A Case of Human Thelaziasis from Himachal Pradesh
Indian Journal of Medical Microbiology, (2006) 24 (1):67-9 Case Report A CASE OF HUMAN THELAZIASIS FROM HIMACHAL PRADESH *A Sharma, M Pandey, V Sharma, A Kanga, ML Gupta Abstract Small, chalky-white, threadlike, motile worms were isolated from the conjunctival sac of a 32 year-old woman residing in the Himalaya mountains. They were identified as both male and female worms of Thelazia callipaeda. To the best of our knowledge, this is the second case report of human thelaziasis from India. Key words: Human thelaziasis, Oriental eyeworm Thelazia callipaeda was first reported by Railliet and Case Report Henry in 1910 from a Chinese dog and it is also known as Oriental eyeworm. The first human case was reported by During autumn of 2004 a 32 year old woman from a rural Stucky in 1917, who extracted four worms from the eye of a mountainous region presented with the complaint of small, coolie in Peiping, China. Since then a number of species of white, threadlike worms in her right eye. She was suffering eyeworm have been reported in certain animals and birds from with foreign body sensation and itching in her right eye for a different countries of the world.1 The two important species few days. Zoonotic Nematodes of Wild Carnivores

However, human activities and politics (e.g., fragmentation of the environment, land use, recycling in urban settings) have consistently favoured the encroachment of urban areas upon wild environments, ultimately causing alteration of many ecosystems with changes in the composition of the wild fauna and destruction of boundaries between domestic and wild environments. Therefore, the exchange of parasites from wild to domestic carnivores and vice versa have enhanced the public health relevance of wild carnivores and their potential impact in the epidemiology of many zoonotic parasitic diseases. The risk of transmission of zoonotic nematodes from wild carnivores to humans via food, water and soil (e.g., genera Ancylostoma, Baylisascaris, Capillaria, Uncinaria, Strongyloides, Toxocara, Trichinella) or arthropod vectors (e.g., genera Dirofilaria spp., Onchocerca spp., Thelazia spp.) and the emergence, re-emergence or the decreasing trend of selected infections is herein discussed. In addition, the reasons for limited scientific information about some parasites of zoonotic concern have been examined. A correct compromise between conservation of wild carnivores and risk of introduction and spreading of parasites of public health concern is discussed in order to adequately manage the risk of zoonotic nematodes of wild carnivores in line with the ’One Health’ approach. The current report addresses the finding of ocular filariosis in a domestic adult hen, raised in a backyard, in a community near La Silla River, south Nuevo Leon, in Mexico. The hen was submit- ted to a veterinary practice due to weakness and diarrhea. During necropsy, three nematodes were found underneath the nictitating membranes in both of the bird’s eyes. Because of the absence of any phylogenetic studies using molecular methods in this nematode group, an attempt to evaluate affinities among these parasites and their zoogeography on the basis of morphological features and host-parasite relationships has been carried out. Only true definitive hosts should be considered for such evaluation. It appears that, during the evolution of Rhabdochona spp., there occur an increase in the num- ber of anterior prostomal teeth, loss of lateral alae, the right spicule attains a boat-like shape and a dorsal barb develops on its distal tip, and the distal tip of the left spicule attains gradually a more complex structure; the eggs with a smooth surface are considered to be more primitive. Nematodes of this genus most probably originated in the region of present southern Asia at the beginning of Tertiary. Closest to the initial type appear to be members of the subgenus Globochona Moravec, 1972, from which lineages leading to the subgenera Globochonoides Moravec, 1975, Rhabdochona Railliet, 1916 and Sinonema Moravec, 1975 can be derived. -

Importantly, zoonotic helminths that affect human eyes (HIE) may cause blindness with severe socio-economic consequences to human communities. These infections include nematodes, cestodes and trematodes, which may be transmitted by vectors (dirofilariasis, onchocerciasis, thelaziasis), food consumption (sparganosis, trichinellosis) and those acquired indirectly from the environment (ascariasis, echinococcosis, fascioliasis). Adult and/or larval stages of HIE may localize into human ocular tissues externally (i.e., lachrymal glands, eyelids, conjunctival sacs) or into the ocular globe (i.e., intravitreous retina, anterior and or posterior chamber) causing symptoms due to the parasitic localization in the eyes or to the immune reaction they elicit in the host. Unfortunately, data on HIE are scant and mostly limited to case reports from different countries. The biology and epidemiology of the most frequently reported HIE are discussed as well as clinical description of the diseases, diagnostic considerations and video clips on their presentation and surgical treatment. The oriental eyeworm, Thelazia callipaeda, infects a range of mammalian definitive hosts, including canids, felids and humans. Although this zoonotic parasite is of socio-economic significance in Asian countries, its genetics, epidemiology and biology are poorly understood. Mitochondrial (mt) DNA is known to provide useful genetic markers to underpin fundamental investigations, but no mt genome had been characterized for any members of the family Thelaziidae. In the present study, we sequenced and characterized the mt genome of T. callipaeda. This AT-rich (74.6%) mt genome (13,668 bp) is circular and contains 12 protein-coding genes, 22 transfer RNA genes and two ribosomal RNA genes, but lacks an atp8 gene. -
