BREO ELLIPTA 100/25 is also indicated to reduce exacerbations of COPD in patients with a history of exacerbations. BREO ELLIPTA 100/25 once daily is the only strength indicated for the treatment of COPD. Important Limitation of Use BREO ELLIPTA is NOT indicated for the relief of acute bronchospasm 1.2 Treatment of Asthma BREO ELLIPTA is indicated for the once-daily treatment of asthma in patients aged 18 years and older. BREO ELLIPTA should be used for patients not adequately controlled on a long-term asthma control medication such as an inhaled corticosteroid (ICS) or whose disease warrants initiation of treatment with both an ICS and long-acting beta2-adrenergic agonist (LABA). Important Limitation of Use BREO ELLIPTA is NOT indicated for the relief of acute bronchospasm. 2 DOSAGE AND ADMINISTRATION BREO ELLIPTA should be administered as 1 inhalation once daily by the orally inhaled route only. BREO ELLIPTA should be used at the same time every day. Do not use BREO ELLIPTA more than 1 time every 24 hours. After inhalation, the patient should rinse his/her mouth with water without swallowing to help reduce the risk of oropharyngeal candidiasis. QUALITATIVE AND QUANTITATIVE COMPOSITION Each delivered dose (the dose leaving the mouthpiece of the inhaler) contains 92 micrograms fluticasone furoate, 55 micrograms umeclidinium (equivalent to 65 micrograms umeclidinium [as bromide]) and 22 micrograms vilanterol (as trifenatate). This corresponds to a pre-dispensed dose of 100 micrograms fluticasone furoate, 62.5 micrograms umeclidinium (equivalent to 74.2 micrograms umeclidinium bromide) and 25 micrograms vilanterol (as trifenatate). TRELEGY ELLIPTA should not be used for the initiation of COPD treatment. 4.2. Dose and method of administration Patients can be changed from their existing inhalers to TRELEGY ELLIPTA at the next dose. However it is important that patients do not take other LABA or LAMA or ICS while taking TRELEGY ELLIPTA. A stepwise approach to the management of COPD is recommended, including the cessation of smoking and a pulmonary rehabilitation program.

    Therapeutic Class Overview Respiratory Corticosteroid/Long-Acting β-Agonists Combinations Therapeutic Class Overview/Summary: The combination inhaled corticosteroid (ICS)/long-acting β2- agonist (LABA) products include Advair® (fluticasone propionate/salmeterol), Breo Ellipta® (fluticasone furoate/vilanterol), Dulera® (mometasone/formoterol) and Symbicort® (budesonide/formoterol), with fluticasone furoate/vilanterol being the most recent agent to be approved by the Food and Drug Administration (FDA). Fluticasone propionate/salmeterol, mometasone/formoterol, budesonide/formoterol and fluticasone furoate/vilanterol are approved for the treatment of asthma; however, only fluticasone propionate/salmeterol, fluticasone furoate/vilanterol and budesonide/formoterol have been approved for the treatment of chronic obstructive pulmonary disease (COPD). The ICSs exert their anti-inflammatory effect by binding to the glucocorticoid receptors with a subsequent activation of genes involved in anti- inflammatory processes, as well as via the inhibition of pro-inflammatory genes involved in the asthmatic response. The LABAs have selective action on β2 receptors which stimulate adenyl cyclase, thereby increasing intracellular cyclic adenosine monophosphate level, and subsequently relaxing bronchial smooth muscles.
