Alberta Health, Continuing Care Branch For General Reference Purposes Only March 31, 2016 List of Publicly Funded Designated Supportive Living Accommodations and Long-Term Care Facilities This document is a list of publicly funded designated supportive living (DSL) accommodations and long-term care (LTC) facilities in Alberta as of March 31, 2016. The list is broken down by: Alberta Health Services (AHS) Zone (i.e., South, Calgary, Central, Edmonton and North); Location (Alphabetically); Facility Name, which is hyperlinked to the Accommodation Standards Licensing and Public Reporting Website; Facility Address, including phone number; Operator Name; Operator Type (i.e., AHS, Private or Voluntary); Level of DSL accommodation (DSL 3, DSL 4 or DSL 4-D); and, Type of LTC Facility (Auxiliary Hospital or Nursing Home). DSL Accommodations In DSL accommodations, a higher level of care is provided than what is provided in other supportive living settings. DSL settings have staff on-site to provide personal care and health support services 24 hours a day. These staff may include Health Care Aides, Licensed Practical Nurses, on-call Registered Nurses and staff providing therapeutic services, such as Physical and Occupational Therapists. The levels of DSL accommodations include (in order of lowest to higher care need provided): DSL 3: This community-based living option includes 24-hour on-site (scheduled and unscheduled) personal care and support services which are provided by Health Care Aides. DSL 4: This DSL level provides 24-hour on-site (scheduled and unscheduled) professional health and personal care and support, provided by Licensed Practical Nurses and Health Care Aides. Professional health services include Registered Nurse services with 24-hour on call availability, case management, assessment and other consultative services. DSL 4-D: This setting provides residents with dementia a home-like design with small groupings of private bedrooms and Source: AHS Website associated spaces in a secured therapeutic environment. Read more on DSL 4-D at http://www.albertahealthservices.ca/cc/Page13338.aspx. © 2017 Government of Alberta 1 Alberta Health, Continuing Care Branch For General Reference Purposes Only March 31, 2016 LTC Facilities In LTC facilities, care is provided for people with medically complex, unpredictable health needs who are unable to remain at home or in a supportive living setting. Registered Nurse services are available 24 hours a day. Most personal care services are provided by Health Care Aides and Licensed Practical Nurses who are all available 24 hours a day, 7 days a week. Individuals also have access to therapeutic services, such as Physical and Occupational Therapists, and Physician services. While there are two types of LTC facilities (i.e., auxiliary hospitals and nursing homes), the care provided in each type does not differ. Operator Type While the health care services in DSL and LTC facilities are publicly funded, they can be operated by: AHS, which includes CapitalCare and Carewest; Private operators, which includes for-profit organizations; and, Voluntary operators, which includes non-profit organizations such as faith-based and charitable organizations. Admission to DSL or LTC To gain admission into either DSL or LTC, individuals must be assessed by an AHS health care professional to determine their unmet health needs. An assessment can be arranged by contacting the respective AHS Zone’s Community Care Access office. Please note that Pincher Creek Health Centre is not currently listed on the Accommodation Standards Licensing and Public Reporting website. This list is current as of March 31, 2016 and as such may not be complete/accurate when it is read. This list is expected to be updated once a year. Questions regarding specific facilities appearing on this list should be directed to AHS. Some facilities may be known by an alternative name.
