washington Street had been taken over by the Gur Oil Company, for use as a fuel yard. Itserved as apart of their buk storage facility until the early 1970s. Records from this period indicated the construction of several buildings, used as office and warehouse space, a garage, motor repair shop, truck loading sheds, and a pump house. Inaddition, records from the mid 1930s and early 1940s indicate thata gasoline station, known as the Harry WMacPhee gas station, operated on this portion of the property crca 1935, until approximrnately 1942 (the Boston Fire Department has no record of this!). The portion of the site known as 3615 Washington Street on Parcel 2617-1 was the location of a diner from at least as early as the late 1920s until the early 1970s. During the early period, it was known as Joseph Letourneau's Lunch. Circa 1931, the diner was taken over by Priscilla Baking Lunch Company. In the early to mid 1950s, this property became Ernie's Diner. t remained Ernie's Diner until its' closure in the early 1970s.

The remainder of Parcel 2617-1 was occupied by the Metropolitan Coal Company coal yard until the late 1950s. Following the departure of Metropolitan Coal, the parcel remained vacant fora brief period of time before being taken over by Gulf Oil Company.

Gutlf Oil Comparny vacated the entire site by the early 1970s. Itremained an unoccupied parcel for a few years. In the mid 1970s, the ste was taken over as office and warehouse space for Engineering Corporation, Bafco Manufacturing Company, General Oil Burner Service, Inc., M&A on the Arborway, Emergency Roofing Company, and Universal Sections of New England (drywall systems).

In 1981, Arborway Corporation changed the occupancy of the site to office and warehouse space for contractor's equipment and the Boston Water and Sewer Commission, and performed electrical work on the property fora radio dispatch facility.

Over the years, the Burneft Street area has remained residential. Properties in the immediate site area of interestto this report include 3651 Washington Street, formerly a part of the Metropolitan Coal Company yard, 3594 Washington Street, which was the home of an auto body repair shop from the early 1940s through the early 1960s, when it was taken over by Wentco Industries, Inc., a wire and iron works. 3600 Washington Street was the home of the Weller-Sterling Farms milk distribution company from the early 1930s until the early 1960s, when it was replaced by Roger's Truck Sales. A review of the Massachusetts Water Stupply Protection Atlas overlays2, for the Boston South Quadrangle was conducted on October4, 1989.

The Drainage Basin Overlay indicated that the subject property islocated in the Charles River Drainage Basin, Subbasin CH-1 31.

According to the Aqurifer Information Overlay, the aquifer for this region is not covered by the USGS Hydrogeologic Atlas.

The Waste Sources Overlay indicated no sturface impoundments, NPDES discharge points, injection wells, confrmned or suspect hazardous waste sites, landfills, sat storage areas, or junkyards within the immediate site area.

The Water Sources Overlay indicated no potable water supples within one mile of the subject site.

DEP Hazardous Materials Spills, Miscellaneous Oil Spills, and Ste Assessment Files for Boston, Jamaica Plain, and West Roxbtxy were requested for review at the Northeast Region Offices of the DEP inWoburn on October 4, 1989. Prior to requesting these files, the DEP Sites Database, an in house inventory of site assessments performed on properties within the northeast region, was consuted for properties within the general site area. Because there were no properties referenced within the general site area, Site Assessment Files were not reviewed. The files requested were pulled from the files and presented to KF Reddy, Inc. by DEP personnel. According to Inspector Hendricks, there are no underground storage tank permis on file for the subject property, and no records of UST installations or excavations.

Mr. John Shea, Principal Heath Inspector for the City of Boston was interviewed by Kay Fitzgerald Reddy of KF Reddy, Inc. on October 5, 1989. According to Mr. Shea, the OEA does not have any records of incidents on file connected with the subjectproperty.

NMr. Shea's opinion was that the prior usage of the subject property would suggest that petroleum releases could have to occurred on site.

NtMr. Shea reported that the OEA did have records on file regarding the de-leading of the orange line over Washington Street last year. According to Mr. Shea, lead paint flakes were knocked down into the street during the dismantling of the orange line, which could have resulted in some soil contamination in the area. Mr. Shea was of the opinion that this should have been cleaned up by now.

Mr. Shea also reported problems with illegal dumping of construction waste on vacant lots within the general site area in the past. Dear Mr. Horan:

The Emergency Response Branch of this Office has received and reviewed information/data relative to contaminant conditions at the above referenced location. Such information/data was collected subsequent to a determination that a release of #2 fuel oil had occurred at this site. This incident was investigated by personnel from the Emergency Response Branch of this Office on October 28, 1988. At that time, you were issued a Notice of Responsibility pursuant to M.G.L. c21E and you accepted such responsibility for the release.

This letter is intended to notify you in writing that: (1) contaminant conditions at this location render the site an "LTBI" (Location To Be Investigated) pursuant to the Massachusetts Contingency Plan (MCP), 310 CMR 40.520(1); (2) a decision on the final disposition of the site will be made by the Site Management Branch of this Office after all the pertinent data and response actions have been evaluated.

Pending final determination of the site disposition, groundwater and subsurface soils cannot be considered "clean". Therefore, no excavation and removal of soils or pumping of groundwater from the site should occur without prior DEQE notification and approval.

Site Disposition

The Emergency Response Branch of the Department has concluded that at this time there is no need for any further emergency response actions at the site. Be advised that all further investigative and/or remedial response measures at this site must conform with the provisions of 310 CNR 40.00, the Massachusetts Contingency Plan (MCP). As a first step in this process, you are advised to contract with a professional environmental consultant firm.to conduct a Preliminary Assessment (40.541) and a Limited Site Investigation (40.543) as defined in the ICP, and submit the results of your findings to the Site Management Branch.

Subsequent to the completion of the above site activities, no further investigative and/or remedial response actions may be initiated at this site without specific approval from the Site Management Branch unless a "Waiver" application is filed and approved by the Department pursuant to the provisions of 310 c:R 40.537.

Finally, be advised that 310 CMR 40.520 and recent statutory amendments to M.G.L. Chapter 21E compel the Department to publish the addresses of all sites and locations of confirmed or suspected releases of oil/hazardous materials to the environment. Contingent upon an additional review of available incident/site information, this location may be included on a future list publication.

QUALITY ASSURANCE The laboratory equipment used to perform the analysis of the water has a sensitivity of about one part per billion for the organic compounds of interest in this study. It is imperative that the drilling and sampling equipment, therefore, be kept scrupulously clean relative to these compounds. The procedures described below have been designed to reduce any outside contamination to insignificant levels.

Drilling Operations To reduce the potential of bringing outside contamination to the site or cross-contaminating a monitoring well, the boring equipment is steam cleaned and inspected before use. Also as the drilling rig is moved from one boring to the next, all equipment, i.e. auger, bits, split spoon, retrieval rod and auger head are manually cleaned of all soil and then steam cleaned to assure cleanliness and removal of any contaminants which may be present in the previous boring. In addition, all roadway boxes are steam cleaned prior to installation. No lubricate, (i.e. petroleum based grease) is used on any of the equipment that enters the bore hole. When soil samples are taken via the Standard Penetration Test with the split spoon sampler, the sampler is cleaned between each sample. The sampler is first washed in distilled water, then rinsed in distilled in glass methanol, then rinsed again in distilled water. All well screen installed in the monitoring wells is of the screw-joint variety and no glue is used on any portion of the well. The annulus of the well is filled with silica sand to one foot above the top of the screen. Above the silica sand a seal of bentonite pellets approximately 1.5 feet thick is placed. The seal is set by pouring distilled water onto the bentonite pellets. Above the seal the well is backfilled with the material from the drill hole unless this material is grossly contaminated in which case a clean backfill material is used. All monitoring wells are installed by a certified well driller and the work is supervised by a geologist experienced in well construction. Immediately after installation, each monitoring well is "developed" with a bottom entry bailer by gently surging the well and bailing the water and silt sufficiently to remove several volumes of water. The bailer is cleaned by the same procedure described under groundwater sampling prior to insertion to the well. Groundwater Sampling Groundwater samples are obtained by means of a bottom entry bailer suspended by a cotton cord. Prior to sampling, each well is pumped sufficiently to remove a minimum of four volumes of water. A four-step cleaning procedure of the bailer precedes each bailing and sampling. First the cotton cord is cut from the bailer. Then, the bailer is washed in distilled water to remove any mud or silt from the previous sampling. It is then washed with "distilled in glass" methanol. Finally it is rinsed again with distilled water. The cotton cord is replaced with new clean cord prior to each bailing sequence. This cleaning procedure is employed prior to the bailing and sampling for each monitoring well. All of these precautions reduce cross-contamination between samples to insignificant levels. r'.1I
