2 Water Supply Schemes in Constituency NA-80. 122.000

Pre-qualification of contacting firms will be governed / carried out under P&D and Chief Engineer (South) PHED Department Punjab guidelines.

Each firm / contractor is required to provide the following information / documents with application for pre-qualification up to 18.07.2017 at 03:00-PM in the office of Executive Engineer, Public Health Engg: Division Faisalabad.

1. Interested firms should obtain pre-qualification Proformas / a set of pre-qualification documents and self-marking sheet which are immediately available with this office after the date of publication up to 18.07.2017 for Rs. 5000/- for the each scheme as documentation charges / processing fee, in shape of CDR / Pay Order (non-refundable) Pre Qualification Notice in the name of undersigned and submit dully filled and signed / stamped along with the application and requisite documents. 2. All the proformas should be filled / sealed / stamped by the owner / Managing Director of the firm and supported by the attested document, and submitted to this office along with application / documents within due date. 3. Details of any Arbitration / Litigation or similar proceeding against any Government / Semi Government Department showing extent and result. 4. Undertaking on judicial paper that his firm was never black listed by any of the Government or Semi Government organization. 5 The applicant will present the employed graduate Engineer (as allowed / considered by Pakistan Engineering Council at the time of Registration of firm / Contractor) before the pre-qualification committee on 21.07.2017. 6. The contractor / firm will, not be considered for pre-qualification, how have already participated in tenders but have not good repute in quality of work or not competed the scheme well in time.

7. Year of Establishment supported by certificate from the Registrar of firms.

8. List of complete permanent Business Management, Finance Management and Engineering / Technical Staff with their complete Bio-data and proof of stay with the firm.

9. List of equipment with its No. Make / Model condition and location along with the proof of ownership.

10. Detail of similar projects completed by the contractor / firm giving location approximate cost, time taken for completion duly supported with a certificate for client Department.

11. List of project handle during last three years giving their location, approximate cost, time allowed / taken duly supported with a certificate from client Department.

12. Detail of works in hand indicating name of Client Department, consultants, scope of works completed / payment received upto date by the contractor/ firm.

13. Authentic proof of their financial position such as Bank Statement of previous one year.

14. Total assets work capital and liabilities duly certificate.

15. Income Tax registration certificate. RULES AND REGULATIONS. 1 Any fake documents and concealment about the information / details mentioned above, if found at any stage, will result in disqualification and blacklisting of the firm. 2 In case of incomplete information, pre-qualification committee will reject the application and it will not be considered for pre-qualification. Pre Qualification Notice 3 No Court proceeding against the Pre-qualification will be allowed and decision of the committee will be final. 4 Applicant firms are required to give under taking that they have carefully studied the pre- qualification notice / documents and will abide by the rules and regulation. 5 Each page of documents provided dully numbered, signed and attested by the applicant, documents attached to Sr, No. as mentioned above and indexed with page number be provided. 6 Information to firm regarding pre-qualification will be given after approval from the committee constituted by the Chief Engineer (South) Punjab Public Health Engg: Department Lahore, the competent authority. 7 Performance / Reputation certificate from employer is necessary for pre-qualification.
